The study was conducted over a five-month period in the Intensive Care Unit (ICU), Royal Childrens Hospital, Melbourne, and was approved by the hospital ethics committee. The ICU is an 18-bed multidisciplinary unit treating critically ill infants and children. During the study, 807 patients were admitted: 37% had undergone cardiac surgery, 27% had respiratory illnesses, 9% had general medical illnesses, 8% had newborn medical and surgical illnesses, 8% had trauma, and 8% had undergone neurosurgery and 3% general surgery. Mechanical ventilation was provided for 84% of patients. Most had invasive monitoring or therapeutic devices inserted into peripheral arteries, peripheral or central veins, the thoracic cavity, the urinary bladder or the extradural space. The mortality rate was 5.9%. The subjects of the study were any medical officers who made hand contact with any patient in the ICU. The ICU is newly constructed and comprises four rooms adjacent to each other along a corridor. Windows enable ...

Introduction: Implementing foundation and specialty training programmes within emergency medicine raised concerns about the potential work productivity and effectiveness of new junior doctors. Between August 2006 and July 2007 senior house officers (SHO) on 6-month posts and foundation year 2 (FY2) doctors on 4-month placements worked on the same roster, rotating between the emergency department at Ninewells Hospital, a university teaching hospital in Dundee, and a smaller affiliated unit at Perth Royal Infirmary. To compare the efficiency and productivity of both groups of junior medical staff.. Methods: A prospective observational study was performed at both departments using the number of patients seen per hour as an indicator of productivity. These rates were calculated using information gathered from a computerised patient record and management system. Analysis was performed using unpaired t tests.. Results: Both groups demonstrated a significant rise in performance between the first and ...
Aims: To explore the depth of knowledge and understanding of blood products and their indications for use among junior medical staff in a teaching hospital paediatric department that contains a haematology/oncology unit but without dedicated juniors out of hours. Regular queries regarding blood product administration to consultants covering the unit precipitated this study.. Methods: Questionnaires were distributed to junior medical staff in the paediatric department. This covered basic knowledge of transfusion medicine, the use of special blood products, the use of departmental guidelines and personal confidence in ordering and prescribing.. Results: 29 questionnaires were distributed with 21 returns, from 15 specialist registrars and six senior house officers. Three doctors (14%) prescribed blood products on a weekly basis and 16 (76%) prescribed monthly. Only 85% of respondents had an accurate understanding of the ABO system and knowledge about blood product storage and administration was ...
Research on the quality of US resident physician performance levels has often been limited by lack of a comparison group or strict focus on specific diseases and geographical areas. In order to gain insight on differences in quality of care provided by resident physicians versus staff physicians, Boston Medical Center and Palo Alto Medical Foundation researchers investigated performance of physicians in 33,900 hospital-based outpatient visits throughout the US.
Background: Performing a lumbar puncture in an unwell child can cause anxiety in both the parent and the junior doctor. There is increasing evidence of post-lumbar-puncture complications in this age group.. Aims: To improve the documentation, consent for and technical performance of paediatric lumbar punctures to 100% of the required standard within 3 months.. Setting: The paediatric emergency department of a the Royal North Shore Hospital (University of Sydney, Sydney, Australia).. Participants: Paediatric emergency staff, including residents, registrars and consultants.. Methods: Medical records of 40 consecutive children who had undergone a lumbar puncture in the 6 months before the introduction of the lumbar-puncture proforma were reviewed. After introduction of the proforma, the records of 25 consecutive patients were reviewed to assess changes in the outcome measures. Before introduction of the proforma, junior medical staff were instructed in the procedure using specialised lumbar ...
Numerous randomised trials have shown that thrombolytic treatment reduces mortality from acute myocardial infarction irrespective of the patients age, sex, blood pressure, and previous history of myocardial infarction or diabetes.1 Maximum benefit, however, is seen in those patients treated within 4-6 hours of their symptoms starting. Patients do not always seek medical help soon enough, and this accounts for much of the delay in receiving thrombolytic treatment, but important delays also occur in hospital. These are not related to the route by which the patient is admitted to hospital and vary widely between hospitals.2. Although accident and emergency departments are in an important position to minimise any delay in giving thrombolytic treatment, a recent questionnaire study of junior hospital doctors in Scotland showed that thrombolysis is rarely given in accident and emergency departments there.3 We surveyed consultants in accident and emergency departments … ...

I am also very worried by the rise in the number of cases of pulmonary tuberculosis. There was a 22 per cent. increase in the 1990s, which is horrendous when one considers that when I was a junior hospital doctor we thought the disease was dying out. It was also very disturbing to find out a couple of years ago that we had run out of vaccine. As the House probably knows, that became a saga because only one manufacturer made TB vaccine and it has taken some time to put the situation right. There are questions to be asked about that, including whether all those who should be vaccinated against TB have indeed received the vaccine. I am particularly concerned about 11 to 14-year-old schoolchildren, many of whom must have missed out when no vaccine was available. ...
We studied the relationship between blood glucose concentration and clinical outcomes using a retrospective study. This had the advantage of allowing efficient collection and analysis of data from a large number of patients with AECOPD in whom the clinical outcome was already established. Our retrospective study had a number of limitations, some of which we were able to address during study design and analysis.. We identified people for inclusion in this study using the ICD-10 code J44.0 (acute exacerbation of COPD with lower respiratory tract infection). ICD-10 codes refer to the International Classification of Diseases, and disease codes are allocated by clinical clerks on the basis of summaries for hospital inpatients written by junior hospital doctors at the time of patient death or discharge. As this system is therefore imprecise, we chose to select patients with ICD-10 code J44.0. We felt that this code required a positive diagnosis of COPD exacerbation rather than other codes such as ...

At best, the vaccine is only partly effective; vaccinated women still need Pap smears. A MedLine search of the medical literature shows that no study has actually shown cancer prevention yet, only a reduction in abnormal Pap smears. This is called a surrogate endpoint (a substitute for the real thing), in the words of Dr. Kenneth Hatch of the University of Arizona, in a presentation to hospital medical staff about the vaccine. Such findings may resolve on their own, or may progress to cancer, over a period of years. The longest follow-up period in published vaccine studies is about 5 years; it usually takes decades for cervical cancer to develop. By 2020, there may be proof of efficacy against cancer (carcinoma in situ) in an ongoing Finnish study, which has enrolled 2,404 of 24,046 women invited to receive vaccine (Intl J STD & AIDS 2006;17:512-521). The vaccine only protects against 70 percent of the strains associated with invasive cancers, according to the Gardasil package insert. ...
Hospital medical staff may soon be able to more quickly visualize the presence of biofilm in endotracheal tubes, lessening the risk of ventilator-associated pneumonia, reports a new article the Journal of Biomedical Optics. The response rate was 62%: 594 competed questionnaires were returned and 48 were returned undelivered by the post office. Not all questions were answered by all participants. Fifty four per cent (321) of the participants were house officers or senior house officers, 39% (230) registrars, 3% (18) senior registrars, and 3% (20) other junior grades. Half were men (294 v 296) and 70% (413 v 174) were white. JUNIOR doctors have demanded concrete assurances from Tory health secretary Jeremy Hunt before considering re-entering negotiations.. Junior doctors are furious at the Tory governments attempts to impose a 90-hour working week on them and cut their pay by 30 per cent. They have called a demonstration for Saturday October 17th that will be bigger, better and louder than the last one on September 28th. It will start at Waterloo Place at 2.00pm, march to Parliament Square and finish at 6.00pm.. Chair of the British Medical Associations Junior Doctors Committee, Dr Johann Malawana, wrote to to health secretary Hunt on Monday demanding concrete assurances on the burning issues for junior doctors, anti-social working hours and pay. This follows on from a letter sent by Hunt to Dr Malawana on October 8th in which Hunt, fearing the junior doctors would proceed with their ballot for industrial action, began backpeddling.. In that letter Hunt claimed: I can give an absolute guarantee that ...

One thing we can count on in the healthcare industry is change. Healthcare reform. Changes in reimbursements. New technologies to help us do our jobs. Keeping up with change presents challenges and sometimes, decisions made based on current information need to be re-evaluated because it turns out that they may not have been the best choices.. In consideration of evolving employment models for in-house physician recruiters, ASPR has taken time to re-evaluate the decision made in 2011 to re-categorize members into three distinct categories. Through this review, we realized that some of the newer in-house recruitment models operate in much the same way as the more traditional model and that we are missing opportunities to learn and grow from one another by segmenting membership into multiple categories. We believe it is imperative that we embrace the changes in our industry, be more inclusive, and continue to evolve and grow. To this end, the ASPR Board of Directors voted to combine the Affiliate ...

Kissing down is an incredibly powerful method - most doctors, myself included, can usually count the number of moments of real praise given to us by a senior doctors on one hand. We treasure these moments and use them to model our own behavior and gain our sense of confidence. But why are they so few and far between? There are many reasons, but partly because kissing down is such a rarity in medicine and kicking down is so much more common. Think about Cox from Scrubs, the archetypal senior doctor that all junior doctors are terrified of - if he speaks a word of praise to one of them then they are rendered insensate for days as they become enveloped in a blissful shell of validation, purpose, and recognition. Perhaps it is because we idolise these Cox figures and their praise is hard won and therefore meaningful when given. Instead of these moments occurring on an annual basis, why not have these moments of meaningful praise occur on a monthly or a weekly basis?. Ultimately this problem points ...
Based on the most recent data, this week, there is a slight adjustment to priority zip codes - zip code 20001 is no longer considered a priority zip, and 20003 is being added as a priority zip code. The current list of priority zip codes includes: 20422, 20011, 20017, 20018, 20002, 20003, 20019, 20020, 20032, 20593. As a reminder, priority zip codes are determined by identifying areas where residents have been disproportionately burdened by COVID-19 and areas that are lagging in COVID-19 vaccination coverage.. This new pre-registration system will reduce the stress involved with making a vaccination appointment and to make it easier for the community to work together to get all eligible residents registered. Mayor Bowsers three operating principles for the new registration system are: 1) an equitable distribution of the vaccine; 2) a consistent scheduling cadence so residents know what to expect; and 3) a shared responsibility, across our community, to get people pre-registered.. Mayor Bowsers ...

As a department, we must be aware of the implications of employment law and the impact of national and EU regulations as we increasingly draw our staff from overseas, and the regulations regarding the employment of doctors from non-EU countries, for example; applying for Certificate of Sponsorship and giving advice on visas and using the appropriate and current Home Office guidance.. It is imperative that as a department we are aware of the external influences, which affect our role. Failure to be aware of, for example the correct GMC registration procedures and equal opportunities legislation could result in financial penalties for the Trust if a case was brought by a patient against an inappropriately or un-registered doctor, or if we were found guilty of racial or sexual discrimination in our procedures.. Cancer patients are having their operations cancelled as hospitals struggle to cope with the winter crisis, senior doctors have warned.. Surgeries for cancer patients, which are normally protected by guidelines requiring rapid treatment, are being called off in increasing numbers as hospitals face staff and bed shortages.. Ian Eardley, vice-president of the Royal College of Surgeons (RCS), said that members had reported an increasing number of cancellations within the past week and a shortage of beds was contributing to the problem.. The number of cancelled operations has been going up, Mr Eardley told the BBC. The NHS is under tremendous pressure: more and more patients are going to A&E and there is more difficulty in getting patients home.. His remarks came after another senior doctor,…. ...

N2 - With the changing emphasis of the modern medical school curriculum, many senior doctors and authors are commenting on a decline in anatomical knowledge of trainees. This study was conceived to provide a snapshot of medical students and junior doctors knowledge of anatomy. The carpal bones were chosen as a benchmark for anatomical knowledge as they are easily objectively examined and have clinical relevance to junior doctors in a number of disciplines. Twenty five fourth and final year medical students, fifteen pre-registration house officers (PRHOs) and ten senior house officers (SHOs) were recruited and the questionnaires were completed in the presence of one author. Of all participants recruited only fifteen could correctly name all eight carpal bones. Seven of these were SHOs in Surgery or Accident and ...

Within these limitations 81% of patients in our series undergoing urgent abdominal surgery were in pain at the time of giving consent to surgery. However, the majority (66%) perceived that this did not affect their ability to give informed consent. This is consistent with the findings of our previous study.3 This agreement between our two studies supports the impression that at least some patients with acute abdominal pain perceive that they retain the ability to give informed consent and is interesting, given that the two studies were separated by a time period of over 12 months and involved two distinct cohorts of junior hospital doctors.. Although 52 (70%) patients undergoing urgent surgery had received analgesia before signing consent only 14 (27%) reported an adverse effect of analgesia on the ability to give informed consent. This is perhaps fortunate as in only 57% of cases did the consent obtainer ascertain whether or not the patient had received analgesia prior to the informed consent ...

In without cause terminations, the notice period should be long enough to allow the physician to secure other employment. If the physician is permitted to initiate a without-cause termination, he or she should make sure the termination clause does not conflict with the non-compete clause.. Notice Period: Time needed for either party to end the contract.. Assignment: Can the contract be owned by someone else? This is usually not done in this type of agreement.. Patient Records: Who owns them and how are they handled. Contract should also address your access to patient records after employment ends.. Restrictive Covenants: Conditions, distance and timeframes. These non-compete clauses typically prevent a physician from working for a competitor located within a specific geographic area for a certain period of time. Non-compete clauses are illegal in some states, and theyre enforceable only if deemed reasonable in other states. Some employers have begun to use non-solicitation agreements. This ...

Hello ! ! ! I have a patient thats been going to UAMS for years for his myeloma care. So far hes been happy with the care hes recd. He started with a smoldering myeloma that ultimately became more aggressive. He was subsequently transplanted with his own donor stem cells and had 2 years of remission. Now his myeloma has recurred and he is currently on Kyprolis and Cytoxan with a slow response. He just left UAMS today after a re-evaluation and his bone marrow went from 65% blasts to around 45% after a few months of the latest treatment. He is considering getting a second opinion. He is seeing one of the staff physicians at UAMS, not Dr. Barloge. Is there a center where youd suggest a second opinion and if so what oncologist do you like? Thanks. Dr. Danziger ...
