TY - JOUR. T1 - Role of paraventricular nucleus (PVH) in baroreflex-mediated changes in lumbar sympathetic nerve activity and heart rate. AU - Patel, K. P.. AU - Schmid, P. G.. PY - 1988/4. Y1 - 1988/4. N2 - Electrophysiological and neuronanatomical studies indicate that reciprocal connections between the paraventricular nucleus (PVH) and medullary sites are involved in cardiovascular regulation. To determine whether the PVH is involved in the regulation of baroreflex responses, lumbar sympathetic nerve activity (LSNA) and heart rate (HR) changes were recorded in response to increases in arterial pressure (produced by bolus doses of phenylephrine i.v.) prior to, during, and 60 min following the injection of lidocaine (2% lidocaine, 200 nl) bilaterally in the PVH of chloralose-anesthetized rabbits. Baseline blood pressure, HR, and LSNA did not change in response to the administration of lidocaine in the PVH. The magnitude of baroreflex responses in HR and LSNA were expressed as the ratios of ...

The anteroposterior view of the lumbar plain radiograph (AP-LPR) was chosen as the original and first radiographic tool to determine and classify lumbosacral transitional vertebra with morphological abnormality (MA-LSTV) according to the Castellvi classification. However, recent studies found that AP-LPR might not be sufficient to detect or classify MA-LSTV correctly. The present study aims to verify the reliability of AP-LPR on detecting and classifying MA-LSTV types, taking coronal reconstructed CT images (CT-CRIs) as the gold criteria. Patients with suspected MA-LSTVs determined by AP-LPR were initially enrolled. Among them, those who received CT-CRIs were formally enrolled to verify the sensitivity of AP-LPR on detecting and classifying MA-LSTV types according to the Castellvi classification principle. A total of 298 cases were initially enrolled as suspected MA-LSTV, among which 91 cases who received CT-CRIs were enrolled into the final study group. All suspected MA-LSTVs were verified to be real

Because training of the lumbar muscles is a commonly recommended intervention in low back pain (LBP), it is important to clarify whether lumbar muscle atrophy is related to LBP. Fat infiltration seems to be a late stage of muscular degeneration, and can be measured in a non-invasive manner using magnetic resonance imaging. The purpose of this study was to investigate if fat infiltration in the lumbar multifidus muscles (LMM) is associated with LBP in adults and adolescents. In total, 412 adults (40-year-olds) and 442 adolescents (13-year-olds) from the general Danish population participated in this cross-sectional cohort study. People with LBP were identified through questionnaires. Using MRI, fat infiltration of the LMM was visually graded as none, slight or severe. Odds ratios were calculated for both age groups, taking into account sex, body composition and leisure time physical activity for both groups, and physical workload (in adults only) or daily bicycling (in adolescents only). Fat infiltration
Birds are bipedal animals with a center of gravity rostral to the insertion of the hindlimbs. This imposes special demands on keeping balance when moving on the ground. Recently, specializations in the lumbosacral region have been suggested to function as a sense organ of equilibrium which is involved in the control of walking. Morphological, electrophysiological, behavioral and embryological evidence for such a function is reviewed. Birds have two nearly independent kinds of locomotion and it is suggested that two different sense organs play an important role in their respective control: the vestibular organ during flight and the lumbosacral system during walking.

The erector spinae group forms the majority of the muscle mass of the back and it is the primary extensor of the vertebral column. It controls flexion, lateral flexion, and rotation of the vertebral column, and maintains the lumbar curve. The erector spinae comprises the iliocostalis (laterally placed) group, the longissimus (intermediately placed) group, and the spinalis (medially placed) group.. The iliocostalis group includes the iliocostalis cervicis, associated with the cervical region; the iliocostalis thoracis, associated with the thoracic region; and the iliocostalis lumborum, associated with the lumbar region. The three muscles of the longissimus group are the longissimus capitis, associated with the head region; the longissimus cervicis, associated with the cervical region; and the longissimus thoracis, associated with the thoracic region. The third group, the spinalis group, comprises the spinalis capitis (head region), the spinalis cervicis (cervical region), and the spinalis ...
If oral analgesics are ineffective in sciatica pain relief, the attending physician uses Novocain injections. Injections are made as close to the sciatic nerve as possible by the method of lumbar puncture, which allows to locally influence the inflamed nerve and achieve a rapid and strong analgesic effect.. One of the most advanced and modern methods of sciatica treatment is cryotherapy. The method consists in alternating exposure to heat and cold on the lumbosacral region. In outpatient practice, cryo chambers are used. However, patient can independently achieve sciatica pain relief at home.. For cryotherapy at home, people use plastic bags with basic salt solution. After freezing in a refrigerating chamber, patient should apply the bag to his low back and perform light slow rotational movements. The skin turns pale at first, and then starts to redden quickly.. Exposure to cold should be discontinued with intense skin reddening. Patient must put aside the package with ice, warm the reddened ...

INTRODUCTION Paragangliomas arising in the spinal canal are rare and, when they occur, they are usually found in the extramedullary intradural compartment of the lumbosacral region. It is very rare to find them in the thoracic region, and so far only five cases have been described(1-3).. They were first described by Alfred Kohn, in 1903. These lesions are rare neuroendocrine neoplasms that originate in specialized cells derived from the neural crest. They are extremely vascularized and continuous with tissues that are integrated in autonomic functions. They are most frequent in the carotid or jugulotympanic region (around 80% to 90% of the cases), but they may develop at any site where chromaffin cells exist, from the base of the cranium to the bladder(1,2).. Pheochromocytomas are tumors that originate from chromaffin cells, which are specialized non-epithelial cells of the neural crest that are associated with sympathetic ganglia during fetal life. After birth, most of these cells degenerate ...
Answer: Myelomeningocele with ulceration. Myelomeningocele and other associated malformations of the spinal cord arise from restricted failure of posterior neuropore which usually closes with in 26 weeks of gestation. Myelomeningocele is characterized by herniation of the meninges and spinal cord at the site of the lesion. These lesions are most common in lumber or lumbosacral region. It is very difficult to diagnosis in intra uterine life. Ultrasonography may help but better diagnostic tool is MRI. Alpha feto protein {AFP} is increased in amniotic fluid as well as maternal serum because of leakage of AFP from meningomyelocele. {1} It is not pathognomic as there are other conditions where AFP is raised like esophageal and duodenal atresia, annular pancreas, omphalocele, congenital nephrosis, polycystic kidneys, renal agenesis or fetal demise. Maternal AFP is raised in multiple pregnancies. {2} The optimal time of determination of maternal serum AFP is 16-18 weeks of gestation and in amniotic ...

An L4-L5 laminectomy was performed in a 62-year-old man. Short-duration (,0.1 ms) mechanical force, manually assisted spinal manipulative thrusts (150 N) were delivered to the lumbosacral spine with an Activator II Adjusting Instrument. With the spine exposed, spinal manipulative thrusts were delivered internally to the L5 mammillary process, L5-S1 joint, and the sacral base with various force vectors. This protocol was repeated by contacting the skin overlying respective anatomic landmarks. Mixed-nerve root recordings were obtained from gas-sterilized platinum bipolar hooked electrodes attached to the S1 nerve root at the level of the dorsal root ganglion during the spinal manipulative thrusts and during a 30-second baseline period during which no spinal manipulative thrusts were applied. ...

The purpose of this preliminary study was to assess lumbar multifidus, erector spinae, and quadratus lum-borum muscle activity during lifts as measured by changes in transverse relaxation time (T2) from magnetic resonance imaging (MRI). Thirteen healthy adults performed dynamic squat, stoop, and asymmetric stoop lifts at a standard load, with each lift followed by MRI. Increase in T2 for the multifidus and erector spinae was greater for the stoop than squat. No difference in T2 increase was noted between the multifidus and erector spinae for the squat or stoop. Increase in T2 for the contralateral multifidus was less for the asymmetric stoop than stoop. Future research using MRI and other biomechanical techniques is needed to fully characterize lumbar muscle activity during lifts for various populations, settings, postures, and loads. ...
BACKGROUND: Electromyography (EMG) for suspected cervical or lumbosacral root compression is often negative, producing expense and physical discomfort that could have been avoided. To improve patient selection for testing, we sought to identify clinical features that would accurately predict presence of radiculopathy on EMG. METHODS: Adult patients consecutively evaluated for suspected cervical or lumbosacral root compression at an academic clinical neurophysiology laboratory were prospectively enrolled. Presence of clinical features suggesting root disease (neck or back pain, dermatomal pain or numbness, myotomal weakness, segmental reflex loss, and straight leg-raising) was recorded prior to testing. EMG examination to confirm root compression was conducted per standard protocols. Analysis was based on computation of sensitivity, specificity, predictive values, and accuracy. RESULTS: A total of 200 patients (55% male; mean age 46.4 years; 38% suspected of cervical and 62% of lumbosacral disease) were

We have examined the organization of muscle afferent projections to motoneurons in the lumbosacral spinal cord of chick embryos between stage 37, when muscle afferents first reach the motor nucleus, and stage 44, which is just before hatching. Connectivity between afferents and motoneurons was assessed by stimulating individual muscle nerves and recording the resulting motoneuron synaptic potentials intracellularly or electrotonically from other muscle nerves. Most of the recordings were made in the presence of DL-2-amino-5- phosphonovaleric acid (APV), picrotoxin, and strychnine to block long- latency excitatory and inhibitory pathways. Activation of muscle afferents evoked slow, positive potentials in muscle nerves but not in cutaneous nerves. These potentials were abolished in 0 mM Ca2+, 2mM Mn2+ solutions, indicating that they were generated by the action of chemical synapses. The muscle nerve recordings revealed a wide-spread pattern of excitatory connections between afferents and ...

Id consider that the other spinal extensors and stabilizers hold a much smaller, but a necessary role in the back. When talking about erector spinae or multifidus or the spinalis muscles, they are not as big as lats, dont have much coverage, or they dont have much leverage so to speak. They are mainly connected directly on the spine with lots of insertions, and provide more of an isometric and stabilizing role in my mind. But, this is my opinion, and I should point out that none of these muscles should be counted out when talking about important muscles of the back ...
