A system for electronic medication administration records comprising a cart station located at a skilled nursing facility, administration web pages, and a healthcare server not located at the skilled nursing facility. The cart station utilizes a plan builder algorithm to build a plan for each patient based on data contained in the cart station database. The plan comprises a list of the patients procedures for a given day and information indicating whether such procedures have been completed. The cart station utilizes a plan runner algorithm to maintain and update the plan for each patient. The administration web pages allow an administrator to manage utilization of the system. The healthcare server and the cart station utilize a synchronization algorithm to maintain accurate and timely data for all of the skilled nursing facilities that use the system. A method of maintaining electronic medication administration records using the system described above.

Stage 1 Major ancillary clinical systems are installed (i.e., pharmacy, laborat ory, radiology). Stage 2 Major ancillary clinical systems feed data to a clinical data repository (CDR) that provides physician access for retrieving and reviewing results. The CDR contains a controlled medical vocabulary, and the clinical decision suppor t/ rules engine (CDS) for rudimentary conflict checking. Information from document imaging systems may be linked to the CDR at this stage. The hospital is health information exchange (HIE) capable at this stage and can share whatever information it has in the CDR with other patient care stakeholders. Session presented on Sunday, July 24, 2016: Purpose: To assess senior baccalaureate nursing students self-rated competence of safe medication administration and their actual competence to do so by using simulation scenario. Methods: This is a cross-sectional, descriptive study. A self-rated questionnaire of safe medication administration was developed. Participants were asked to rate levels of their knowledge, confidence, competence, and experience in safe medication administration by 5-point Likert scale (0 is none, and 5 is always) before the simulation scenario. The higher points the participants give, the higher levels of confidence the participants think that they have. A simulation scenario was developed to assess participants competence in applying the six rights (right patient, right medication, right dose, right time, right route, and right reason) at the medication administration phase. Participants were expected to identify nine errors, including: 1). Use two-identifiers to identify ...
Results Analysis of variance identified that violation reports were highest for emergency situations, rather than for routine operations, highest by HOT unit nurses, followed by PICU nurses and then Med/Surg unit nurses, and highest during patient identification checking, followed by matching a medication to a medication administration record, and then documenting an administration. There was also a significant three-way interaction among violation situation, step in the process, and unit. ...

As appropriate, list associated brand names in a requisite field using all upper case letters (e,g, LASIX) to differentiate them from generic names, ensuring that all product labels match the way the drug appears on the original order and medication administration record. Trademark symbols should not be used [e.g., TM or (r)]. For medications that contain multiple ingredients, use the brand name to provide clarification and reduce the risk of error ...
Updated 13 December 2016. Out of the 94 incidents reported to the palliative team, we saw eight were in relation to low staffing levels. We noted some resulted in palliative patients not being attended to or observed as often as they required and "Care was compromised". Staff on surgical wards told us they would struggle to ensure end of life patients received the care that they needed. However, they told us that the palliative team were aware of their pressures and were very supportive.. The palliative team were not solely responsible for end of life patients but they supported the medical and nursing teams in providing specialist advice.. We reviewed 20 medication administration records across the wards and units inspected and found these were consistently well completed. Although improvement was needed to ensure that controlled medicines were safely and appropriately administered.. We reviewed medical and nursing paper care records and Do Not Attempt Cardio Pulmonary Resuscitation (DNACPR) ...
Of 14,041 medication administrations and 3,082 order transcriptions reviewed, approximately 30, 52, and 17% were observed in medical, surgical, and intensive care units, respectively. There was a 41.4% relative reduction in nontiming errors in units that used the bar-code eMAR (P , 0.001). The most common types of errors in units that did versus those that did not use the system included errors in oral versus nasogastric tube administration (4.4% vs . 3.6%), administration documentation (2.9 vs . 0.6%), dose (2.0 vs . 1.1%), and wrong medication (1.0 vs . 0.4%). Errors occurred more frequently in surgical and intensive care units compared with medical units. There was a 50.8% relative reduction in the rate of potential adverse drug events (other than those associated with timing errors) in units that used the bar-code eMAR (P , 0.001). The rate of timing errors in medication administration was reduced by 27.3% (P , 0.001). Transcription errors occurred at a rate of 6.1% on units that did not use ...

For all medications, you should only give the dosage described in the prescription label or other instructions. Dosage is carefully determined by your doctor and can be affected by your age, weight, kidney and liver health, and other health conditions.. For some medications, dosage must be determined by trial and error. For these drugs, your healthcare provider would need to monitor you when you first start treatment. For instance, if your doctor prescribes thyroid medications or blood thinners, you would likely need to have several blood tests over time to show if the dosage is too high or too low. The results from these tests would help your doctor adjust your dosage until they find the one thats right for you. To be effective, many medications need to reach a certain level in your bloodstream. They need to be given at specific times, such as every morning, to keep that amount of drug in your system. Taking a dose too soon could lead to drug levels that are too high, and missing a dose or ...

At GDC 07 there was a talk about behavior trees. Its the type of AI that Halo 2 uses, as described by described by Damian Isla at GDC 05. The first part of this years talk was by Lauren McHugh whos working on the AI for Spore - EAs delayed mega-hit for 2009. (You can get the audio without slides for a whole $8 from the store.) Anyway, Lauren mentioned that the programmers are responsible for creating the behavior trees in C++. While its not ideal for turn-around times, its certainly a very powerful approach. In fact, Ive already implemented this in Game::AI++ as its a pretty good starting point for creating game behaviors - even if it doesnt involve the designers directly. Macros or Templates On Spore, it seems they make heavy use of macros to do this. These days, however, I try my best to avoid them, so I went for a template-based implementation to see how it would turn out. The results are quite encouraging! The idea is to get the C++ compiler to help build valid trees as much

Medication prescribing for 158 neonates was studied. The rate of nonintercepted medication errors during the NOE period was 40% lower than during the POE period (rate ratio 0.60; 95% confidence interval [CI] .50, .71; P less than .001). During the POE period, 80% of nonintercepted errors occurred at the prescription stage, while during the NOE period, 60% of nonintercepted errors occurred in that stage. Prescription errors decreased from 10.3% during the POE period to 4.6% during the NOE period (P less than .001), and the number of warnings with which physicians complied increased from 44% to 68% respectively (P less than .001). Meanwhile, transcription errors showed a nonsignificant increase from the POE period to the NOE period. The median error per patient was reduced from 2 during the POE period to 0 during the NOE period (P= .005). Underdose and curtailed and prolonged interval errors were significantly reduced from the POE period to the NOE period. The rate of nonintercepted overdose ...

A method and system for intelligent text entry on a keypad. First, a user selects a key on a keypad. Then, an application predicts which character of those corresponding to that key is intended by the user. The predicted character is then presented to the user for confirmation. If the user confirms the selection, the character is stored. If the user rejects the character, the application presents a new character to the user until a character is finally confirmed. It is only when the predicted character is not the intended character that the user is required to iterate through character choices.
