The central giant cell granuloma is felt to be a benign lesion that only occurs in the jaws, though similar lesions occur in the fingers and toes. Its relationship to the giant cell tumor of the long bones is unknown, but it is felt that the giant cell tumor represents a more aggressive and possibly malignant lesion that rarely occurs in the jaws, if at all. Other authorities feel that these lesions represent a spectrum of disease from malignant to very benign. Central giant cell granulomas normally occur in the anterior part of the jaws (in areas where deciduous teeth were present) and normally occur in the second and third decades, although they have been noted in all decades of life. Histologically similar, or even identical, appearances may occur in hyperparathyroidism, cherubism, and aneurysmal bone cysts, which should normally be ruled out prior to establishing the diagnosis. When the central giant cell granuloma was first identified as a distinct lesion (previously, it had been included ...
The radiographic diagnosis of typical Stafne bone cavity could be done easily with cyst-like round or oval radiolucency near the angle of the mandible, under mandibular canal with or without involving mandibular base, and no symptoms. However there are some atypical cases suggesting possible variations of this entity. We report a quite unusual case, where Stafne bone cavity was lastly included in the differential diagnosis list. Histological examination of salivary gland tissues confirmed the final diagnosis. ...

19 cases of central giant cell granuloma (CGCG) were reviewed after screening the records of the past four years of the Dental out patient department of AIIMS Hospital, New Delhi. It was found that CGCG usually occurs in the 2nd and 3rd decades and is more common in females. The mandible is more frequently involved than the maxilla and lesions occur more commonly on the right than left side.

Central giant cell granuloma (CGCG) is a benign intraosseous lesion predominantly involving the mandible in young females. Exact etiology of this lesion is controversial and remains unknown. However, three competing theories are prevailing which states that it could be a reactive lesion, a developmental anomaly or a benign neoplasm. CGCG has been grouped into non-aggressive and aggressive variants on the basis of clinical, radiographic and histopathologic features. This paper reported a case of 29 year old female patient, presented with complain of pain and swelling in right vestibule region for one and half month. Based on clinicoradiologic findings, provisional diagnosis of aggressive CGCG was given and the lesion was surgically intervened. Histopathological report revealed central giant cell granuloma. The healing was uneventful and no complication noted till date. The purpose of this article is to review the clinical, radiological, histopathological features and management of aggressive CGCG ...
Paradental cysts constitute a family of inflammatory odontogenic cyst, that typically appear in relation to crown or root of partially erupted molar tooth. When the cyst is developed in the distal region of partially erupted third molar or in other locations in the dentition, it called simply paradental cyst, but the unique cyst that developed in the buccal bifurcation region of the mandibular first molars in the second half of the first decade of life is called buccal bifurcation cyst and has unique clinical features and management considerations in comparison to the other paradental cysts. Nocardiosis is an unusual infection in companion animals characterized by suppurative to pyogranulomatous lesions, localized or disseminated. Cutaneous-subcutaneous, pulmonary and systemic signs are observed in feline nocardiosis. However, osteomyelitis is a rare clinical manifestation in cats. Nocardia cyriacigeorgica (formerly N. asteroides sensu stricto), Nocardia brasiliensis, Nocardia otitidiscaviarum, and Nocardia nova are the most common pathogenic species identified in cats, based on recent molecular classification (16S rRNA gene). The present report is, to our knowledge, the first case of mandibular osteomyelitis in a cat caused by Nocardia africana, diagnosed based upon a combination of methods, including molecular techniques. A one-year-old non-neutered female cat, raised in a rural area, was admitted to the Companion Animal Hospital-PUCPR, São José dos Pinhais, State of Paraná, Brazil, with a history a progressive facial lesion, difficulty apprehending food, loss of appetite, apathy and
On Oct. 1, 2017, Aarti and Ashish Sarin took their daughter to urgent care because of a low grade fever. The nurse there noticed some swelling along her 3-year-old daughter Shivayas right jaw. Perhaps a swollen lymph node, she thought, from a low-grade fever? But when she touched her childs jaw, she felt a hard mass. Differential diagnoses from ear and staph infections followed until Aarti met with a radiologist for the results of a scan of her daughters right mandible, which had grown dramatically in just a few days.. The radiologist said, What the hell is this? Ive never seen anything like this-go get her admitted, Aarti says, describing a watershed moment. The world stopped.. Aarti and Ashish were told it was a bone cancer, Ewings Sarcoma. What it turned out to be, after workups at hospitals in northern Virginia and Washington, D.C., was a central giant cell granuloma, not a malignant cancer but an aggressive tumor that was eating away her right jawbone and now beginning to inflict ...
It was hypothesized that fluorodeoxyglucose (FDG) uptake on post‑treatment follow‑up positron emission tomography with computed tomography (PET CT; using PET CT to monitor and rule out recurrence and metastasis of head and neck carcinoma) would be useful for detecting and understanding the disease state of osteoradionecrosis (ORN) of the jaw. The present study included 14 patients who developed mandibular ORN following radiation therapy (RT) for head and neck cancer and underwent follow‑up PET CT several times following RT. Areas exhibiting FDG uptake were retrospectively assessed on post‑treatment follow‑up PET CT images and were classified into three types: Spot type: Only spot accumulation of FDG; localized type: Accumulation of FDG restricted to within the bone resorption area; extensive type: Accumulation of FDG extending into surrounding soft tissue. PET classification at the time of clinical diagnosis of mandibular ORN in the 14 patients demonstrated the extensive type in 43%, ...

We had hypothesized that post-treatment follow-up with FDG-PET CT to monitor head and neck carcinoma would be useful for detecting ORN. This retrospective case series study shows the sequential increase in FDG uptake in patients with mandibular ORN, indicating that post-treatment follow-up with FDG-PET CT has the potential not only to evaluate cancer recurrence and metastasis but also to detect and help us understand the disease conditions of ORN.. We introduced a novel classification of ORN based on the range of FDG accumulation in relevant tissues. In the PET classification of ORN, the spot type probably reflects inflammation due to marginal or apical periodontitis. ORN can develop in odontogenic foci (e.g., periodontal and apical disease) or in cases of failure to heal after dental extraction (10). As shown in Fig. 2, ORN sequentially progressed from local odontogenic foci that exhibited spot accumulation of FDG. The spot accumulation of FDG in the mandible on post-treatment follow-up FDG-PET ...

TY - JOUR. T1 - Endodontic intervention in the traumatic bone cyst. AU - Newton, Carl W.. AU - Zunt, Susan. PY - 1987. Y1 - 1987. N2 - A 44-yr-old woman was seen with an intrabony multilocular radiolucency of the posterior mandible in the area of the left first molar. A full gold crown had been placed 7 yr previously, and a periapical radiograph at that time had revealed mild sclerosis of the periapical bone. After vitality of the pulp was confirmed, endodontic therapy was performed prior to the surgical exploration of the lesion. At surgery an empty intrabony cavity was present and a clinical diagnosis of traumatic bone cyst was made. Followup at 2 yr revealed healing with marked sclerosis. This case is reported because of the unusual occurrence of the traumatic bone cyst as well as the role of endodontic treatment in its diagnosis and treatment.. To the best of our knowledge, this is the first pediatric case of severe FGF23 tumor-induced rickets secondary to a CGCG of the maxillary bones. Retrospectively, we think the initial limp was the first sign of weakness associated with rickets. We assumed that the tumor was the source of production of FGF23 because rickets started with the onset of the CGCG, coinciding with an extremely high FGF23 serum level. Also, the rickets and the need for phosphorus supplementation improved as the tumors size gradually decreased. The limitation of our case is that we could not provide the final proof of concept, because we did not perform tests looking for FGF23 production in the surgical samples.. TIO is a rare condition first described by McCance1 in 1947 as a paraneoplastic disorder characterized by phosphaturia, hypophosphatemia, low serum levels of 1,25-dihydroxy-vitamin D, and rickets or osteomalacia.7,10 Tumors responsible for this condition usually affect patients ,30 years old and are typically ...
Introduction and rationale. Each year, 300 danish patients are diagnosed with oral cancer. The majority of these cancers are planocellular carcinomas. The established treatments are surgery and radiation therapy. Depending on multiple factors such as total radiation dose, fractionation, localization and use of brachytherapy, mandibular osteradionecrosis occurs in 5-15% of previously head and neck irradiated patients.. Osteradionecrosis is defined by National Cancer Institute (NCI) as radiation-induced cell death in components of bone (e.g., the marrow fat and mineralized tissue). It represents the final common pathway of several disease entities, which result in impaired blood supply to the bone tissue causing bone necrosis. Often, osteoradionecrosis is clinically expressed in relation to surgery or a similar tissue trauma, but may also arise spontaneously several years after radiation treatment. Oral rehabilitation is difficult in these patients as denture wear is painful. Furthermore, ...

Radiotherapy has been an indispensable primary treatment or adjuvant treatment for malignant oral and maxillofacial tumours. Although it plays an ameliorative role in local control of the tumour and in improving the survival rate, late adverse effects such as osteoradionecrosis of the jaw (ORNJ) cannot always be avoided and may have a tremendous psychological and physical impact on the patient. Regaud first reported ORNJ in 1922, and a great deal of attention has since been paid to this clinical challenge. In 1983, Marx defined ORNJ as an area greater than 1 cm of exposed bone in a field of irradiation that had failed to show any evidence of healing for at least six months. Since then, several different theories and definitions relating to ORNJ have been proposed by various researchers. Despite the lack of a standard and unified definition, ORNJ is characterized by different extents of bone necrosis combined with soft tissue defects. These varying degrees of bone destruction make it essential ...
Osteoradionecrosis (ORN) is a complication that occurs after radiotherapy for head or neck malignancies. ORN of the spine is rare, with only few cases affecting the cervical spine reported to date. To our knowledge, no case of lumbar ORN has been reported. We report a rare case of ORN in the lumbar spine that occurred 2 years after radiotherapy and perform a literature review. We present a case of lumbar ORN that occurred 2 years after radiotherapy for gallbladder carcinoma. The patient was successfully treated conservatively and followed up for | 10 years. ORN of the spine is a rare complication of radiotherapy. Spinal ORN is clinically described as a chronic disease with a slow onset. The most common presenting symptom of spinal ORN is pain. However, as ORN progresses, spinal kyphosis and instability can lead to neurological compression and thus to induced myelopathy or radiculopathy. Treatment of spinal ORN is comprehensive, including orthosis, medication, hyperbaric oxygen therapy, surgery, and new
The implementation of radiotherapy in the multimodal treatment of advanced head and neck cancer has greatly improved survival rates. In some patients, however, this benefit comes at the potential expense of the tissue surrounding the primary site of malignancy. Osteoradionecrosis (ORN) of the facial bones, in particular the maxilla, is a debilitating complication of radiation therapy. Exposure to ionizing radiation results in devitalization of underlying bone with necrosis of adjacent soft tissue. Controversy surrounding appropriate early intervention in ORN persists and no consensus for clinical treatment has been established. In the present article, we review the pathophysiology of maxillary ORN and discuss the role of both conservative medical therapy and reconstruction.
