One of the goals of this study was to learn the coverage, safety and logistics of a mass vaccination campaign against typhoid fever in children and adults using locally produced typhoid Vi polysaccharide (PS) and group A meningococcal PS vaccines in southern China. The vaccination campaign targeted 118,588 persons in Hechi, Guangxi Province, aged between 5 to 60 years, in 2003. The study area was divided into 107 geographic clusters, which were randomly allocated to receive one of the single-dose parenteral vaccines. All aspects regarding vaccination logistics, feasibility and safety were documented and systematically recorded. Results of the logistics, feasibility and safety are reported. The campaign lasted 5 weeks and the overall vaccination coverage was 78%. On average, the 30 vaccine teams gave immunizations on 23 days. Three economic evaluations of pandemic influenza A (H1N1) vaccination campaigns in North America concluded that the mass vaccination campaigns would be, or were, highly cost-effective, if not cost-saving. This paper re-assesses each study's analysis and presents three arguments: 1) prediction of vaccine program cost-effectiveness is unrealistic, if not impossible, unless quality surveillance data is available; 2) even when surveillance data is available, vaccine cost-effectiveness calculations can still vary dramatically and need to undergo wider-ranging sensitivity analysis; 3) H1N1 vaccination campaigns in North America were likely not as cost-effective as previously thought. Several recommendations are then made for improving transparency, accuracy, portability, and sensitivity analysis in pharmacoeconomic studies more generally.   Key words: Pandemic, influenza, vaccination, immunization, cost-effectiveness, health economics, pharmaco-economics.

Since the 1940s mass vaccination programmes have formed important elements in the governmental provision of healthcare services to children. Collectively, they have reduced the incidence and severity of numerous childhood diseases. Yet, they have also been subject to moments of social anxiety and panic over safety issues, necessity and the reach of the nanny state. These issues remain as ever present today as they did in the 1940s. They speak to the wider historical development of vaccinations, their introduction in Britain and how the NHS became the central instrument for providing childhood vaccinations free of charge to the public. While mass vaccination pre-dated the NHS, the launch of the NHS coincided with the considerable increase in the types of diseases for which vaccination programmes were developed and introduced in Britain. The Health Department continues to vaccinate residents against the flu, with another free H1N1 clinic slated for Friday, Health Director James Roosen told the board Wednesday. The flu clinic has been extremely complicated because we have got two different vaccines this year -- the live (H1N1) virus, mist and the seasonal flu, he said. We have had 3,600 people we have already vaccinated, roughly three times what we did last year. The Health Department is continuing to distribute H1N1 to primary care providers, Roosen said. Madathil led a team of UTEP faculty, staff and students who observed several of El Pasos drive-though and walk-in clinics in early 2021. The team identified areas that likely created bottlenecks, which produce delays and other issues. Residents are able to book appointments, or reschedule their second dose appointment (external link) and select a clinic offering either the Pfizer or Moderna vaccine at the time of booking. This means that residents who received an mRNA vaccine (Pfizer or Moderna) as their first dose, are able to choose to book an appointment to receive the same or a different mRNA vaccine for their second dose through our online booking system. A risk of death of only one in a million, while very favorable in the risk-risk trade-off calculus, might be a significant psychological barrier if in the first weeks of a mass immunization campaign the public sees several dozen deaths related to vaccination.
