Firstly, the lamina was removed en-bloc from C3-C5 with the aid of a high speed burr. The spinal cord was then allowed to adapt to its new position for 10 minutes before performing similar en-bloc laminectomy for C6-C7. After waiting for another 10 minutes, the final laminectomy for T1-T3 was performed in a piece meal fashion. Postero-lateral fusion was done using local bone chips and bone substitutes. Ankara Atatürk Research and Education Hospital, Department of Neurosurgery, Ankara, Turkey Diffuse idiopathic skeletal hyperostosis (DISH) or Forestiers disease is a common disorder of unknown etiology that is characterized by ossification of the anterior longitudinal ligament of the spine and various extra-spinal ligaments. We present the case of a 54-year-old woman with progressive dysphagia due to DISH of the cervical spine, which is a relatively rare pathology in neurosurgical practice. The cervical osteophytes extending from C2 to C4 and external compression of the pharyngoesophageal segment by the large osteophytes were demonstrated by X-ray, magnetic resonance imaging, and computed tomography. Surgical removal of the large osteophytes and a shortterm nonsteroidal anti-inflammatory drug regimen led to the resolution of dysphagia. The semi-constrained screw and cervical plate assembly includes a cervical plate and one or more semi-constrained bone screws. Each semi-constrained bone screw includes an elongated shank, which is mechanically and releasably coupled to a head portion. The head portion includes one or more threaded segments which define one or more slits therebetween. As the semi-constrained bone screw is driven through the screw opening in the cervical plate and into the bone, the head portion allows poly-axial movement of the shank relative to the head portion.

The endoprosthesis has a resilient body formed of one or more materials which may vary in stiffness from a relatively stiff exterior annular gasket portion to a relatively supple central nucleus portion. Concaval-convex elements at least partly surround that nucleus portion so as to retain the nucleus portion and gasket between adjacent vertebral bodies in a patients spine. Assemblies of endoprosthetic discs, endoprosthetic vertebral bodies, and endoprosthetic longitudinal ligaments may be constructed. To implant this endoprosthesis assembly, information is obtained regarding the size, shape, and nature of a patients damaged spine. Thereafter, one or more prosthetic vertebral bodies and disc units are constructed in conformity with that information.
