Twenty-six patients participated in a randomized, double-blind study of the efficacy of total lymphoid irradiation in the treatment of intractable rheumatoid arthritis. All 26 patients, for whom therapy with gold compounds and penicillamine had failed, would ordinarily have been considered candidates for cytotoxic or antimetabolite drug therapy. Thirteen patients randomly assigned to receive full-dose total lymphoid irradiation (2000 rad) and 11 patients assigned to receive control low-dose total lymphoid irradiation (200 rad) completed radiotherapy. Alleviation of joint disease activity was significantly greater in the high-dose group as judged by morning stiffness, joint tenderness, and functional assessment (global composite score) at 3 and 6 months after radiotherapy. The high-dose group had a marked reduction in both T-lymphocyte function and numbers, but this finding was not observed in the low-dose group. Complications seen in the high-dose but not low-dose group included transient ...
The ability of preoperative total lymphoid irradiation (TLI) to reduce the need for chronic immunosuppression after cadaveric renal transplantation was examined in 25 recipients who were given a brief course of antithymocyte globulin (ATG) postoperatively with daily low-dose prednisone (0.1-0.2 mg/kg) as the sole maintenance immunosuppressive drug. Patients were selected for the study on the basis of their low levels of cytotoxic antibodies. Grafts were not HLA-matched, and the mean interval between completion of TLI and transplantation was 9 days. During an observation period of up to 25 months, 2 grafts were lost because of rejection. There were two deaths due to disseminated viral infections and two to late cardiovascular complications. At the last observation point, the mean serum creatinine of the 19 patients with functioning grafts was 1.5 mg/dl, and the mean dose of prednisone was 10.2 mg/day. 10 of these patients did not have a rejection episode. Comparison of patients given TLI with a ...

One approach to limit the toxicity of allogeneic transplantation has been the use of nonmyeloablative regimens preceding the infusion of allogeneic cells. In this strategy, patients receive immunosuppressive therapy that allows for the engraftment of donor cells without the immediate eradication of patient hematopoiesis. The primary mechanism by which the underlying disease is eradicated is not through chemotherapy mediated cytoreduction, but rather through the donor lymphocyte mediated graft versus tumor effect. As a result, patients experience far less regimen related toxicity. Therefore, the adoption of this strategy may allow for the use of allogeneic transplantation in disease settings and patient populations for which it had not been readily applicable in the past.. Over the past several years, the use nonmyeloablative transplant has rapidly expanded.Several reduced intensity conditioning regimens have been developed including fludarabine and cyclophosphamide; fludarabine and melphalan; ...

One hundred thirteen patients with cervical metastases from a squamous cell carcinoma and no evidence of the primary tumor were treated for cure by surgery and routine large-field postoperative irradiation. Patients were staged according to the 1987 American Joint Committee on Cancer (AJCC) classification. There were 24 N1, 54 N2, 29 N3, and 6 Nx lesions. One hundred four patients underwent cervical lymph node dissection and 9 had adenectomy. All patients received postoperative external beam therapy to the entire naso-oro-pharyngo-larynx and all cervical lymphatics. The overall nodal failure rate was 13.7%. Nodal failure was significantly correlated with N staging (P = .01) and with the number of histologically involved nodes (P = .05). NOdal failure was 21% when nodes were initially fixed versus 7.5% when they were not (P = .07) and 18% when there was extracapsular spread versus 4.3% when the capsule was intact (P = .11). Eleven patients (9.7%) developed a subsequent primary lesion. In 3 ...
Radiotherapy could function as immunosuppressive agent for allotransplantation due to the ability of ionizing radiation to cause cell death, the technique could be applied to irradiate total body, bone marrow and/or lymphoid tissues of the recipient who receive an allograft as an immunosuppressant to improve the success rate of transplantation. From the past to the present, the radiation techniques in allotransplantation process have been continuously studied and developed across preclinical and clinical settings, in order to achieve better therapeutic outcomes. Currently, total lymphoid irradiation by fractionation and total body irradiation with low dose are most frequently used radiotherapy techniques in allotransplantation process. The advanced radiotherapy techniques such as total marrow and/or lymphoid irradiation by volumetric arc therapy (VMAT) or helical tomotherapy (HT) have gained interests. These advanced techniques could increase therapeutic ratio by improving conformity high ...
Background: The treatment of Hodgkins disease (HD) at advanced stages relies mainly upon multi-agent chemotherapies (CT), while the role of radiation therapy has not been definitely identified. The aim of this report is to analyze the 10-year results of a prospective study including 133 patients with HD clinical stages (CS) IIIA to IVB treated by three monthly courses of ABVD (adriamycin, bleomycin, vinblastin, and dacarbazine) followed by high-dose subtotal or total lymphoid irradiation [(S)TLI].. Patients and methods: From 1 October 1981 to 30 September 1988, 133 adult patients with HD CS IIIA (45), IIIB (33), IVA (seven) and IVB (48) were entered in the non-randomized multicentric prospective trial POF81/34. The number of involved nodal areas (NINA), and the number of visceral sites (NVIS) involved were registered in all patients; patients with bulky mediastinal tumor (BuMT) (mediastinal mass ratio ≥0.45) were also identified. All patients received three monthly cycles of ABVD. Patients in ...
Many patients lack a fully HLA-matched donor for hematopoietic cell transplantation (HCT), and HLA mismatch is typically associated with inferior outcomes. Total lymphoid irradiation and antithymocyte globulin (TLI-ATG) is a nonmyeloablative conditioning regimen that is protective against graft-versus-host disease (GVHD), and we hypothesized that the protective effect would extend beyond HLA-matched donors. We report outcomes for all consecutively transplanted patients at Stanford University from December 2001 through May 2015 who received TLI-ATG conditioning and HCTs from 8 to 9 out of 10 HLA-mismatched unrelated donors (MMUDs, N = 72) compared with 10 out of 10 HLA-matched unrelated donors (MUDs, N = 193 ...
We herein describe the time-dependent development of cytogenetic damage induced in skin fibroblasts after conditioning with high-dose chemotherapy and TBI and allogeneic SCT in a larger cohort of patients with a long term follow-up.. The percentage of cells with an aberrant karyotype in our study was significantly higher at all time points after high-dose conditioning compared to the biopsies taken before conditioning. Genetic damage occurred early, because most metaphases from biopsies taken 3 months after SCT already presented an aberrant karyotype.. It is unlikely, that all these aberrations developed ex vivo in culture, because most skin metaphases were normal before SCT. In the literature there is only one small series of four pediatric patients after TBI or total lymphoid irradiation, where authors found 49 to 88 % aberrant metaphases in the skin biopsies taken within radiotherapy fields [19]. A few case reports have also been published describing chromosomal aberrations in skin ...

Inhibition of T lymphocyte activation by suppressor cells found in primary and secondary lymphoid organs as well as in peripheral tissues has been documented in normal and pathological situations, such as tumor growth, overwhelming infections with microbes, parasites and protozoa, immunization with powerful recombinant vaccines, treatment with cyclophosphamide, following total lymphoid irradiation, during graft-vs-host disease, in pregnancy, and in normal adult bone marrow or fetal and newborn tissues (1, 2, 4, 5, 22, 23, 24). We and others have shown that suppressor cells can be identified as CD11b+/Gr-1+/CD31+ immature myeloid cells that inhibit T lymphocyte functions in seemingly unrelated conditions that all lead to immunosuppression in mice (1, 2, 5, 25). To investigate the common immunosuppressive mechanisms, we have generated, by means of retrovirus-mediated immortalization, two myeloid lines that provide a continuous source of homogeneous suppressor cells. These lines were apparently ...

The next proposed course of action is a round or so of rituximab (the drug that worries me so much), along with total lymphoid irradiation and possibly cytoxan administered intravenously. We dont know if plasmapheresis and IVIG are still to be part of the mix, or if theyve been relegated to the scrap heap of ineffective treatments. For the first time in some time, we are again hearing murmurs that some form of external assist device might become necessary at some point. Sorry for all the medical-speak -- its virtually impossible to explain these things any more easily. Suffice it to say that Patty has worked her way, without real success, through most of the traditional solutions to the antibodies issue, and is now trying lesser-tested approaches in the hope something tips the scales in her favor ...
From August 2014 until August 2015, IORT was considered in 124 patients; 82 patients actually underwent IORT and were enrolled in the phase 2 study. Among the 42 patients who did not undergo IORT, the reasons of IORT cancellation were either preoperative decisions in multidisciplinary conference, including presence of a huge primary mass or multicentric tumor nodules (n = 4); neoadjuvant chemotherapy (n = 3); definitive axillary, supraclavicular, or internal mammary lymph node metastasis (n = 3); and tumor close to the skin (n = 9); and intraoperative decisions including persistent positive resection margins (n = 9), conversion to total mastectomy (n = 8), and shape of tumor cavity unsuitable for IORT (n = 6). Among the 82 patients treated with IORT, 55 patients who underwent a minimum follow-up period of 1 month after WBI were included in the analysis. Follow-up after IORT ranged from 12.7 weeks to 50.3 weeks with a median follow-up of 25.9 weeks. The median patient age was 52 years (range, 32 ...
Please notice, that 4.7 lymph nodes were identified per patient, on average. Its not a lot, by which I mean, NORMAL people and breast cancer patients, have less than 5 internal mammary lymph nodes, in total, in the combined right+left internal mammary regions. Why is this important? Im pointing it out because its quite different from the axillae, where its normal to have 15 to 30 lymph nodes in 1 axilla. That simply isnt the case in the internal mammary region. Nobody knew that, before this study. So I think its a simple item of knowledge to add to our brains.. ...

N2 - Lymphatic spread is an important pathway of progression in non-small-cell lung cancer (NSCLC), along with local spread and distant metastasis. The probability of lymph node (LN) involvement is dependent on the site of the primary tumor, stage, and histology. Elective nodal irradiation (ENI) is the irradiation of clinical and radiological uninvolved LN to account for microscopic tumor invasion in these LNs because we have not been able to determine the extent of LN spread accurately. The clinical value of ENI is uncertain. The impact of ENI is dependent on many (staging-, treatment-, and patient-related) factors. The purpose of this report ...
Abstract: PURPOSE: A retrospective study was performed to evaluate the efficiency and feasibility of twice daily radiation therapy plus concurrent chemotherapy for limited-stage small cell lung cancer in terms of treatment response, survival, patterns of failure, and acute toxicities. MATERIALS AND METHODS: Between February 1993 and October 2002, 76 patients of histologically proven limited-stage small cell lung cancer (LS-SCLC) were treated with twice daily radiation therapy and concurrent chemotherapy. Male was in 84% (64/76), and median age was 57 years (range, 32~75 years). Thoracic radiation therapy consisted of 120 or 150 cGy per fraction, twice a day at least 6 hours apart, 5 days a week. Median total dose was 50.4 Gy (range, 45~51 Gy). Concurrent chemotherapy consisted of CAV (cytoxan 1000 mg/m2, adriamycin 40 mg/m2, vincristine 1 mg/m2) alternating with PE (cisplatin 60 mg/m2, etoposide 100 mg/m2) or PE alone, every 3 weeks. The median cycle of chemotherapy was six (range, 1~9 cycle). ...

To analyze the feasibility of omitting clinical target volume (CTV) for limited small cell lung cancer treated with chemotherapy and intensity modulated radiotherapy. 89 patients were treated from January 1, 2008 to August 31, 2011, 54 cases were irradiated with target volume without CTV, and 35 cases were irradiated with CTV. Both arms were irradiated post chemotherapy tumor extent and omitted elective nodal irradiation; dose prescription was 95% PTV56-63 Gy/28-35 F/5.6-7 weeks. In the arm without CTV and arm with CTV, the local relapse rates were 16.7% and 17.1% (p = 0.586) respectively. In the arm without CTV, of the 9 patients with local relapse, 6 recurred in-field, 2 recurred in margin, 1 recurred out of field. In the arm with CTV, of the 6 patients with local relapse, 4 recurred in-field, 1 recurred in margin, 1 recurred out of field. The distant metastases rates were 42.6% and 51.4% (p = 0.274) respectively. Grade 3-4 hematological toxicity and radiation esophagitis had no statistically
CAMPATH-1 (CDw52) antibodies recognize a very small lipid-anchored glycoprotein that is expressed on the surface of human lymphocytes. They are remarkably lytic with human complement. In addition, CAMPATH-1G (rat IgG2b) and CAMPATH-1H (human IgG1) bind to human Fc receptors and are very effective for cell lysis in vivo. CAMPATH-1M (rat IgM) and CAMPATH-1G have been used to control GVHD and graft rejection in bone marrow transplantation by depletion of the T cells of the donor and recipient. Depletion of donor T cells alone gave excellent control of GVHD but up to 20% of the patients transplanted from HLA-matched siblings, and 51% of those transplanted from nonsibling donors, experienced graft failure caused by immunological rejection. Graft rejection could be partly overcome by additional immunosuppression either with CsA or total lymphoid irradiation (TLI). More effective was the use of CAMPATH-1G in vivo to deplete residual host lymphocytes. Preliminary results from current protocols of antibody

We start working deep in the abdomen. Behind the gut as the central organ - within its lymphatic drainage area - we find a first lymphatic stagnation. Dr. Weiss cultivated the concept of the radixemdema back in 1988 being the central lymphatic stagnation in the dorsal abdomen, which starts to displace the gut. I find a radixedema with 9 out of 10 patients. Keeping parasites in mind is extremly important with any manual intervention in the abdomen. Parasites will be bothered and must be treated appropriately.. Fascial strands proceeding through this stagnated lumbar region become hyoxic and start to contract. We find most disk prolapses and back pain right in this area. Releasing the radixedema in the dorsal abdomen often provides relief for the back.. Scars of the abdomen deteriorate the lymphatic flow additionally … adhesions after appendectomy, cesarean section, hysterectomy, hernia surgery, umbilical hernia or adhesions, piercings must be treated.. With women the stagnation bilds up deep in ...
Due to the specific positioning of the human eyes two slightly different images enter - via the optic nerve - the brain and a three-dimensional image will be formed.. A person fixates point A. Due to the convergence of the eyes this point will be projected onto the area of the highest visual acuity of the retina. Point B lies in greater distance and directly behind Point A for the right eyes view. It will therefore be projected onto the same area as point A; in the left eye, point Bs projection onto the retina will be to the right of point A. The left eye perceives point B in a different, non-corresponding area. The distance point B seems to travel between Bleft and Bright is defined as parallax, the angle as parallactic angle. Retinal disparity is the difference of the distance of two corresponding images on the retina.. ...
If that happens the moving point will reach the stationary point in the next frame.. If there is some sort of goal to be reached, whether it be physical or emotional, theres always a consistency among situations, no matter how different the context- there will, no doubt, be a beginning. Youd be unable to deny this pattern, just as youd never be able to pull off getting from point A to point B( even if using the law of attraction) without first making a move…those first few important strides toward that end of whatever it is. The process of greeting each obstacle, subduing it, getting better at doing it so its a little easier when the bigger obstacles start popping up, repeat, and so on toward the end goal: this is progression. It is a principal that governs most any situation that involves the completion of a task, and one that I find crucial to creating a sound workout plan that does its job. Not just that, but efficiently.. So in order to do this, the idea that every starting point will be different has to be a fact firmly solidified in the mind. Whether creating a ...

The optimal treatment of sinonasal undifferentiated carcinoma (SNUC) remains unclear. We report our results on the outcome and toxicity of patients with SNUC treated by a combined modality and attempt to define the optimal treatment strategies by reviewing the literature. Between 1996 and 2010, 21 consecutive patients with SNUC were treated by any combination of surgery, chemotherapy and radiotherapy. End points were local control (LC), regional control (RC), disease-free (DFS), cause-specific (CSS) overall survival (OS), and late toxicity. Organ preservation was defined as visual preservation without orbital exenteration. After median follow-up of 54 months, the 5-year actuarial rates of LC, RC, DFS, CSS, and OS were 80, 90, 64, 74, and 74 % respectively. On multivariate analysis, T-stage and multimodality treatment approach correlated significantly with LC. Elective nodal irradiation was given to 42 % of high-risk node-negative patients. None of them developed regional failure. The overall ...
Purpose: Hodgkin lymphoma (HL) survivors are at an increased risk of stroke because of carotid artery irradiation. However, for early-stage HL involved node radiation therapy (INRT) reduces the volume of normal tissue exposed to high doses. Here, we evaluate 3-dimensional conformal radiation therapy (3D-CRT), volumetric-modulated arc therapy (VMAT), and proton therapy (PT) delivered as INRT along with the extensive mantle field (MF) by comparing doses to the carotid arteries and corresponding risk estimates. Methods and Materials: We included a cohort of 46 supradiaphragmatic stage I-II classical HL patients. All patients were initially treated with chemotherapy and INRT delivered as 3D-CRT (30 Gy). For each patient, we simulated MF (36 Gy) and INRT plans using VMAT and PT (30 Gy). Linear dose-response curves for the 20-, 25-, and 30-year risk of stroke were derived from published HL data. Risks of stroke with each technique were calculated for all patients. Statistical analyses were performed ...

New Eng J Med 2015r. The EORTC H10 trial was an RCT that assessed PET-directed therapy for EORTC favourable (F) and unfavourable (U) early-stage Hodgkin Lymphoma.r The study aimed to determine if treatment can be de-escalated by omitting INRT in patients with a negative early PET (ePET) after 2 cycles of ABVD. ePET negative was defined as Deauville 1-2. This study also assessed if intensification of treatment with escalated doses of BEACOPPesc chemotherapy after 2 cycles of ABVD would improve outcome in ePET positive patients.. F group patients in the standard arm received one additional cycle ABVD and INRT, regardless of ePET results. F patients in the experimental arm with ePET negative results received an additional 2 cycles of ABVD and no INRT. For F patients with ePET negative results, the 5 year PFS was 99% (standard arm) and 87% (experimental arm). The 5 year OS was 100% (standard arm) and 99.6% (experimental arm).. U group patients in the standard arm received an additional 2 cycles of ...
There is no such thing as distance as we know it. Distance, measured in visual terms, is a halfway point between the physical and the purely, for lack of a better word, immaterial realm. When someone becomes distant, she goes to another location and to do so she figuratively or literally becomes smaller to an observer who remains behind. On our physical material plane, extreme manifestations are named distance and they exist in a perceptibly continuous space. However, the immaterial plane, which intersects with the physical, is made up of what is termed discontinuous space.. Let us say you are at point a with one friend and you want to visit another friend at point b, which lies at the extreme end of this wide, elongated valley. However, you do have a direct line of sight to point b, ...

1. Measure from the outside of your shoulder, to the opposite hip. The measurement should be somewhere between 20-30 inches. 2. Trim your fabric to be exactly 23 wide. Fold the length of fabric in half lengthwise, then in half crosswise. Line up the 2 folded edges on the bottom and left. 3. Add about 4 to your measurement.
