South Africas maternal mortality rate (625 deaths/100,000 live births) is high for a middle-income country, although over 90% of pregnant women utilize maternal health services. Alongside HIV/AIDS, barriers to Comprehensive Emergency Obstetric Care currently impede the countrys Millenium Development Goals (MDGs) of reducing child mortality and improving maternal health. While health system barriers to obstetric care have been well documented,
Responding to this need, the Foundation launched a four-year (June 2015- September 2019) maternal health quality of care (MHQoC) strategy in June 2015, which aims to catalyze another paradigm shift, this time in the maternal health arena within India-changing the focus from increasing access to maternal health services to improving the quality of these services. The MHQoC strategy marks the final stage of the Foundations grant-making to support PRH in India and seeks to build a foundation and momentum for further maternal health quality improvements after the Foundation exits the country. To this end, it includes three key sub-strategies: (1) strengthening the supply of quality maternal health services in both the public and private sectors, (2) building demand for quality services through accountability mechanisms, and (3) building evidence and supporting advocacy for quality maternal health services. To carry out the work of the strategy, the Foundation has funded 20 grantees through 28 ...
Result: The results showed that 38.5 percent (154) of the women received antenatal care, 32.3 percent (129) of the women received delivery services while 48.3 percent (193) received postnatal care services. In the logistic regression model, reduced income level was associated with decreased use of antenatal care (OR=0.1, P,0.05). In assisted delivery, increased distance to the health facility was associated with reduced use (OR=0.1, P ,0.05). In postnatal care, reduced education level of the respondent was found to be associated to decreased use of postnatal services (OR=0.51, P,0.05). Increased maternal age was associated with increased utilization of maternal health services (OR=2.1, P,0.05). On the other hand, decision making roles by the male spouse alone or both partners was associated with increased use of postnatal services (OR=4.5,P,0.05 ...

The need to promote maternal health in Ghana led to the governments commitment to extend maternal healthcare services to the door steps of rural families through community-based health planning and services (CHPS). The CHPS programme uses community health officers (CHOs) to provide a range of services including antenatal care (ANC), postnatal care (PNC) and emergency delivery [1, 2]. By reducing geographic barriers to healthcare, the CHPS strategy enables the Ghana Health Service (GHS) to reduce health inequalities and promote equity of health outcomes [2, 3]. The implementation of CHPS requires the cooperation of the health sector and involvement of all stakeholders, especially men, in the provision and reception of healthcare [3].. The need for male involvement in maternity care can be traced back to the 1994 International Conference on Population and Development [4] in Cairo, that emphasised the inclusion of men as partners and role players in maternal healthcare. In recognition of the ...
The International Safe Motherhood initiative provides a focus for programmes and research to improve maternal health in low - income countries. Antenatal care is one of the key pillars of the initiative. This study sought to examine the association between background characteristics and choice of skilled providers of antenatal care services in Ghana. The study used data from the six rounds of the Ghana Demographic and Health Survey (GDHS). Binary logistic regression models were applied to analyse the data. Results show that the proportion of women who received antenatal care (ANC) services from skilled providers improved over the period. Also, women with secondary education (OR = 1.42, CI = 1.07-1.88), richest wealth status (OR = 5.10, CI = 2.28-11.85) were more likely to utilise antenatal care services from skilled providers. Whereas women from rural areas (OR = 0.55, CI = 0.41-0.74), with four births or more (OR = 0.55, CI = 0.36-0.85) and from the northern ethnic group were less likely to utilise

Although the maternal mortality ratio in Bangladesh has decreased, significant underutilization of facilities continues to be a persistent challenge to policy makers. Women face long distances and significant transportation cost to deliver at health facilities. This study identifies the distance traveled to utilize facilities, associated transportation cost, and transport mode used for maternal healthcare services. A total of 3,300 mothers aged 18-49 years, who had given birth in the year before the survey, were interviewed from 22 sub-districts in 2010. Findings suggest that facility-based maternal healthcare service utilization was very poor. Only 53% of women received antenatal care, 20% used delivery care. and 10% used postnatal care from health centers. Median distance traveled for antenatal and postnatal check-ups was 2 kilometers but 4 kilometers for complication management care and delivery. Most women used non-motorized rickshaw or van to reach a health facility. On average, women spent ...
Access to antenatal care services promotes safe motherhood and delivery. It also improves maternal and neonatal outcomes. This study identified factors influencing the utilization of antenatal care services and health facility-based delivery. Methods: A community based cross sectional survey was conducted. Households were enumerated, and a systematic sampling method was used to selected one in four HHs.A sample size of 200 was used. Of the 200 HHs, 198 women who were pregnant or had previous deliveries were interviewed using pretested semi-structured questionnaires. Data was collected on socio-demographic characteristics, attendance at the antenatal clinic (ANC), institutional delivery, and reasons for non-utilisation of ANC services. Data analysis was conducted using Epi Info version 3.5.3. Results: The median age of respondents was 29 years (range: 17 - 55). Overall, 192 (97%) women had at least one ANC visit, 116 (58.6%) had ≥ 4 ANC visits and 93 (47.9%) commenced ANC in the second ...

Reducing maternal mortality and morbidity remains a challenge worldwide. In 2013, 289,000 women died from pregnancy-related causes, and maternal death was identified as the second biggest killer of women of reproductive age for that year [1]. The majority of those deaths occurred in developing countries, especially in sub-Saharan Africa, and could have been prevented by improving access to quality care. In line with this, skilled birth attendance was recognized as a crucial intervention to reduce maternal mortality and morbidity [2, 3]. However, skilled birth attendance cannot be achieved if maternal services are not sufficiently utilized. If health services were more responsive to the citizens demands and made changes on the basis of ideas or concerns raised by or with community members [4], their utilization would probably improve [5]. A key way to ensure better utilization of services is to strengthen the community voice and take into account community concerns and views in health planning ...

Maternal mortality is a huge concern globally leading to more than a quarter of a million deaths every year. Similarly, an estimated 4 million neonates die every year worldwide, contributing to the majority of deaths of under-5 children. The majority of these deaths take place in under-developed countries, and specifically, in the sub-Saharan Africa region. It is evidenced that maternal ill-health and death contribute to the majority of child deaths. Reducing the death of children under 5 years by two thirds and also improving maternal health/reducing maternal death by three quarters between the years 1990 and 2015 are two of the eight aims of the Millennium Development Goals (MDGs), MDG-4 and MDG-5 respectively. The report on maternal health services in Eritrea, a nation in the sub-Saharan Africa, shows a low utilization of antenatal care, family planning services, and postnatal care. Furthermore, there is insufficient use of skilled assistance at delivery. The purpose of this study was to explore the

Contribution and significance. The practical contribution is to improve maternal healthcare in Uganda specifically through improved predictions of pregnancy complications in order for Uganda to meet the MDG5. Furthermore, the research supports the task shifting strategy by increasing access to quality care in low resource settings. According to the World Health Organization, maternal mortality is defined as the death of a woman while pregnant or within 42 days of termination of pregnancy.[17] The maternal mortality rates in Texas have been a source of concern as well as much discussion. From 2000 to 2010, the maternal mortality rate in Texas increased from 17.7 (for every 100,000 live births) to 18.6.[3] It must be noted that during this period, in 2006, Texas included the consideration of pregnancy on its death certificate.[3] However, this was not seen to visibly affect the maternal mortality rates in the ten-year time period. After 2010, the maternal mortality rate doubled, exceeding 35 between 2010 and 2014 and remaining higher than 30 in 2014.[3] In the US, from 1987 to 2009, the leading causes of pregnancy-related deaths included hemorrhage, sepsis, and hyperintensive disorders.[18] From 2006 to 2009, these causes changed, with cardiovascular conditions accounting for more than a third of pregnancy-related ...

Caesarean sections (CSs) are associated with increased maternal and perinatal morbidity, yet rates continue to increase within most countries. Effective interventions are required to reduce the number of non-medically indicated CSs and improve outcomes for women and infants. This paper reports findings of a systematic review of literature related to maternity service organisational interventions that have a primary intention of improving CS rates. A three-phase search strategy was implemented to identify studies utilising organisational interventions to improve CS rates in maternity services. The database search (including Cochrane CENTRAL, CINAHL, MEDLINE, Maternity and Infant Care, EMBASE and SCOPUS) was restricted to peer-reviewed journal articles published from 1 January 1980 to 31 December 2017. Reference lists of relevant reviews and included studies were also searched. Primary outcomes were overall, planned, and unplanned CS rates. Secondary outcomes included a suite of birth outcomes. A series

BACKGROUND: This study was undertaken to investigate womens accounts of interactions with health care providers during labour and delivery and to assess the implications for acceptability and utilisation of maternity services in Ghana.. METHODS: Twenty-one individual in-depth interviews and two focus group discussions were conducted with women of reproductive age who had delivered in the past five years in the Greater Accra Region. The study investigated womens perceptions and experiences of care in terms of factors that influenced place of delivery, satisfaction with services, expectations of care and whether they would recommend services.. RESULTS: One component of care which appeared to be of great importance to women was staff attitudes. This factor had considerable influence on acceptability and utilisation of services. Otherwise, a successful labour outcome and non-medical factors such as cost, perceived quality of care and proximity of services were important. Our findings indicate that ...

Brazil is a signatory to the Millennium Declaration, and the most recent Brazilian National Progress Report on the Millennium Development Goals points to important strides, especially in the fight against extreme poverty and hunger, the reduction of infant mortality, and the struggle against diseases like HIV/AIDS, malaria, tuberculosis, and leprosy. However, forecasts are bleak for achieving the goal of improved maternal health. Despite progress, the inequalities are striking. Gender, ...

Introduction: Understanding the preventive dental health care utilization of children is essential to reducing child dental disease rates in low income families. Previous studies have not examined the relationship between the mothers health status and the childs dental care utilization. The aim of this study is to examine the association between maternal health status and use of preventive dental care among children enrolled in Medicaid and CHIP programs. Methods: We conducted a cross sectional study of children enrolled in Medicaid and CHIP programs. A sample of 88,460 children was obtained from the 2011-2012 National Survey of Childrens Health (NSCH). We used multivariable logistic regression modeling (SAS 9.3) to estimate the adjusted odds ratio for having at least one dental visit in the past 12 months associated with self-reported maternal health. We statistically adjusted for childs age, sex, maternal education, family structure, and parental satisfaction with the childs health care provider.
Expanding access to quality, affordable maternal health medicines is critical to making progress in reducing maternal mortality. However, significant challenges often impede such access. Chief among them is a lack of data on the needs, gaps, systems and financing for maternal health medicines.
