Almost all melanomas with popliteal drainage also had concurrent inguinal drainage. Several studies have explained at least three distinct patterns of lymphatic drainage to the popliteal basin. First, the popliteal basin is an in-transit node, so the single lymphatic channels drain serially to the popliteal area and continue to the inguinal region. In the second pattern, both popliteal and inguinal basins are first-order sentinel node, so two or more lymphatic channels drain into each basin. In the third pattern, the serial and parallel drainage are mixed, so the inguinal basin can be a first- or second-order basin [6,7]. Introduction: In the current study we evaluated the incremental value of lateral pelvic lymphoscintigraphy imaging of endometrial or cervical cancer patients who underwent sentinel node mapping. Methods: Operable endometrial and cervical cancer patients without clinical or paraclinical evidence of lymph node involvement were included in the study. The day before surgery the patients were sent to the nuclear medicine department for injection of the radiotracer. All patients received two intra-cervical injection of 1 mCi/0.2 cc radiotracer in the 6 and 12 hour locations. 18-24 hours after the radiotracer, lymphoscintigraphy imaging in anterior/posterior and lateral views was done. After induction of anesthesia, 2 mL Methylene blue in two aliquots was injected intra-cervically in the same location as the radiotracers. The SLNB technique is now widely used in numerous medical institutions worldwide and has become the standard of care to reduce upper limb morbidities commonly encountered following axillary lymph node dissection, including lymphoedema, shoulder stiffness and chronic pain (7,8). Qualitative analysis of lymphoscintigrams is subject to wide variability and may miss subtle differences in ilioinguinal uptake between normal and abnormal limbs. This study compared quantitative analysis to qualitative analysis of lower‑limb lymphoscintigraphy in diagnosing lymphedema. Fifty‑two lymphoscintigrams performed using standardized protocol, 99‑metastable technetium nanocolloid intradermal injection at the first interdigital space, were analyzed quantitatively. Fifty‑three normal and 51 abnormal limbs were analyzed. For each limb, a region of interest (ROI) was drawn around the injection site, and ilioinguinal nodes on the 1.5 h static images and the counts in these ROIs were recorded. BACKGROUND: Lymphedema is a chronic disease causing significant morbidity for affected patients. It can be difficult to diagnose, and patients are often frustrated by multiple referrals and inadequate therapies. Centralized, comprehensive care for the patient presenting with lymphedema or other causes of localized swelling allows for appropriate evaluation and provides improved management and treatment. METHODS: A 4-year review of the first 100 patients seen at the Beth Israel Deaconess Medical Center Lymphedema Clinic was performed. On the basis of the clinical experience from these patients, an algorithm for diagnosing and managing patients with localized swelling was developed. RESULTS: The mean age of the patients was 50 years, and 81 percent of the patients were women. On average, patients had experienced their symptoms for 11.6 years (range, 3 months to 60 years). I am not certain which type of hibiscus you are growing, if it is the hardy shrub Rose of Sharon type (Hibiscus syriacus) or if it is one of the herbaceous perennials such as Lord Baltimore but these plants both start to grow fairly late in the season so it may be weather related with the one in the warmest spot starting first. Wetter soil can also keep the soil colder longer into the spring so the drainage pattern could also affect when they start. The shrub hibiscus is particularly sensitive to excess wetness during the winter, so this might have caused some problems for them this year, either with the rainstorms or if there were any changes to the drainage patterns in your yard.
