This study aims to explore the effects of Low Level Laser Therapy (LLLT) on breast cancer-related lymphedema when utilized in conjunction with Complex Decongestive Therapy (CDT) when compared to CDT treatment alone. The investigators hypothesize that the addition of LLLT to CDT will result in statistically significant improvements and greater long-term benefits as measured by changes in arm volume and girth, pain levels, tissue texture, symptoms experienced, and quality of life when compared to the benefits of CDT alone for the treatment of breast cancer-related lymphedema ...
Background Lymphedema is a common consequence of breast cancer treatment requiring life-long treatment to reduce symptoms and prevent complications. Evidence to inform the optimal decongestive lymphedema treatment package is lacking.. Objective To identify the effect of decongestive lymphedema treatment on excess arm volume or patient-centered outcomes for women presenting within either 12 months or mean 9 months of developing arm lymphedema following breast cancer treatment.. Inclusion criteria Types of participants: women who received lymphedema treatment within either 12 months or mean 9 months of developing unilateral breast cancer-related arm lymphedema. Types of intervention: any decongestive lymphedema treatment delivered with the purpose of reducing arm lymphedema, compared to another form of lymphedema treatment (whether self or practitioner administered), placebo or no treatment. Types of outcomes: clinical outcome was excess arm volume; patient-centered outcomes were health related ...
Physiotherapy treatments for breast cancer-related lymphedema: a literature review.. Leal NF, Carrara HH, Vieira KF, Ferreira CH. Universidade de Sao Paulo, Brazil. [email protected]. Breast cancer is the second most frequent cancer among women. Surgery is part of the therapeutic process to prevent metastases, but it can also cause some complications, including lymphedema. Physiotherapy contributes to its treatment, using different techniques that have been developed over the years. This systematic literature review aims to present physiotherapy modalities applied for lymphedema therapy. The literature review was conducted using textbooks and Lilacs, Pubmed and Scielo databases, from 1951 to 2009. Physiotherapy resources used for lymphedema treatment include complex decongestive therapy (CDT), pneumatic compression (PC), high voltage electrical stimulation (HVES) and laser therapy. The analyzed literature shows that better results are obtained with combined techniques. CDT is the most used ...
TY - JOUR. T1 - Lower extremity lymphedema. Update. T2 - Pathophysiology, diagnosis, and treatment guidelines. AU - Kerchner, Katherine. AU - Fleischer, Anne. AU - Yosipovitch, Gil. PY - 2008/8/1. Y1 - 2008/8/1. N2 - Lower extremity lymphedema is an important medical issue which causes morbidity and is frequently seen by dermatologists. The subject has not been adequately addressed in dermatologic literature for many years. Primary lymphedema is caused by an inherent malfunction of the lymph-carrying channel, in which no direct outside cause can be found. Secondary lymphedema is caused by an outside force, such as tumors, scar tissue after radiation, or removal of lymph nodes, which results in dysfunction of the lymph-carrying channels. Treatment is based on rerouting the lymph fluid through remaining functional lymph vessels. This is accomplished through elevation, exercises, compression garments/devices, manual lymph drainage, and treatment is combined with good skin care practices.. AB - ...

Primary lymphedema is caused by developmental and functional defects of the lymphatic vascular system that result in accumulation of protein-rich fluid in tissues, resulting in edema. The 28 currently known genes causing primary lymphedema can explain ,30% of cases. Angiopoietin 1 (ANGPT1) and ANGPT2 function via the TIE1-TIE2 (tyrosine kinase with immunoglobulin-like and epidermal growth factor-like domains 1 and 2) receptor complex and α5β1 integrin to form an endothelial cell signaling pathway that is critical for blood and lymphatic vessel formation and remodeling during embryonic development, as well as for homeostasis of the mature vasculature. By screening a cohort of 543 individuals affected by primary lymphedema, we identified one heterozygous de novo ANGPT2 whole-gene deletion and four heterozygous ANGPT2 missense mutations. Functional analyses revealed three missense mutations that resulted in decreased ANGPT2 secretion and inhibited the secretion of wild-type (WT)-ANGPT2, ...

Several months ago one of our readers asked whether pregnancy caused lymphedema to get worse. I reviewed the published literature and, as is too often the case, found very little published information. In addition, what information was available was based on very limited numbers of patients. To gain additional understanding about pregnancy and lymphedema, I created a pregnancy survey for our readers. 13 women have responded to that survey and I am including a summary of their responses and insights. 12 of the 13 women had primary lymphedema. The average age of onset was 10 years of age and the range was from 1 to 16 years of age. 1 patient had breast cancer and a mastectomy and developed lymphedema during her first pregnancy 9 years later. Of the 12 patients with primary lymphedema, 9 of 12 (75%) had the onset of lymphedema or developed worse lymphedema during their pregnancy. Most of these women found that the lymphedema got worse in the final months of their pregnancy. 2 women reported having ...

Whenever the normal drainage pattern in the lymph nodes is disturbed or damaged (as often happens during surgery to remove the lymph nodes), severe swelling of the arm may occur. Radiation may also damage lymph nodes and cause swelling of the arm. This swelling, caused by an abnormal collection of too much fluid, is called lymphedema. The swelling can be noticed in the arm, chest, and breast area on the side of surgery.. When the lymph nodes under the arm have been removed, a woman is at higher risk of lymphedema for the rest of her life. Lymphedema may occur immediately following surgery, or months or years later. Not every woman who has a mastectomy will experience lymphedema.. There are several types of lymphedema. The acute, temporary, and mild type of lymphedema occurs within a few days after surgery and usually lasts a short period of time. The acute and more painful type of lymphedema can occur about four to six weeks following surgery. However, the most common type of lymphedema is slow ...

Lymphedema remains a poorly understood entity that can occur after lymphadenectomy. Herein, we will review the pathogenesis of lymphedema, diagnostic modalities and the natural history of extremity involvement. We will review the incidence of upper extremity lymphedema in patients treated for breast malignancies and lower extremity lymphedema in those treated for gynecologic malignancy. Finally, we will review traditional treatment modalities for lymphedema, as well as introduce new surgical treatment modalities that are under active investigation.
The Lymphedema program at Mt. Ascutney Hospital offers patients treatment to reduce or eliminate swelling and improve function. Mt. Ascutney Hospital has therapists trained in the Vodder method of Manual Lymph Drainage and Combined Decongestive Therapy, a special therapeutic massage technique used to treat swelling associated with Lymphedema. (vodderschool.com). The massage is performed in a gentle, rhythmic, precise and sequential manner. Following the manual treatment, compression bandages or garments are often applied to prevent further swelling.. Lympedema treatment is covered under most insurance plans with a physicians referral. The cost of the compression garments are not always covered, but there are some less expensive alternatives and some funding options to explore if compression garments are recommended by the treating therapist.. Lymphedema treatment is not for people who have congestive heart failure that is not managed by medications, for swelling due to congestive heart failure, ...

Lymphedema refers to the abnormal accumulation of protein-rich interstitial fluid secondary to lymphatic dysfunction. It may be primary, referring to a developmental abnormality of the lymphatic system, or secondary (acquired). Primary lymphedema is rare in children, with secondary lymphedema being far more common. Primary forms of lymphedema are categorized by age of onset. These include congenital (onset before age 1 year), lymphedema praecox (onset near puberty), and lymphedema tarda (onset after age 30 ...
In most cases, the diagnosis of lower limb lymphedema should be quite straightforward, and treatment interventions, particularly if undertaken early in the course of the disease, effectively stabilize the affected limb and produce significant improvement in limb size and function. Nevertheless, patients with swollen limbs continue to suffer from the persistent limited motivation of healthcare professionals to recognize and treat the problem. The published medical literature documents that, in cancer-associated lymphedema, patients encounter delays in diagnosis, barriers in the access to treatment, and conflicting advice on the management of their lymphedema. In general, treatment of lower limb lymphedema, when it is recognized and evaluated, receives treatment at a relatively advanced stage. The lay public awareness of lymphedema of the leg lags behind its recognition of breast cancer-associated lymphedema of the upper limb. In cancer treatment centers, healthcare professionals increasingly ...

Manual lymphatic drainage (MLD), sometimes called manual lymphatic therapy, uses light touch to move excess lymph and fluid out of the tissues and back into the lymphatic vessels. Although often referred to as a type of massage, MLD is very different from traditional forms of massage that rely on deep and rigorous rubbing. MLD is much gentler and should feel as if the skin is being brushed, not rubbed or kneaded. One session of MLD typically takes about 20-45 minutes.. Often, MLD is recommended as a component of the treatment plan known as complete decongestive therapy (CDT), although it may be used in combination with other treatments. Research studies havent clearly proven the effectiveness of MLD, but they have shown that CDT is effective - and CDT usually includes MLD. Many insurance plans cover MLD because its done during an office visit with your lymphedema therapist. However, youll need to check the details of your own insurance plan.. It often takes many hours of training in MLD, ...

Lymphedema is characterized by swelling of subcutaneous tissues due to the accumulation of excessive lymph fluid resulting from impairment of the normal clearing function of the lymphatic system and/or from an excessive production of lymph. Lymphedema is divided into two broad classes according to etiology. Primary lymphedema is a relatively uncommon, chronic condition due to congenital absence of lymph vessels and nodes, and may be due to Milroys Disease. Secondary lymphedema, which is much more common, results from the destruction or damage of formerly functioning lymphatic channels. Examples include radical surgical procedures with removal of regional groups of lymph nodes (for example, after radical mastectomy), post-radiation fibrosis, and spread of malignant tumors to regional lymph nodes with lymphatic obstruction. Treatment for lymphedema may include mechanical measures (for example, compression garments, bandaging, manual massage, pneumatic compression devices), drugs, and in rare ...
Secondary lymphedema associated with cancer therapy occurs in about 30% of breast cancer survivors, in addition to those treated for other malignancies. Lymphedema arises from lymphatic vessel injury and dysfunction, and patients afflicted with lymphedema suffer from swelling as a result of progressive functional impairment of their diseased limbs. Lymphedema causes a variety of morbidities that affect patient quality of life including physical discomfort, disfigurement, sensory deficits, recurrent inflammation, fibrosis, and impaired wound healing. The onset of lymphedema is often delayed by several years after cancer treatment, and though there is increasing evidence that chronic inflammation contributes to the pathogenesis, currently little is known about the mechanisms that underlie lymphedema development and no curative therapies exist.. To investigate the mechanisms involved in the development and progression of lymphedema, Gousopoulos et al. used a surgical mouse tail lymphedema model and ...
RESULTS: Mean initial lymphedema volume was 709 mL, and the percentage of lymphedema was 31%. The induction phase of CDT reduced this to 473 mL and 18%, respectively. Before therapy, 76 patients had chronic pain and 41 required oral pain medication. CDT reduced this to 20 and 11, respectively. The degree of pain was also assessed on a numerical scale from 0 to 10. Those patients with chronic pain initially rated their pain at an average of 6.9. After treatment, this was reduced to 1.1 ...
If the lymphatic system is not functioning properly this will lead to swelling, or fluid retention of the extremity. The question is then why does this occur? The most likely causes are family history, also known as primary lymphedema which often presents either at birth or at the onset of puberty (lymphedema praecox), or at later adult life (lymphedema tarda). It may affect both legs, or arms, but very often only one leg or arm is affected. It also may occur as a result of an injury to the lymphatic system, also known as secondary lymphedema, or perhaps as a result of a compressive force on the extremity.. It is often the result of a lymph node dissection during surgery, or as a secondary reaction to radiation therapy. This type of lymphedema may present several years after the initiating event.. Accidents, disease process e.g. cellulitis, or fungi will inhibit the lymphatics system ability to function properly, may overwhelm the main lymphatic channels, and as result the affected extremity ...
Nicole L. Gergich MPT, MLD/CDT Lymphedema Specialist, Penn Therapy and Fitness Posting Date: May 6, 2001 Last Modified: January 3, 2002 Why Should I Exercise? One very important component of a comprehensive treatment plan for cancer-related lymphedema is exercise. A program consisting of flexibility, strengthening and aerobic exercise is beneficial in reducing lymphedema when administered under the correct conditions. Exercise also allows cancer survivors a more active role in their own lymphedema management. Recent studies have shown no significant increase in the incidence of lymphedema after breast cancer, between women participating in an exercise program when compared to women who did not exercise. What Type of Exercises are Helpful To Someone with Lymphedema? Flexibility exercises help to maintain joint range of motion and allow for elongation or stretching of tissues. Flexibility exercises also help to prevent joint stiffness and postural changes after cancer surgeries or treatments. ...

ISSN 0365-0596. http://dx.doi.org/10.1590/S0365-05962012000600013.. Chronic lymphedema presents as interstitial fluid retention due to a failure in the lymphatic system drainage. The affected region becomes more vulnerable immunologically and predisposed to the onset of neoplasms. Basal Cell Carcinoma is the most common sort of neoplasm, nevertheless it rarely metastisizes. Sarcomas are malignant mesenchymal neoplasms, locally aggressive, which can spread. Here is reported an infrequent case of multiple basal cell carcinoma, synchronous to a poorly differentiated pleomorphic sarcoma, both spreading to lymph nodes and arising from tissue compromised by chronic lymphedema.. It is most frequently seen after lymph node dissection, surgery and/or radiation therapy, in which damage to the lymphatic system is caused during the treatment of cancer, most notably breast cancer. In many patients with cancer, this condition does not develop until months or even years after therapy has concluded. Lymphedema may also be associated with accidents or certain diseases or problems that may inhibit the lymphatic system from functioning properly.[5] In tropical areas of the world, a common cause of secondary lymphedema is filariasis, a parasitic infection. It can also be caused by damage to the lymphatic system from infections such as cellulitis.[citation needed]. Primary lymphedema may be congenital or arise sporadically. Multiple syndromes are associated with primary lymphedema, including Turner syndrome, Milroys disease, and Klippel-Trenaunay-Weber syndrome. It is generally thought to occur as a result of absent or malformed lymph nodes and/or lymphatic channels. Lymphedema may ...
Lipedema is a disproportional obesity featuring frequent hematoma formation due to even minor traumatic injuries. On the basis of clinical observations, complete decongestive physiotherapy diminishes the incidence of hematomas due to minor injuries beyond leg volume reduction. Hematoma development may be caused by altered capillary resistance (CR) or altered capillary fragility (CF). We measured capillary fragility (CF) before and after complex decongestive physiotherapy (CDP) to examine, whether CDP could reduce CF. 38 women with lipedema were included in the study. Twenty-one (21) patients were treated with CDP and 17 using exclusively moisturizers as the control group. CDP comprised once daily manual lymph drainage, intermittent pneumatic compression and multilayered short-stretch bandaging performed throughout a 5-day-course. CF was evaluated with the vacuum suction method (VSM) using Parrots angiosterrometer in both groups. Decongestive therapy resulted in a significant reduction of the ...
Breast cancer related upper extremity lymphedema (BCRL) reduces the quality of life of those who have had surgery for breast cancer. The aim of this study is to evaluate the risk factors for BCRL and determine whether immediate autologous tissue breast reconstruction is one of them. A case control study was conducted comparing patients with BCRL (n=97) to surgically treated breast cancer patients without BCRL (control, n=126). The groups were matched for age, type of breast surgery and radiation therapy. Postoperative upper extremity infection, body mass index (BMI), occupation (level of hand-use), and immediate autologous tissue breast reconstruction were investigated as a risk factor of BCRL. Mastectomy was performed on 47.6 % (n=60) and 37.2% (n=36) of patients in the control and the BCRL groups, respectively. Eight patients (13.3%) had immediate autologous tissue breast reconstruction in the control mastectomy group. Six of 36 BCRL patients (16.7%) underwent mastectomy with immediate ...

Surgical Treatment Approaches. If significant symptoms persist despite good adherence to conservative management, evaluation at a lymphatic surgical center can be considered. Lymphatic to venous anastomoses (LVA, or lympho-venous bypass) and vascularized lymph node transfer (VLNT) are the two functional/physiological surgeries used for treatment.25. LVA creates shunts for lymphatic fluid to utilize the venous system as a primary transport back to the heart and bypass potential obstruction, like the axilla in patients with prior breast cancer treatment. VLNT involves an autologous transplant of a pedicle of lymph nodes (deep inferior epigastric artery perforator and superficial inguinal lymph node flaps are commonly used for breast cancer-related lymphedema) to the affected limb. The exact mechanism by which lymphedema is improved with this method is unclear, however, hypotheses include stimulation of lymphangiogenesis with drainage via the implanted pedicle or development of new lymphatic ...

Abstract. Lymphatic filariasis (LF) is a vector-borne parasitic disease that can clinically manifest as disabling lymphedema. Although the LF elimination program aims to reduce disability and to interrupt transmission, there has been a scarcity of disease morbidity management programs, particularly on a national scale. This report describes the implementation of the first nationwide LF lymphedema management program. The program, which was initiated in Togo in 2007, focuses on patient behavioral change. Its goal is two-fold: to achieve a sustainable program on a national-scale, and to serve as a model for other countries. The program has five major components: 1) train at least one health staff in lymphedema care in each health facility in Togo; 2) inform people with a swollen leg that care is available at their dispensary; 3) train patients on self-care; 4) provide a support system to motivate patients to continue self-care by training community health workers or family members and providing in home
Data regarding patients ages, duration of the disease, etiology of the penoscrotal lymphedema, and follow-up period are shown in Table 1.. The patients were followed postoperatively for a minimum 6-month and maximum 6-year period, during which only 1 patient who had undergone surgery with inguinal lymphadenectomy and radiation therapy because of penile cancer had recurrent scrotum lymphedema. He underwent a reoperation 2 years after the previous procedure and had a new recurrence, making a reintervention necessary. The patient had a favorable outcome in the long-term follow-up.. Although this surgical site is subject to contamination because of neighboring structures, no infections, dehiscence, or necrosis was observed, probably thanks to technical care and proper local hygiene.. DISCUSSION. Penoscrotal lymphedema is a condition leading to the progressive enlargement of the scrotum and penis causing significant discomfort for patients. In addition to the grotesque aspect, the progression of the ...

In healthy adults the lymphatic system is stable when homeostasis is maintained and lymphangiogenesis, or expansion of the lymphatic network occurs in response to a variety of circumstances including acute inflammation, tissue repair, and around some tumors [10,15].. Several signaling pathways have prolymphangiogenic effects. However, the vast majority of studies aiming to treat lymphedema in preclinical models have focused on the VEGF-C/VEGFR-3 signaling pathway. VEGF-C stimulation regulates lymphatic endothelial cell proliferation, differentiation, tubule formation, and migration [1,10]. Over-expression of VEGF-C in transgenic mice results in lymphangiogenesis and expansion of the superficial lymphatic system. These lymphatic vessels are functional and can induce physiologic changes such as decreasing the severity of acute skin inflammation and edema in response to inflammatory stimuli [16,17], suggesting that VEGF-C probably can be exploited as an ideal therapeutic target for lymphedema. ...

What is Lymphedema?. The term edema refers to the accumulation of an excessive amount of fluid in cells, tissues, or organs and is clinically characterized by swelling. A variety of conditions can produce edema including chronic venous insufficiency, infection, and trauma. Lymphedema is a progressive condition that occurs when the lymphatic transport system falls below the capacity needed to handle the fluids that normally leak from the blood vessels into the lymph system. The accumulation of fluids leads to the swelling of subcutaneous tissue as a result of obstruction of lymphatic vessels or lymph nodes. Any disease process that interferes with or disrupts the normal mechanical process by which the lymphatic system collects, filters, and returns lymph fluid back into the circulatory system can result in excessive accumulation of lymph fluid in the area affected by the blockage. Since the arterial capillaries continue to function normally and deliver fluid to the tissue, the volume of fluid ...

ABSTRACT Stewart-Treves syndrome (STS) is defined as a lymphangiosarcoma in a setting of postmastectomy upper extremity lymphedema. On the other hand, positivity of nontreponemal and treponemal syphilis tests can occur in some healthy conditions and diseases besides syphilis. Here we report a case of chronic lymphedema that progressed to STS in a 68-year old woman with a
MLD is a very light rhythmic technique that induces relaxation and reduces edema. Edema can occur anywhere in the body and is not always as obvious as swollen ankles at the end of a long day. Micro edemas in the body can cause the body to work harder to stay healthy. This is stressful for the body. Not only does MLD reduce this edema, it also helps the body relax. It feels good and it is good for you. MLD is different than massage. It has a much lighter touch yet has profound effects. Sometimes less really is more.. MLD has a cumulative benefit. In order to truly realize the benefits of MLD, sessions are usually scheduled close together for the first few weeks. Once benefits are sustainable, sessions are typically less frequent. ...
