OBJECTIVE. Our goal was to determine if there were any changes in risk-adjusted mortality after the implementation of a computerized provider order entry system in our PICU. METHODS. Study was undertaken in a tertiary care PICU with 20 beds and 1100 annual admissions. Demographic, admission source, primary diagnosis, crude mortality, and Pediatric Risk of Mortality III risk-adjusted mortality were abstracted retrospectively on all admissions from the PICUEs database for the period October 1, 2002, to December 31, 2004. This time period reflects the 13 months before and 13 months after computerized provider order entry implementation. Pediatric Risk of Mortality III mortality risk adjustment was used to determine standardized mortality ratios. RESULTS. Aligning social structures and technology capabilities is a significant challenge to information technology-related organizational change. It is particularly challenging in institutionalized settings such as hospitals. We report an interpretive field study of computerized physician order entry (CPOE) at an acute-care hospital, in which we investigated how institutionally triggered and technology-triggered change interacted in complementary processes to engender alignment. Social structure changes included increased interdependency among clinical departments, multidisciplinary cooperation across clinical disciplines, and standardization in clinical decision-making. Organization members also enacted institutionalized interaction patterns with physicians by deferring to their preferences for CPOE use. We found that before either intervention, patients in the NICU were at significantly higher risk for a wrong-patient order compared with patients in non-NICU pediatric units, with the risk being even higher for multiples. The implementation of the ID reentry intervention and the addition of the distinct naming intervention for newborns were associated with a large combined total reduction of wrong-patient orders (61.1%) in the NICU compared with baseline. The combined effect of the 2 interventions moved the overall rate of wrong-patient orders in the NICU from significantly higher than non-NICU pediatric units to lower than non-NICU pediatric units.
