Inclusion criteria were severe wheezing, classified as a score of ≥6 points as assessed by the Respiratory Distress Assessment Instrument (RDAI) after initial treatment with salbutamol, and the symptoms of acute viral infection. This study was conducted to determine maternal and neonatal outcome of patients exposed to prolonged tocolytic magnesium sulfate. Methods. A retrospective review of maternal and neonatal charts (1995-2003) of pregnancies that received tocolytic magnesium sulfate. Cases who received magnesium sulfate ,48 h (n=78) were compared to controls who received it ≤48 h (n=77) for maternal side effects and neonatal outcome. Results. Cases were more likely to be nulliparous (55.1% versus 37.7%, p=0.044), multiple gestations (33.3% versus 10.4%, p=0.001), and of lower gestational age (29.0±3.9 versus 30.5±3.8 weeks, p=0.017) compared to controls. The purpose of this study was to evaluate the fetal cardiovascular function during prolonged magnesium sulfate tocolysis. We performed a fetal ultrasonographic examination in 15 patients (Mg group) during magnesium sulfate tocolysis for the treatment of preterm labor. The maternal serum magnesium concentration was 5.7 ± 0.5 mg/dl at the time of the examination. Sixteen fetuses in normal pregnancies at similar gestational ages were used as the control group. The fetal heart rate and the middle cerebral artery pulsatility index in the Mg group were lower than in the control group (p , 0.01). This study was conducted at the University of Birmingham. 19 participants bathed in a bath with epsom salts for one week and had serum and urine sulfate and magnesium levels measured before and after the week of magnesium sulfate baths. Following 7 days of baths 17 participants experienced a raise in serum magnesium levels. The 2 participants that did not experience a serum raise in magnesium levels did experience an increase in urine magnesium levels. The study suggested this could be due to magnesium being absorbed and excreted due to serum magnesium levels already being sufficient. Similar results were found with regard to sulfate, with both serum and urine sulfate levels higher following 7 days of baths. The onset of eclamptic convulsions can be antepartum (38-53%), intrapartum (18-36%), or postpartum (11-44%). Recent data reveal an increase in the proportion of women who develop eclampsia beyond 48 hours after delivery. Other than early detection of preeclampsia, there are no reliable tests or symptoms for predicting the development of eclampsia. In developed countries, the majority of cases reported in recent series are considered unpreventable. Several trials have shown that converting enzyme inhibitors (CEI) reduce mortality in patients with heart failure. More recent studies have tested CEIs after acute MI. In the CONSENSUS II trial (1), enalapril was begun intravenously and a nonsignificant higher mortality rate was found, whereas investigators of the more recent GISSI-3 (2) found that oral lisinopril significantly reduced 6-week mortality. Now, the ISIS-4 investigators have also found a significant lower mortality rate at 5 weeks with the use of oral captopril. Hypotension was more common among patients treated with CEIs in each trial. The findings suggest that CEIs begun soon after onset of MI reduce short-term mortality, although care is necessary in starting therapy, especially in patients with systolic blood pressure , 100 mm Hg.
