OBJECTIVE To determine whether adult cochlear implant (CI) users with superior canal dehiscence syndrome (SCDS) or asymptomatic superior semicircular canal dehiscence (SCD) have different surgical, vestibular, and audiologic outcomes when compared to CI users with normal temporal bone anatomy. METHODS A retrospective single institution review of CI users with either superior semicircular canal dehiscence syndrome or asymptomatic superior semicircular canal dehiscence identified eight post-lingually deafened adults with unilateral or bilateral cochlear implantation between 2006 and 2010. Preoperative and postoperative speech perception scores as well as medical and epidemiological data were recorded and analyzed. RESULTS One patient with superior canal dehiscence syndrome and seven patients with asymptomatic superior semicircular canal dehiscence were identified, representing 7% or 8/113 of CI patients that fulfilled selection criteria. Average dehiscence length was 3.3 mm ± 0.79 SEM. Three patients

N2 - Anal squamous cell carcinoma (ASCC) is a rare, potentially fatal malignancy primarily caused by high-risk human papillomaviruses (HPV). The prognostic implication of programmed death-ligand 1 (PD-L1) expression remains controversial, and glucose transporter 1 (GLUT1) expression has never been examined in ASCC. Covalently closed circular RNAs have recently been shown to be widespread in cancers and are proposed to be biomarkers. We discovered HPV16 expresses a circular E7 RNA (circE7) which has not been assessed as a potential biomarker. A retrospective, translational case series at UT Southwestern was conducted to analyze PD-L1, GLUT1, ...

Rectal tightness and muscle spasm. What is the treatment for Levator Ani Syndrome?. The common types of treatment for levator ani syndrome are usually muscle relaxants, hot baths, and proper diet and exercise. In most cases, there is an underlying element of chronic stress and anxiety which causes these muscles to tighten up and spasm. This pelvic floor muscle tension causes pain and burning. In order for most cases of levator ani syndrome to be resolved, trigger point therapy or internal massage on the levator ani/rectal muscles can bring great relief. The treatment itself can be very painful, but the overall relief from treatment can be felt almost immediately. ...

OBJECTIVE To evaluate the impact of transanal endoscopic microsurgery (TEM) on postoperative anal function and quality of life in patients with benign rectal tumor and early rectal cancer. METHODS Clinical data of 50 patients with rectal adenoma and early rectal cancer undergoing transanal endoscopic microsurgery in our hospital from October 2008 to June 2013 were retrospectively analyzed. Anorectal manometry, endorectal ultrasonography (ERUS), the fecal incontinence severity index (FISI), and the physical and mental health status scores (SF-36) were used to evaluate preoperative and postoperative anorectal function and quality of life. RESULTS Anorectal manometry indicated anal resting pressure (ARP), maximum squeeze pressure (MSP), rectal volume at sensory threshold(RVST), maximum tolerable volume(MTV) decreased significantly at the first month after surgery (P|0.05). MSP returned to preoperative level at the 3rd month (P|0.05). ARP and MTV returned to normal values at the 6th month (P|0.05). RVST
PURPOSE: Disconnection of an ileal pouch-anal anastomosis with repeat ileal pouch-anal anastomosis has been proposed for treatment of ileal pouch-anal anastomosis failure caused by septic or functional complications. We report our experience with repeat ileal pouch-anal anastomosis, and document functional outcome and quality of life.. METHODS: Of 101 patients undergoing laparotomy, ileoanal disconnection, and repeat ileal pouch-anal anastomosis, 80 were referred from other institutions. Indications included: chronic anastomotic leak (n=27), perineal or pouch-vaginal fistula (n=47), anastomotic stricture (n=22), dysfunction/long efferent limb of S-pouch (n=36), and previous ileal pouch-anal anastomosis excision or exclusion (n=6). In 64 cases a "septic" indication was observed. Pathologic features of Crohn's disease were present in 4 patients preoperatively and 15 more after repeat ileal pouch-anal anastomosis. Four patients had clinical features of Crohn's disease.. RESULTS: Three patients had ...
In previous studies, the frequency of PSN in LC-BPPV has been reported to vary from 40.1% to 76%, and to increase to 56.2-96% after mild head shaking [3,4,10]. In the present study, this ratio for patients showing PSN was only 16.8%, which is considerably lower than in previous reports. Two situations at the outpatient clinic can account for this difference. First in the present study, the time devoted at the clinic to examining spontaneous nystagmus was relatively short, although it was sufficient to detect apparent nystagmus (minimum 10 seconds). Second, and more important, the head position of the patients was not precisely controlled. The existence and direction of PSN were observed immediately after a patient sat on a chair with his/her head roughly earth vertical. Patients with LC-BPPV naturally prefer to bend their head slightly forward, since this posture minimizes movement of otolith particles in the lateral semicircular canal. Otherwise, patients favor at least having the head fixed in ...

Anaplastic thyroid carcinoma, which is one of the most aggressive, malignant tumors in humans, results in an extremely poor prognosis despite chemotherapy and radiotherapy. The present study was designed to evaluate therapeutic effects of radiation by glycerol on p53-mutant anaplastic thyroid carcinoma cells (8305c cells). To examine the effectiveness of glycerol in radiation induced lethality for anaplastic thyroid carcinoma 8305c cells, we performed colony formation assay and apoptosis analysis. Apoptosis was analyzed with Hoechst 33342 staining and DNA ladder formation assay. 8305c cells became radiosensitive when glycerol was added to culture medium before X-ray irradiation. Apoptosis was induced by X-rays in the presence of glycerol. However, there was little apoptosis induced by X-ray irradiation or glycerol alone. The binding activity of whole cell extracts to bax promoter region was induced by X-rays in the presence of glycerol but not by X-rays alone. These findings suggest that glycerol is

To compare volumetric-modulated arc therapy (RapidArc) plans with conventional intensity-modulated radiation therapy (IMRT) plans in anal canal cancers. Ten patients with anal canal carcinoma previously treated with IMRT in our institution were selected for this study. For each patient, three plans were generated with the planning CT scan: one using a fixed beam IMRT, and two plans using the RapidArc technique: a single (RA1) and a double (RA2) modulated arc therapy. The treatment plan was designed to deliver in one process with simultaneous integrated boost (SIB) a dose of 59.4 Gy to the planning target volume (PTV2) based on the gross disease in a 1.8 Gy-daily fraction, 5 days a week. At the same time, the subclinical disease (PTV1) was planned to receive 49.5 Gy in a 1.5 Gy-daily fraction. Plans were normalized to 99% of the PTV2 that received 95% of the prescribed dose. Planning objectives were 95% of the PTV1 will receive 95% of the prescribed dose and no more than 2% of the PTV will receive more
Traditional Chinese medicine believes that hemorrhoids are mostly caused by rheumatism, dryness, heat, and pulsation of evil wounds, so that qi and blood are stagnant and congregate in the anus; anal fissures are mostly caused by dry fire, damp heat accumulating in the anus, or blood heat and intestinal dryness; anal fistula It is formed by producing damp heat and fire poison in the body and betting on the anus. It can be seen from the above that most of anorectal diseases are related to damp heat, and damp heat mainly comes from diet, so Chinese medicine emphasizes that diet should be controlled.. There are two reasons for anorectal disease:. 1. Disorders of life patterns. 1. Excessive play: Most people think that they are busy with work and do not have time to entertain with family and friends. They should gather together during holidays and have fun. As a result, playing all night long, sleeping during the day, the yin and yang are reversed, disrupting the original rhythm of life, leading to ...
The standard treatment of rectal adenocarcinoma is total mesorectal excision (TME). The technique involves a low anterior rectal or colo-anal resection, very often associated with a protective stoma or abdominal-perineal resection with permanent colostomy. Transanal endoscopic microsurgery (TEM) allows access to tumors up to 20 cm from the anal margin, with minimal postoperative morbidity and mortality. Recent studies of T1 rectal adenocarcinomas consider TEM to be the technique of choice. However the treatment of T2 rectal cancers remains controversial. Chemotherapy and radiotherapy (CT/RT) has achieved a concomitant reduction in local recurrence and an increase in survival.. Hypothesis: Patients with rectal adenocarcinoma less than 10 cm from the anal margin and up to 4 cm in size, staged after endorectal ultrasound and MRI as T2 or superficial T3 N0-M0-N0-M0, who underwent surgery after preoperative local chemoradiotherapy (TEM), achieve effective results in terms of local recurrence similar ...

Mutation of the BRAF gene is common in thyroid cancer. Follicular variant of papillary thyroid carcinoma is a variant of papillary thyroid carcinoma that has created continuous diagnostic controversies among pathologists. The aims of this study are to (1) investigate whether follicular variant of papillary thyroid carcinoma has a different pattern of BRAF mutation than conventional papillary thyroid carcinoma in a large cohort of patients with typical features of follicular variant of papillary thyroid carcinoma and (2) to study the relationship of clinicopathological features of papillary thyroid carcinomas with BRAF mutation. Tissue blocks from 76 patients with diagnostic features of papillary thyroid carcinomas (40 with conventional type and 36 with follicular variant) were included in the study. From these, DNA was extracted and BRAF V600E mutations were detected by polymerase chain reaction followed by restriction enzyme digestion and sequencing of exon 15. Analysis of the data indicated ...
Anal HPV infection can be a risk factor for anal cancer, which is a common non-AIDS-defining cancer among HIV-infected MSM. Screening for anal cancer is not widely available and can be difficult to implement. People who receive the FDA-approved quadrivalent HPV vaccine, Gardasil, may have a reduced risk of developing anal HPV infection, which may in turn reduce the risk of developing anal cancer. The purpose of this study is to evaluate the effectiveness of the quadrivalent HPV vaccine, Gardasil, at reducing the incidence of anal HPV infections in HIV-infected MSM and HIV-infected women.. This study enrolled HIV-infected MSM and HIV-infected women. Participants were randomly assigned to receive the HPV vaccine or a placebo vaccine. At the screening study visit, participants underwent a physical examination, blood collection, anal swab procedure, oral examination, questionnaires, a high-resolution anoscopy (HRA), and if female, a pregnancy test, vaginal swab, and gynecologic exam. Lymphoepithelioma-like carcinoma (LELC) is a medical term referring to a histological variant of malignant tumor arising from the uncontrolled mitosis of transformed cells originating in epithelial tissue (or in cells that display epithelial characteristics) that bear microscopic resemblance to lymphoepithelioma (nasopharyngeal carcinoma). There is considerable variation in the classification of LELC-while it is perhaps most commonly considered a subtype of squamous cell carcinoma, it can also be classified as a form of large cell carcinoma (i.e. when occurring in the lung), and can be considered as a separate, unique entity. In most anatomical sites, many cases are associated with the Epstein-Barr virus. In the breast, the macroscopic, microscopic, epidemiologic, and prognostic features of LELC are very similar to medullary carcinoma; EBV status is one differentiator. Merkel cell carcinoma is a rare and aggressive skin malignancy that arises from primary neural cells and has a tendency for local recurrence and regional lymph node metastases. There are only a few cases in the literature reporting metastases of Merkel cell carcinoma to the gastrointestinal tract. We present a 70 year old Caucasian female with distant history of Merkel cell carcinoma who presented with iron-deficiency anemia. Colonoscopy performed later for the evaluation of anemia revealed 1 cm polyp in ascending colon which turned out to be the recurrence of Merkel cell carcinoma. Metastatic Merkel cell carcinoma to the gastrointestinal tract or any other organ should be considered in patients with a history of Merkel cell carcinoma.
TY - JOUR. T1 - Involvement of cysteine-rich protein 61 in the epidermal growth factor-induced migration of human anaplastic thyroid cancer cells. AU - Chin, Li Han. AU - Hsu, Sung Po. AU - Zhong, Wen-Bin. AU - Liang, Yu Chih. PY - 2016. Y1 - 2016. N2 - Anaplastic thyroid cancer (ATC) is among the most aggressive types of malignant cancer. Epidermal growth factor (EGF) plays a crucial role in the pathogenesis of ATC, and patients with thyroid carcinoma typically exhibit increased cysteine-rich protein 61 (Cyr61). In this study, we found that EGF treatment induced cell migration, stress fiber formation, Cyr61 mRNA and protein expressions, and Cyr61 protein secretion in ATC cells. The recombinant Cyr61 protein significantly induced cell migration; however, inhibition of Cyr61 activity by a Cyr61-specific antibody abrogated EGF-induced cell migration. EGF treatment also affected epithelial-to-mesenchymal transition (EMT)-related marker protein expression, as evidenced by an increase in vimentin and ...
Sacral nerve stimulation using a tined lead as an extended testing phase to evaluate the predictive value of this form of testing by comparing the long-term (6 months) response to permanent sacral nerve stimulation in the groups classified by the test.. The tined lead test stimulation (TiLTS) is of 6 weeks duration and involves an 'active' period of 2 weeks of active subsensory sacral nerve stimulation, and a placebo or 'sham' period of 2 weeks of pretend subsensory sacral nerve stimulation. HIV infected men having sex with men (MSM) are at increased risk of developing anal cancer compared to the general population and the incidence continues to increase despite better control of HIV infection with HAART (Highly Active Anti-Retroviral Therapy). The causative agent is known to be Human Papilloma Virus infection which can lead to dysplastic changes in the anus, detectable by High Resolution Anoscopy with biopsies. The analysis of the abnormal tissue can then be graded as Anal Intraepithelial Neoplasia 1 to 3, with AIN 2 or 3 considered as high grade dysplasia. These lesions are cancer precursors, but the proportion of lesions progressing to invasive anal cancer and the time to event are unknown. There is currently no recognized treatment to offer as standard of care although it is of general belief that treating these lesions, as it is done for women with CIN 2 and 3 (Cervical Intraepithelial Neoplasia) could help decrease the number of progressions to invasive anal cancer in MSM ...
to the editor: Sebaceous cell carcinoma of the eyelid is the fourth most common malignancy in the periocular region in the United States (basal cell carcinoma, squamous cell carcinoma, and melanoma are the leading three causes) and the second most common malignancy in China (basal cell carcinoma is the leading cause).1,2 Diagnosis and therapy tend to be delayed because sebaceous carcinoma is often mistaken for benign entities such as chalazion, conjunctivitis, or blepharitis.3. A 74-year-old woman presented to her ophthalmologist with conjunctival erythema and slight tenderness of her upper eyelid. She was diagnosed with conjunctivitis and prescribed erythromycin 0.5% ophthalmic ointment. At her one-month follow-up visit, she continued to have erythema and had also developed a small tender papule. A diagnosis of chalazion was made, and the patient was instructed to continue her antibiotic ointment and to apply warm compresses four times daily to the eye. OUTLINE: This is a multicenter study.. Patients undergo anal swab collection at baseline to obtain samples for anal cytology, anal human papillomavirus (HPV) typing, and other HPV-related testing (e.g., HPV viral load). Digital rectal examinations (DRE) are also performed as part of the baseline physical examination. Female patients also undergo cervical swab collection for cervical HPV testing and cytology, as well as colposcopy (if available) of the cervix and vulvovaginal region to completely assess lower genital tract HPV-related lesions. At sites where high-resolution anoscopy (HRA) is available, patients are encouraged, but not required, to have an HRA with biopsy of any visualized lesions within 30 days of collection of the swabs.. After baseline assessments, patients undergo treatment with the investigative agent according to the study protocol requirements. If study treatment continues beyond 6 months, additional anal and cervical swabs are obtained for anal and cervical HPV and ...

Since inflammatory lymph nodes sometimes manifest false positive FDG findings, we should therefore carefully examine patients with cervical lymph node swelling and consider the anatomic location of both the primary lesion and cervical lymph nodes. Other morphological imaging modalities (CECT, MRI) may therefore also be necessary to obtain more precise anatomic information.. Pudendal nerve entrapment (PNE), also known as Alcock canal syndrome, is an uncommon source of chronic pain, in which the pudendal nerve (located in the pelvis) is entrapped or compressed. Pain is positional and is worsened by sitting. Other symptoms include genital numbness, fecal incontinence and urinary incontinence. The term pudendal neuralgia (PN) is used interchangeably with "pudendal nerve entrapment", but a 2009 review study found both that "prevalence of PN is unknown and it seems to be a rare event" and that "there is no evidence to support equating the presence of this syndrome with a diagnosis of pudendal nerve entrapment," meaning that it is possible to have all the symptoms of pudendal nerve entrapment (otherwise known as pudendal neuralgia) based on the criteria specified at Nantes in 2006, without having an entrapped pudendal nerve. A 2015 study of 13 normal female cadavers found that the pudendal nerve was attached or fixed to the sacrospinous ligament in all cadavers studied, ...
The scariest rectal prolapse symptom is when the rectum, part of the lower intestine, falls out of the anal opening.. With rectal prolapse, the rectum can fail to come out at all, only appear with bowel movements or sneezing, or chronically protrude from the body at all times.. The symptoms of internal intussception rectal prolapse are a feeling of fullness, bloating, or internal discomfort, as the rectum slides out of its appointed position and falls against other internal organs.. With rectal prolapse, extreme pain and difficulties with bowel movements may also be noticed, especially if the rectum undergoes any torsion (twisting) during its collapse in any form of the rectal prolapse.. Other forms of rectal prolapse cause the protrusion of some or all of the rectum from the anal sphincter.. Other symptoms of rectal prolapse include bleeding or mucus discharge from the damaged tissue, loss of urge to defecate, and fecal incontinence.. Rectal prolapse that is protruding, will most likely look ...

The external anal sphincter (or sphincter ani externus ) is a flat plane of muscular fibers, elliptical in shape and intimately adherent to the skin surrounding the margin of the anus. The external anal sphincter measures about 8 to 10 cm in length, from its anterior to its posterior extremity, and is about 2.5 cm opposite the anus, when defecation occurs the sphincter muscle retracts. It consists of two strata, superficial and deep. The superficial, constituting the main portion of the muscle, arises from a narrow tendinous band, the anococcygeal raphe, which stretches from the tip of the coccyx to the posterior margin of the anus; it forms two flattened planes of muscular tissue, which encircle the anus and meet in front to be inserted into the central tendinous point of the perineum, joining with the superficial transverse perineal muscle, the levator ani, and the bulbospongiosus muscle also known as the bulbocavernosus. The deeper portion forms a complete sphincter to the anal canal. Its ...
Detection of high-grade squamous intraepithelial lesions (HSILs) on an anal Pap smear had a sensitivity of 47% (95% CI, 35%59%) and specificity of 90% (95% CI, 81%96%) for detection of a high-grade histological finding (AIN level 2, AIN level 3, or SCC) in the paired specimen. Likewise, detection of low-grade squamous intraepithelial lesions (LSILs) on an anal Pap smear had a sensitivity of 68% (95% CI, 56%78%) and a specificity of 48% (95% CI, 36%59%) for detection of AIN level 1 by the paired examination. A Pap smear finding of atypical squamous cells of uncertain significance showed essentially an equal distribution of pathological grades on paired histological examinations. The positive and negative predictive values were 81% (95% CI, 66%92%) and 65% (95% CI, 55%74%), respectively, for HSIL on Pap smears predicting high-grade histological findings (P < .01). The positive and negative predictive values for LSIL were 59% (95% CI, 48%69%) and 57% (95% CI, 44%70%), respectively, for prediction ...
Intractable ulcerating enterocolitis is an uncommon inflammatory bowel disease syndrome of neonatal onset first described in 1991. Intractable ulcerating enterocolitis usually presents in the neonate with a mouth ulceration and the subsequent development of perianal disease and colitis.. In this case report, an infant, 18 days from birth, with ulcerative lesion on hard palate for systemic differential diagnosis about oral lesion is referred from the department depiatrics. At that time, there is no abnormality, except oral lesion-like Aphthous ulcer. The patient was discharged from pediatrics, but returned to the hospital 3weeks later with blood diarrhea. As a result of endoscopy, there were large ulcerating lesions and the patient was diagnosed intractable ulcerating enterocolitis.. Early recognition of Intractable ulcerating enterocolitis appears to be beneficial because colectomy, as opposed to immunosuppression, appears to be effective in controlling disease symptoms and progression. Most of ...
Results Sixty-one papers (35 costs; 24 utilities; 2 costs and utilities) were selected from 10 742 initial records. Cost per case ranges were US$124-US$883 (anogenital warts), US$6912-US$52 579 (head and neck cancers), US$12 936-US$51 571 (anal cancer), US$17 524-34 258 (vaginal cancer), US$14 686-US$28 502 (vulvar cancer) and US$9975-US$27 629 (penile cancer). The total cost for 14 adult patients with recurrent respiratory papillomatosis was US$137 601 (one paper).. Utility per warts episode ranged from 0.651 to 1 (12 papers, various utility elicitation methods), with pooled mean EQ-5D and EQ-VAS of 0.86 (95% CI 0.85 to 0.87) and 0.74 (95% CI 0.74 to 0.75), respectively. Fifteen papers reported utilities in head and neck cancers with range 0.29 (95% CI 0.0 to 0.76) to 0.94 (95% CI 0.3 to 1.0). Mean utility reported ranged from 0.5 (95% CI 0.4 to 0.61) to 0.65 (95% CI 0.45 to 0.75) (anal cancer), 0.59 (95% CI 0.54 to 0.64) (vaginal cancer), 0.65 (95% CI 0.60 to 0.70) (vulvar cancer) and 0.79 ...

The abdominoperineal resection of the rectum is a classical operation performed in case of patients diagnosed with rectal cancer. The development of laparoscopic techniques in recent years, introduced yet another method of treatment, considering patients with rectal cancer- laparoscopic abdominoperineal resection of the rectum. The aim of the study was to present initial treatment results considering the above-mentioned patients. Material and methods. The study group comprised 25 patients (16 male and 9 female) diagnosed with low-rectal cancer, subjected to surgery by means of the above-mentioned method. Mean patient age amounted to 66 years. Three (12%) patients required conversion to classical surgery (laparotomy), while one patient required reoperation, due to presacral vascular bleeding. Complications were observed in 10 (40%) patients. Average hospitalization was 7 days. In case of all patients the radial margin was negative, and mean number of removed lymph nodes amounted to 9.6. Mortality ...
