Electrospray ionization (ESI) mass spectrometry (MS) in combination with liquid chromatography (LC) is an excellent tool for the identification of drug metabolites. Utilizing this hyphenated technique in combination with proper sample pretreatment, the metabolic pathways of the analgesic drug ketobemidone were investigated in human urine and rat microdialysate from blood and brain. Two novel phase I metabolites (ketobemidone N-oxide and meta-hydroxymethoxyketobemidone) and three novel phase II metabolites (glucuronic acid conjugates of ketobemidone, norketobemidone and hydroxymethoxyketobemidone) were identified in human urine. Further, norketobemidone and ketobemidone N-oxide were identified in rat microdialysate from brain after regional distribution of ketobemidone in striatum. This indicates that the brain itself has the possibility to metabolize ketobemidone. Synthetic ketobemidone metabolites were used for comparison of retention times and tandem MS spectra with the possible metabolites ...

Injection: 10-, 25-, 50-, 100 mg/mL. Dose: Oral, intramuscular, or subcutaneous: 50-100 mg/dose every 3-4 hours (50-75 mg of meperidine is roughly equivalent to 10 mg morphine). The oral dose is less potent than the intramuscular dose.. Indications: Pain not controlled by nonopioid drugs. Morphine is generally preferred. Do not use to treat chronic pain.. Mechanism of Action: Binds to opioid receptors in the CNS. Contraindications: Hypersensitivity to meperidine, substance abuse, patients receiving MAOIs in the previous 14 days, renal failure. Precautions: Use caution in patients with hypersensitivity to other opioids, seizure disorder, respiratory disease, renal or hepatic impairment. Decrease the dose if there is hepatic or renal impairment. The metabolite normeperidine accumulates in patients with impaired renal function and causes CNS stimulation, anxiety, tremors and seizures. Meperidine is not suitable for long-term use. Use the lowest dose necessary to control pain. Escalate dose only ...

Meperidine is a synthetic opiate agonist belonging to the phenylpiperidine class. Meperidine may produce less smooth muscle spasm, constipation, and depression of the cough reflex than equivalent doses of morphine. The onset of action is lightly more rapid than with morphine, and the duration of action is slightly shorter. The chemical structure of meperidine is similar to local anesthetics. Meperidine is recommended for relief of moderate to severe acute pain and has the unique ability to interrupt postoperative shivering and shaking chills induced by amphotericin B. Meperidine has also been used for intravenous regional anesthesia, peripheral nerve blocks and intraarticular, epidural and spinal analgesia. Meperidine is considered a second-line agent for the treatment of acute pain ...

The effect of the potent analgesic, fentanyl, on minute volume, tidal volume and respiratory frequency at a controlled alveolar carbon dioxide tension of 46 mm Hg in six normal adult men was determined at intervals over a 4-hr period. Fentanyl was administered i.m. in two doses (0.1 mg and 0.2 mg) and the effects were compared with those of i.m. physiologic saline (2.0 ml) and meperidine hydrochloride (75 mg), all on the basis of 70 kg of body weight. In the doses employed (fentanyl, 0.1 mg; meperidine, 75 mg), the magnitude of maximal depression of the two narcotics is similar. Thus, in equianalgesic doses (fentanyl, 0.1 mg; meperidine, 65 mg) maximal ventilatory depression with fentanyl would be slightly greater. However, the onset and peak depression of minute ventilation, as well as the recovery toward control levels, occurs more rapidly with fentanyl. Thus, fentanyl may be a useful narcotic agent when rapid onset and recovery from the effects of the analgesic are desired.. ...
Mice became tolerant within a few days to the analgesic and exciting effects of narcotics as a result of drinking dilute evaporated milk containing dihydromorphinone HCl (Dilaudid). Tolerant mice could be maintained in good physical condition for 1 month or more while drinking between 50 and 70 mg per kg of dihydro-morphinone HCl daily.. When tolerant mice were withdrawn they lost as much as 15% of their body weight over-night, indicating that physical dependence was established.. The extent of tolerance increased with increasing daily intake of dihydromorphinone and with the length of time, up to about 7 days, that animals were maintained on one given dose.. Mice made tolerant by drinking dihydromorphinone showed decreased excitability. This was manifested by decreased spontaneous activity and a decreased response to the exciting effects of morphine, meperidine, codeine, cocaine, and, to a lesser extent, d-amphetamine. There was tolerance to the analgesic effects of morphine, meperidine and ...

Opiates, sometimes referred to as narcotics, are a group of drugs which are used medically to relieve pain, but also have a high potential for abuse. Some opiates come from a resin taken from the seed pod of the Asian poppy. This group of drugs includes opium, morphine, heroin, and codeine. Other opiates, such as meperidine (Demerol), are synthesized or manufactured. Opium appears as dark brown chunks or as a powder and is usually smoked or eaten. Heroin can be a white or brownish powder which is usually dissolved in water and then injected. Most street preparations of heroin are diluted, or cut, with other substances such as sugar or quinine. Other opiates come in a variety of forms including capsules, tablets, syrups, solutions, and suppositories. Heroin (junk, smack) accounts for 90 percent of the opiate abuse in the United States. Sometimes opiates with legal medicinal uses also are abused. They include morphine, meperidine, paregoric (which contains opium), and cough syrups that ...

BACKGROUND Pain management experts have recommended a diminished role for pethidine because of lack of superiority for any indication and greater potential for adverse effects compared with morphine. OBJECTIVES To evaluate the impact of a multifaceted education initiative on prescribing patterns and to minimize pethidine use in the ED. METHODS This was a pre- and post-intervention study. The intervention was an education initiative, introduced between December 2003 and January 2004. It comprised ED pharmacist in-services for medical and nursing staff, championing by senior medical staff (ED consultants, surgical units, Drug and Therapeutics Committee) and concurrent prescriber feedback by ED pharmacists. The pre-intervention prescribing practices guided the development of in-services and the educational programme. The intervention was evaluated by a medical record review of analgesic prescribing in March-April of both 2003 (pre-intervention period) and 2004 (post-intervention period). Outcome
results, the differences between the three groups proved to be statistically significant.. When compared to placebo, metamizol showed a better response (P= 0.03), but meperidine showed to ...
