TY - JOUR. T1 - Current Perspectives in Intra Operative Airway Management in Maxillofacial Trauma. AU - Vidya, B.. AU - Cariappa, K. M.. AU - Kamath, Abhay T.. PY - 2012/6/1. Y1 - 2012/6/1. N2 - Objective: Maxillofacial trauma presents a complex problem due to the disruption of normal anatomy. In such cases, we anticipate a difficult oral intubation that may hinder intraoperative IMF. Nasal and skull base fractures do not advocate use of nasotracheal intubation. Hence, other anesthetic techniques should be considered in management of maxillofacial trauma patients with occlusal derangement and nasal deformity. This study evaluates the indications and outcomes of anesthetic management by retromolar, nasal, submental intubation and tracheostomy. Methodology: Of the 49 maxillofacial trauma cases reviewed, that required intraoperative IMF, 32 underwent nasal intubation, 9 patients had tracheostomy, 5 patients utilized submental approach and 3 underwent retromolar intubation. Results: Among patients ...
Results For the 46 patients, the female to male ratio was 1.3:1 and the mean age was 36.7 years. Most patients (41, 89.1%) were Tibetan. The most frequent cause of maxillofacial injury was pressing/burying (34 patients, 73.9%). All patients with maxillofacial injuries sustained soft-tissue injuries, 13.0% had facial fractures and 4.3% had dentoalveolar injuries. The soft-tissue injuries were largely combined injuries; 84.8% were bruises and 80.4% were lacerations. The most common injury site was the zygomatic region (54.3%), followed by the forehead (43.5%) and the orbital region (34.8%). Of the six facial fractures, four involved nasal-orbital-ethmoidal region fractures. Most of the maxillofacial injuries (78.3%) were associated with other injuries, of which extremity injuries (55.6%) were the most common. ...

Maxillofacial trauma is often diagnosed and treated by specialists in emergency medicine. About 50 percent of patients with facial injuries have suffered trauma to other organ systems or other parts of the body, however, and may need care from specialists in ophthalmology, plastic surgery, otolaryngology, trauma surgery, oral surgery, and psychiatry as well as from doctors with specialized training in emergency medicine. Injuries to the face and jaw area require special attention because they involve the senses of sight, hearing, taste, and smell as well as such vital functions as breathing. From a psychological perspective, maxillofacial trauma can be additionally upsetting if the patients appearance is permanently affected. The doctor will begin by taking a history, either from the patient if he is able to talk or from the parents or other witnesses. In the case of a known accident, sports injury, or assault, the doctor begin with the ABCs, which means that he or she will check the child or ...

Severely injured patients often exhibit injuries in the maxillofacial region, ranging from small lacerations to multiple and life-threatening fractures of the facial bones. In the literature the incidence of these concomitant facial injuries in multiple injured patients range between 15 and 22 % [3, 14]. The result in the present investigation is in line with these findings. However, comparison between epidemiologic investigations is difficult due to differences in study population, geographic region, and socioeconomic status. Further, the time of the year can influence the results as demonstrated in the present collective. In our investigation, injuries occured mainly in June and August. This observation might be explained by the circumstance that the data was collected at a level-one trauma centre located in an urban setting and most of the injuries were caused by motorcycle and bicycle accidents. In contrast, increasing incidences during the winter can be observed in regions located nearby ...

The common causes of maxillofacial injuries are road traffic accidents , assaults followed by pathologies. The congested roads of poor quality , increasing population and traffic and the poor driving sense of the people and illiteracy about traffic rules are all contributing factors to road traffic accidents .The prominency of the facial skeleton and the tendency to fall contribute to more injuries to this part of the body. The assault and fights along with sports injuries are also a common cause of the maxillofacial injuries.3rd contributing factor though not as common as others is pathologies.. The facial bones are very complex as a number of bones are linked with each other. So it is difficult that a single bone breaks without damaging its neighbouring one. The lower jaw bone is a u shaped and the impact on one side mostly leads to fracture of other side along with the site of impact. Another important thing is that the jaw and facial bones are associated with many features other than ...

Swelling of the conjunctiva (chemosis) is common in association with subconjunctival hemorrhage and resolves spontaneously. A tear of the conjunctiva is suggestive of a more severe blunt injury. In every case, internal injury to the globe must be sought.. Loss of the corneal epithelium (Fig. 18-7) is fairly common and causes the same signs and symptoms as a corneal erosion. The corneal endothelium is comprised of a monolayer of cells that probably do not replicate following injury. Their function is to maintain the clarity of the cornea by pumping water out of the cornea and into the anterior chamber. Damage to the corneal endothelium results from a combination of contusion, reactive inflammation, and raised intraocular pressure.11 This condition may culminate in permanent edema if the endothelial cell population is reduced below a critical level. Recovery of corneal clarity can take place in some patients after a number of/>. ...

UPPER THIRD FRACTURES. In this sample, frontal fractures accounted for only 1.0% of the total fractures in both units. There was no significant difference (p= 0.088) in the occurrence of the problem between CMJAH (1.6%) and PMHC (6.1%).. MULTIPLE FRACTURED SITES. Multiple fracture sites accounted for 7.0% of the total 227 fractures. The difference of the frequencies between CMJAH (5.5%) and PMHC (9.1%) was not statistically significant (p= 0.346).. DISCUSSION. The aim of this study was to analyse and compare epidemiologic characteristics of maxillofacial fractures between CMJAH, which is in Johannesburg and PMHC, which is in a mainly rural province of Limpopo. There have been a number of epidemiological studies of maxillofacial fractures completed mostly in metropolitan cities of South Africa,8,10 but no similar studies were undertaken in rural provinces. Understanding the patterns helps in determining the level of skills required in both provinces and will assist in the planning for ...

Facial traumas include injuries to the skin covering, underlying skeleton, neck, nasal (sinuses), orbital socket, or oral lining, as well as the teeth and dental structures. Sometimes these types of injuries are called maxillofacial injury. Facial trauma is often recognized by lacerations (breaks in the skin); bruising around the eyes, widening of the distance between the eyes (which may indicate injury to the bones between the eye sockets); movement of the upper jaw when the head is stabilized (which may indicate a fracture in this area); and abnormal sensations on the cheek.. Hospitalization is often required for treatment, which is similar to that for fractures in other parts of the body. The bones must be lined up and held in place to allow time to heal them in the correct position.
