Ambulatory Blood Pressure Monitoring (ABPM) is a completely painless advanced technique that more accurately measures the patient's blood pressure than traditional methods. Some patients experience temporary increased blood pressure levels when they visit the doctor. Ambulatory Blood Pressure Monitoring avoids this problem. The patient performs his or her routine activities over a 24-hour period. The National Institute for Health & Care Excellence (NICE) suggests that patients with blood pressure readings of 140/90 mm Hg or higher receive ambulatory blood pressure monitoring. The process can be used to confirm the patient's hypertension. For patients with poorly-controlled hypertension and/or suspected resistance to blood pressure management medications, ABPM is a diagnostic tool. Patients who develop pregnancy-related hypertension or those considered at high-risk for complications related to hypertension benefit from ABPM. Diabetic patients or individuals with certain cerebrovascular disease, ...
White coat hypertension, more commonly known as white coat syndrome, is a phenomenon in which patients exhibit a blood pressure level above the normal range, in a clinical setting, though they don't exhibit it in other settings. It is believed that the phenomenon is due to anxiety that those afflicted experience during a clinic visit. The patient's daytime ambulatory blood pressure is used as a reference as it takes into account ordinary levels of daily stress. Many problems have been incurred in the diagnosis and treatment of white coat hypertension. The term "masked hypertension" can be used to describe the contrasting phenomenon, where a patient's blood pressure is above the normal range during daily living, although it isn't above the normal range when the patient is in a clinic setting. In studies, white coat hypertension can be defined as the presence of a defined hypertensive average blood pressure in a clinic setting, although it isn't present when the patient is at home. Diagnosis is ...

Keywords: Caffeine; heart rate variability; healthy volunteers. Introduction. The caffeine is one of the most widely used pharmacologically active substances. It has many different physiological effects on the cardiovascular function. Also the cardiac autonomic control could be altered by caffeine consumption. However, findings regarding the effect of caffeine consumption on cardiac autonomic nervous system (ANS) activity are controversial. The aim of the present study was to assess the effects of caffeine on cardiac ANS in young healthy subjects. Methods and Materials. In sixteen healthy volunteers (7 women), age 24-35, the electrocardiogram (ECG) was recorded continuously at rest before as well as 30 min and 60 min after caffeine ingestion (200 mg of caffeine in 50 ml of water). Before each the beginning of ECG registration the arterial blood pressure was measured. As assessment of cardiac ANS activity the power spectral analysis (autoregressive spectra) and the Poincaré plot analysis of the ...
The combination of benazepril plus amlodipine was shown to be more effective than benazepril plus hydrochlorothiazide in reducing cardiovascular events in the Avoiding Cardiovascular Events through Combination Therapy in Patients Living with Systolic Hypertension (ACCOMPLISH) trial. There was a small difference in clinic systolic blood pressure between the treatment arms favoring benazepril plus amlodipine. Ambulatory blood pressure monitoring provides a more rigorous estimate of blood pressure effects. A subset of 573 subjects underwent ambulatory blood pressure monitoring during year 2. Readings were obtained every 20 minutes during a 24-hour period. Between-treatment differences (benazepril plus amlodipine versus benazepril plus hydrochlorothiazide) in mean values were analyzed using ANOVA. Treatment comparisons with respect to categorical variables were made using Pearson's χ2. At year 2, the treatment groups did not differ significantly in 24-hour mean daytime or nighttime blood pressures ...
TY - JOUR. T1 - Role of paraventricular nucleus (PVH) in baroreflex-mediated changes in lumbar sympathetic nerve activity and heart rate. AU - Patel, K. P.. AU - Schmid, P. G.. PY - 1988/4. Y1 - 1988/4. N2 - Electrophysiological and neuronanatomical studies indicate that reciprocal connections between the paraventricular nucleus (PVH) and medullary sites are involved in cardiovascular regulation. To determine whether the PVH is involved in the regulation of baroreflex responses, lumbar sympathetic nerve activity (LSNA) and heart rate (HR) changes were recorded in response to increases in arterial pressure (produced by bolus doses of phenylephrine i.v.) prior to, during, and 60 min following the injection of lidocaine (2% lidocaine, 200 nl) bilaterally in the PVH of chloralose-anesthetized rabbits. Baseline blood pressure, HR, and LSNA did not change in response to the administration of lidocaine in the PVH. The magnitude of 'baroreflex responses' in HR and LSNA were expressed as the ratios of ...
0031] Referring to FIG. 6, in virtue of the ball-specific recommended pressure dial 60, a user reading the reading pointed by the air-pressure indicator 40 can at the same time be informed of whether the air pressure inside the ball 90 is appropriate for the particular ball. If the measured air pressure inside the ball 90 is lower than the recommended range for that ball, inflation can be conducted again to raise the air pressure inside the ball 90. If the air pressure inside the ball 90 is greater than the recommended range, the pressure-resealing valve 30 can be operated to allow the air inside the ball 90 to communicate with the external, ambient atmosphere through the probe 20, thereby releasing some air to reduce the air pressure inside the ball 90. Then, when the air-pressure indicator 40 points the air pressure as one applicable to the ball, the pressure-resealing valve 30 can be closed, so the adjustment of air pressure inside the ball 90 is completed ...
Arterial baroreflex and cardiac autonomic control play important roles in hemodynamic instability after carotid artery stenting (CAS). Spontaneous baroreflex sensitivity (BRS), heart rate variability (HRV) and blood pressure variability (BPV) are established tools for the assessment of arterial baroreflex and cardiac autonomic activity. Aim of the study was to evaluate cardiac autonomic activity (by means of HRV, BPV and BRS) after CAS and to explore the impact of internal carotid artery stenosis on BRS changes after CAS. 37 patients (68±10.45 years) with internal carotid stenosis underwent CAS. HRV, BPV and BRS were measured in all subjects before and at 1 and 72h after CAS. ANOVA was performed to compare BRS, HRV and BPV parameters before and after CAS. Spearman analysis was performed to determine a possible correlation between carotid stenosis degree (or carotid plaque diameter) and BRS changes (ΔBRS). LF/HF (index of sympatho-vagal balance) decreased during postoperative period, in ...

An ambulatory blood pressure monitor is a small device that you wear throughout the day, usually for 24 or 48 hours. The device takes your blood pressure automatically while you do your normal daily activities.. The device periodically inflates and takes blood pressure measurements, which are recorded for later printout and analysis. The devices are usually loaned by a clinic, hospital, or pharmacy.. Your doctor might ask you to use one of these monitors after measuring your blood pressure in the doctor's office, to make sure that you actually have high blood pressure. This is because your blood pressure can change during the day. And sometimes blood pressure is higher only because you are seeing a doctor. This is called white-coat hypertension. To diagnose high blood pressure, your doctor needs to know if your blood pressure stays high throughout the day. If you are required to use an ambulatory blood pressure monitor, keep in mind that it is important for a health professional to properly size ...
Cricoid pressure is considered to be the gold standard means of preventing aspiration of gastric content during Rapid Sequence Intubation (RSI). Its effectiveness has only been demonstrated in cadaveric studies and case reports. No randomised controlled trials comparing the incidence of gastric aspiration following emergent RSI, with or without cricoid pressure, have been performed. If improperly applied, cricoid pressure increases risk to the patient. The clinical significance of aspiration in the emergency department is unknown. This randomised controlled trial aims to; 1. Compare the application of the 'ideal' amount of force (30 - 40 newtons) to standard, unmeasured cricoid pressure and 2. Determine the incidence of clinically defined aspiration syndromes following RSI using a fibrinogen degradation assay previously described. 212 patients requiring emergency intubation will be randomly allocated to either control (unmeasured cricoid pressure) or intervention groups (30 - 40 newtons cricoid pressure

TY - JOUR. T1 - The impact of continuous positive airway pressure on the lower esophageal sphincter. AU - Shepherd, K.. AU - Holloway, R.. AU - Hillman, D.R.. AU - Eastwood, Peter. PY - 2007. Y1 - 2007. N2 - The lower esophageal sphincter ( LES) is the primary barrier to gastroesophageal reflux. Reflux is associated with periods of LES relaxation, as occurs during swallowing. Continuous positive airway pressure ( CPAP) has been shown to reduce reflux in individuals with and without sleep apnea, by an unknown mechanism. The aim of this study was to determine the effect of CPAP on swallow- induced LES relaxation. Measurements were made in 10 healthy, awake, supine individuals. Esophageal ( Pes), LES ( Ples), gastric ( Pg), and barrier pressure to reflux ( Pb = Ples - Pg) were recorded using a sleeve catheter during five swallows of 5 ml of water. This was repeated at four levels of CPAP ( 0, 5, 10, and 15 cmH(2)O). Pressures were measured during quiet breathing and during the LES relaxation ...
We do not encounter abdominal compartment syndrome often in the emergency department. With that said, this diagnosis enters our differential from time to time. Consider the patient with an active arterial bleed from a liver mass with large hemoperitoneum who is transferred from an outside institution for IR embolization. His abdomen is rock solid, and his lactate is 10. While you address his hemorrhagic shock, you have to at least consider the possibility of abdominal compartment syndrome. This is a case I was involved in several months ago; IR emergently drained his hemoperitoneum given the concern for abdominal compartment syndrome.. Abdominal compartment syndrome results from a sustained increase in intraabdominal pressure. The resulting decreased perfusion pressure to the viscera leads to ischemia and organ failure. While more common in post surgical patients in the ICU, this is a diagnosis the emergency physician should be familiar with.. Risk factors in ED population (this is far from a ...

Premature ventricular contractions is one of the most common symptomatic arrhythmia. Antiarrhythmia drugs for premature ventricular contractions, such as beta-blockers and sodium channel blockers, can cause bradycardia. For sinus bradycardia patients complicated with premature ventricular contractions, it's hard for doctors to make decision. The purpose of the study is to assess the effects of Chinese medicine Shansong Yangxin capsule for sinus bradycardia complicated with ventricular premature beats, which based on the numbers of premature ventricular contractions and average hear rate in 24-hour ambulatory electrocardiogram (ECG) after 8 weeks treatment as the primary endpoint of the study. Secondary endpoints are evaluation of the Shensong Yangxin capsule on quality of life. This study is a randomized, double-blind, placebo controlled, multi-center trial. Sinus bradycardia patients(average heart rate 45-59 beat per minute) associated with premature ventricular contractions (PVC number ,10000 ...

The double peak profile, or unreduced hernia, was present for 626 minutes. In all patients, both pressure profiles were observed. The transition rate between the 2 profiles was 8 per hour. The team noted that more reflux occurred when the lower esophageal sphincter and diaphragm were separated versus the reduced hernia state. The researchers found that the proportions of acidic reflux episodes during the single and double pressure peaks were similar. In the 2-pressure-zone state, there was an increase in all reflux mechanisms except transient lower esophageal sphincter relaxation. Dr Bredenoord's team concluded, In patients with a small hiatal hernia, intermittent reduction of the hernia occurs frequently. Spatial separation of the diaphragm and lower esophageal sphincter in the nonreduced state results in a 2-fold increase in acidic and weakly acidic reflux due to mechanisms other than transient lower esophageal sphincter relaxation. ...

The importance of counterregulatory mechanisms triggered by arterial vasodilation for the antihypertensive response to the calcium entry blocking agent nifedipine was investigated in 13 men with mild to moderate essential hypertension. Blood pressure and systemic vascular resistance were significantly reduced 30 minutes after sublingual administration of 10 mg of nifedipine while heart rate, cardiac index and plasma norepinephrine concentrations increased (all p , 0.01). Also, changes in mean blood pressure correlated inversely with arterial baroreflex sensitivity (r = -0.74, p , 0.01), suggesting that arterial baroreflex mechanisms by means of sympathetic activation tend to limit the acute antihypertensive response. Blood pressure, but not systemic vascular resistance, decreased further (p , 0.01) after 6 weeks of therapy with nifedipine 20 mg three times daily, while average heart rate, cardiac index and plasma norepinephrine concentrations had returned toward pretreatment values.. Thus, a ...
TY - JOUR. T1 - Both central command and exercise pressor reflex activate cardiac sympathetic nerve activity in decerebrate cats. AU - Tsuchimochi, Hirotsugu. AU - Hayes, Shawn G.. AU - McCord, Jennifer L.. AU - Kaufman, Marc P. PY - 2009/4. Y1 - 2009/4. N2 - Both static and dynamic exercise are known to increase cardiac pump function as well as arterial blood pressure. Feedforward control by central command and feedback control by the exercise pressor reflex are thought to be the neural mechanisms causing these effects during exercise. It remains unknown as to how each mechanism activates cardiac sympathetic nerve activity (CSNA) during exercise, especially at its onset. Thus we examined the response of CSNA to stimulation of the mesencephalic locomotor region (MLR, i.e., central command) and to static muscle contraction of the triceps surae muscles or stretch of the calcaneal tendon in decerebrate cats. We found that MLR stimulation immediately increased CSNA, which was followed by a gradual ...

Background: Left ventricular end diastolic pressure could be estimated collectively using various measures of mitral valve and pulmonary venous flow velocities. In patients with aortic regurgitation, the AR velocity reflects the diastolic pressure difference between the aorta and the left ventricle. We sought to predict the left ventricular end diastolic pressure by a new Doppler index as aortic regurgitation peak early to late diastolic pressure gradient ratio.Patients and Methods: Fifty three patients with at least moderate aortic regurgitation were enrolled in this study. Physical examination, electrocardiography and echocardiography were performed one day before cardiac catheterization. The severity of AR was graded according to the recommendations of American society for echocardiography. The pressure half time, aortic regurgitation early diastolic velocity , aortic regurgitation early diastolic pressure gradient , aortic regurgitation end diastolic velocity, aortic regurgitation end diastolic
Monitoring your blood pressure at home used to be a rather tricky prospect for many people. Those who had arthritis couldn't squeeze the bulb used to pump up the pressure cuff, and those with hearing loss couldn't hear the heart sounds through the stethoscope. Even those with no physical problems had to learn the proper rate of cuff deflation and how to distinguish subtle heart sounds. Today, however, all of those problems have been solved by the development of digital monitors.. If you're using a fully automatic digital blood pressure monitor, all you have to do is position it on your arm and push a button. The cuff inflates and deflates automatically at the proper rate. Since the blood pressure is measured by an oscilloscope, there's no need to listen for heart sounds. The systolic and diastolic numbers are displayed on a small screen. Pulse rate is usually displayed as well. A semiautomatic digital monitor does everything except inflate the pressure cuff. Digital blood pressure monitors are ...
The aim of present study was to evaluate relationships between degree of portal hypertension, severity of the disease, and bleeding status in patients with liver cirrhosis. Patients and methods. All study patients with liver cirrhosis underwent hepatic venous pressure gradient measurements, endoscopy, clinical and biochemical evaluation. Liver function was evaluated according to Child-Turcotte-Pugh (Child's) scoring system. Patients with decompensated cirrhosis (presence of severe ascites, acute variceal bleeding occurring within 14 days, hepatorenal syndrome, cardiopulmonary disorders, transaminase levels >10 times higher the upper normal limit), active alcohol intake, use of antiviral therapy and/or beta-blockers were excluded from the study. Results. One hundred twenty-eight patients with liver cirrhosis (male/female, 67/61; mean age, 53.8±12.7 years) were included into the study. Etiology of cirrhosis was viral hepatitis, alcoholic liver disease, cryptogenic and miscellaneous reasons in 57, 49,
It is often used in high blood pressure. Losartan 50 mg once daily for four weeks, followed by losartan … Summary: We compare the side effects and drug effectiveness of Olmesartan medoxomil and Losartan potassium. Systolic and diastolic blood pressure readings taken in the final 2, 4, and 6 hours of the 24-hour ABPM cycle are summarized. Olmesartan medoxomil; hydrochlorothiazide has active ingredients of olmesartan … In week 4, participants arrived at the site in the morning without having taken that day's dose of medication. Change From Baseline in Mean Daytime (8am to 4pm) and Mean Nighttime (10pm to 6am) Ambulatory Blood Pressure at Week 8. Cozaar (losartan) is available in the following dosages: 25 mg; 50 mg; 100 mg; As you can see, Benicar (olmesartan) is a more potent drug, on a milligram basis, than Cozaar (losartan). Olmesartan is better than losartan at lowering blood pressure and generally provides good 24-hour coverage. Treatment as a fixed effect and the seated cuff diastolic blood
