The study was conducted over a five-month period in the Intensive Care Unit (ICU), Royal Childrens Hospital, Melbourne, and was approved by the hospital ethics committee. The ICU is an 18-bed multidisciplinary unit treating critically ill infants and children. During the study, 807 patients were admitted: 37% had undergone cardiac surgery, 27% had respiratory illnesses, 9% had general medical illnesses, 8% had newborn medical and surgical illnesses, 8% had trauma, and 8% had undergone neurosurgery and 3% general surgery. Mechanical ventilation was provided for 84% of patients. Most had invasive monitoring or therapeutic devices inserted into peripheral arteries, peripheral or central veins, the thoracic cavity, the urinary bladder or the extradural space. The mortality rate was 5.9%. The subjects of the study were any medical officers who made hand contact with any patient in the ICU. The ICU is newly constructed and comprises four rooms adjacent to each other along a corridor. Windows enable ...

Aims: To explore the depth of knowledge and understanding of blood products and their indications for use among junior medical staff in a teaching hospital paediatric department that contains a haematology/oncology unit but without dedicated juniors out of hours. Regular queries regarding blood product administration to consultants covering the unit precipitated this study.. Methods: Questionnaires were distributed to junior medical staff in the paediatric department. This covered basic knowledge of transfusion medicine, the use of special blood products, the use of departmental guidelines and personal confidence in ordering and prescribing.. Results: 29 questionnaires were distributed with 21 returns, from 15 specialist registrars and six senior house officers. Three doctors (14%) prescribed blood products on a weekly basis and 16 (76%) prescribed monthly. Only 85% of respondents had an accurate understanding of the ABO system and knowledge about blood product storage and administration was ...
Research on the quality of US resident physician performance levels has often been limited by lack of a comparison group or strict focus on specific diseases and geographical areas. In order to gain insight on differences in quality of care provided by resident physicians versus staff physicians, Boston Medical Center and Palo Alto Medical Foundation researchers investigated performance of physicians in 33,900 hospital-based outpatient visits throughout the US.
Background: Performing a lumbar puncture in an unwell child can cause anxiety in both the parent and the junior doctor. There is increasing evidence of post-lumbar-puncture complications in this age group.. Aims: To improve the documentation, consent for and technical performance of paediatric lumbar punctures to 100% of the required standard within 3 months.. Setting: The paediatric emergency department of a the Royal North Shore Hospital (University of Sydney, Sydney, Australia).. Participants: Paediatric emergency staff, including residents, registrars and consultants.. Methods: Medical records of 40 consecutive children who had undergone a lumbar puncture in the 6 months before the introduction of the lumbar-puncture proforma were reviewed. After introduction of the proforma, the records of 25 consecutive patients were reviewed to assess changes in the outcome measures. Before introduction of the proforma, junior medical staff were instructed in the procedure using specialised lumbar ...

We studied the relationship between blood glucose concentration and clinical outcomes using a retrospective study. This had the advantage of allowing efficient collection and analysis of data from a large number of patients with AECOPD in whom the clinical outcome was already established. Our retrospective study had a number of limitations, some of which we were able to address during study design and analysis.. We identified people for inclusion in this study using the ICD-10 code J44.0 (acute exacerbation of COPD with lower respiratory tract infection). ICD-10 codes refer to the International Classification of Diseases, and disease codes are allocated by clinical clerks on the basis of summaries for hospital inpatients written by junior hospital doctors at the time of patient death or discharge. As this system is therefore imprecise, we chose to select patients with ICD-10 code J44.0. We felt that this code required a positive diagnosis of COPD exacerbation rather than other codes such as ...

At best, the vaccine is only partly effective; vaccinated women still need Pap smears. A MedLine search of the medical literature shows that no study has actually shown cancer prevention yet, only a reduction in abnormal Pap smears. This is called a surrogate endpoint (a substitute for the real thing), in the words of Dr. Kenneth Hatch of the University of Arizona, in a presentation to hospital medical staff about the vaccine. Such findings may resolve on their own, or may progress to cancer, over a period of years. The longest follow-up period in published vaccine studies is about 5 years; it usually takes decades for cervical cancer to develop. By 2020, there may be proof of efficacy against cancer (carcinoma in situ) in an ongoing Finnish study, which has enrolled 2,404 of 24,046 women invited to receive vaccine (Intl J STD & AIDS 2006;17:512-521). The vaccine only protects against 70 percent of the strains associated with invasive cancers, according to the Gardasil package insert. ...
Hospital medical staff may soon be able to more quickly visualize the presence of biofilm in endotracheal tubes, lessening the risk of ventilator-associated pneumonia, reports a new article the Journal of Biomedical Optics. Researchers have demonstrated that demonstrated that optical coherence tomography (OCT) can be used to determine the presence of biofilm, providing an alternative to methods requiring arduous sample preparation.

JUNIOR doctors have demanded concrete assurances from Tory health secretary Jeremy Hunt before considering re-entering negotiations.. Junior doctors are furious at the Tory governments attempts to impose a 90-hour working week on them and cut their pay by 30 per cent. They have called a demonstration for Saturday October 17th that will be bigger, better and louder than the last one on September 28th. It will start at Waterloo Place at 2.00pm, march to Parliament Square and finish at 6.00pm.. Chair of the British Medical Associations Junior Doctors Committee, Dr Johann Malawana, wrote to to health secretary Hunt on Monday demanding concrete assurances on the burning issues for junior doctors, anti-social working hours and pay. This follows on from a letter sent by Hunt to Dr Malawana on October 8th in which Hunt, fearing the junior doctors would proceed with their ballot for industrial action, began backpeddling.. In that letter Hunt claimed: I can give an absolute guarantee that ...

One thing we can count on in the healthcare industry is change. Healthcare reform. Changes in reimbursements. New technologies to help us do our jobs. Keeping up with change presents challenges and sometimes, decisions made based on current information need to be re-evaluated because it turns out that they may not have been the best choices.. In consideration of evolving employment models for in-house physician recruiters, ASPR has taken time to re-evaluate the decision made in 2011 to re-categorize members into three distinct categories. Through this review, we realized that some of the newer in-house recruitment models operate in much the same way as the more traditional model and that we are missing opportunities to learn and grow from one another by segmenting membership into multiple categories. We believe it is imperative that we embrace the changes in our industry, be more inclusive, and continue to evolve and grow. To this end, the ASPR Board of Directors voted to combine the Affiliate ...

Kissing down is an incredibly powerful method - most doctors, myself included, can usually count the number of moments of real praise given to us by a senior doctors on one hand. We treasure these moments and use them to model our own behavior and gain our sense of confidence. But why are they so few and far between? There are many reasons, but partly because kissing down is such a rarity in medicine and kicking down is so much more common. Think about Cox from Scrubs, the archetypal senior doctor that all junior doctors are terrified of - if he speaks a word of praise to one of them then they are rendered insensate for days as they become enveloped in a blissful shell of validation, purpose, and recognition. Perhaps it is because we idolise these Cox figures and their praise is hard won and therefore meaningful when given. Instead of these moments occurring on an annual basis, why not have these moments of meaningful praise occur on a monthly or a weekly basis?. Ultimately this problem points ...
As a department, we must be aware of the implications of employment law and the impact of national and EU regulations as we increasingly draw our staff from overseas, and the regulations regarding the employment of doctors from non-EU countries, for example; applying for Certificate of Sponsorship and giving advice on visas and using the appropriate and current Home Office guidance.. It is imperative that as a department we are aware of the external influences, which affect our role. Failure to be aware of, for example the correct GMC registration procedures and equal opportunities legislation could result in financial penalties for the Trust if a case was brought by a patient against an inappropriately or un-registered doctor, or if we were found guilty of racial or sexual discrimination in our procedures.. The Medical Staffing department manages the recruitment and administration of all senior and junior medical staff, for all specialties in the Trust, visit our latest vacancies page for all ...
Cancer patients are having their operations cancelled as hospitals struggle to cope with the winter crisis, senior doctors have warned.. Surgeries for cancer patients, which are normally protected by guidelines requiring rapid treatment, are being called off in increasing numbers as hospitals face staff and bed shortages.. Ian Eardley, vice-president of the Royal College of Surgeons (RCS), said that members had reported an increasing number of cancellations within the past week and a shortage of beds was contributing to the problem.. "The number of cancelled operations has been going up," Mr Eardley told the BBC. "The NHS is under tremendous pressure: more and more patients are going to A&E and there is more difficulty in getting patients home.". His remarks came after another senior doctor,…. ...
Cervical cancer is cancer of the neck of the womb and it is the second most common cancer in women worldwide, second only to breast cancer; however in the UK it is relatively uncommon. The reason for this is the National Screening Programme for Cervical Cancer, which scientists believe saves 5000 womens lives in the UK each year. ...
To put it simply, I would not be here without the junior doctors who have spent hours with me, stayed after their shifts, shared parts of their lives with me, fought to get me the treatment I need, written up my medications when re-boarding my chart over and over, comforted me, laughed and cried with me, chased up test results and more. One junior doctor I remember spent 8 days (yes, 8 days), on top of her usual workload and treating me on a day to day basis along with all the other patients under her care, writing up my history, all my notes, test results and more for my referral for a bowel transplant at Addenbrookes Hospital, Cambridge. 8 whole days work on top of her usual workload, purely for my benefit, only for me to be turned down as I would not survive a transplant and TPN (feeding into the bloodstream) is still successful at the moment. Yet she was not disappointed. She was just glad to have played her part in helping me. There was the junior doctor who whistled her way round the ward ...

A blood test was taken when Savita was first admitted to the hospital on Sunday, October 20th, but it appears that result was not followed up until the following Wednesday. That test showed a raised white blood cell count, which can be an indicator of infection.. Dr Ikechucwu Uzockwu, Senior House Officer at Galway University Hospital, first raised concerns about sepsis at about 7am on Wednesday October 24th.. It was put to Dr Uzockwu at the inquest that a statement from midwife Anne Maria Burke showed that Ms Halappanavars pulse was taken and found to be 110 beats per minute, and that he had been told this.. "I was told the vital signs were stable," he said. "I was not told of an elevated pulse. There is no question that if I had known she had an elevated pulse, this lady had ruptured membranes, I would have attended immediately.". He said in evidence that he had informed a senior specialist registrar, Dr Sarah Campbell, of his concerns, and that Dr Campbell told him she would come to Ms ...

I like this job because I want to help others," says Anna Mzeru. With the NIMART training she received, Mzeru says her work performance and patient attendance rate have both improved.Photo credit: Megan Montgomery/MSH. The idea is to train staff to handle activities that would normally be outside the scope of their duties to make more efficient use of human resources. Sharing tasks with junior staff can free up those with higher-level skills to handle more complicated cases. Task sharing also allows for better distribution of work across the facility so that patients receive more timely care.. As part of the program, Mzeru attended a 10-day Nurse Initiated Management of Antiretroviral Therapy (NIMART) training last year that included lectures, discussions, and practical sessions. While she previously had to refer patients to district facilities for HIV care - even though many could not afford transport - she can now perform all HIV services right in her office. This includes initiating and ...
Considerable effort has gone into perfecting the art of tablet data collection, which is the method typically used to collect data for evaluating education programs. The move away from paper has been a welcome shift, as for many research and evaluation professionals, paper conjures images of junior staff buried under boxes of returned questionnaires manually entering data into computers. Indeed, when our team recently began experimenting with paper-based data collection in our education projects, one colleague with decades of experience remarked warily, "It just seems like were moving backwards here!". Improvements in the software, however, allow us to merge new technology with "old school" methods. Digital scanners can now replace manual data entry, powered by software that is able to read completed questionnaires, and quickly format responses into a data set for subsequent analysis. Our team has been experimenting with a new digital scanning software called Gravic to easily and quickly enter ...
As the Ministers statement (Legislative Council Hansard of 9 December 2008, p 1042) indicates, Wilson was billed $75,049.15 by the Department for the personal information. The state government subsequently tried to retrieve the information on several occasions and sought legal advice on how to force Wilson to hand back or destroy the data after what was characterised as "an honest mistake" made by "a junior staff member". Wilson "declined to return the information and subsequently made several unsuccessful attempts to pay the account ...

This set of instructions is only but a guide. At all times clinical judgment should be used when performing any invasive technique and the ultimate responsibility is yours. If in doubt, always discuss matters with your senior doctors before performing any invasive technique with which you have suboptimal experience and / or confidence. If you are in doubt, it is better to defer performing the technique yourself to someone who can do it (and preferably they can teach you how to do it at the same time). I do not subscribe to the adage of see one, do one, teach one as it is prone to failure and teaching of poor technique. As a friend once said to me it is not practise that makes perfect but it is perfect practise that makes perfect. Now I do subscribe to that ...
