Objective. The aim of this study was to investigate the relationship between stressful life events (LESs) and its effect on the initiation of Graves disease (GD) and toxic nodular goitre (TNG). Patients and method. Forty-five patients with GD, 24 patients with TNG and 36 healthy control (CG) were included to the study. Graves and TNG patients had diagnosed within the last 12 months, with clinical and biochemical confirmation in Endocrinology Metabolism Outpatient Clinic of Cerrahpasa Medical School. The Holmes-Rahe Stress Scale and the Life Experience Survey (LES) was the psychological evaluation instrument used in this study. Results. Piecewise growth curve model (PGCM) is often used when the underlying growth process is not linear and is hypothesized to consist of phasic developments connected by turning points (or knots or change points). When fitting a PGCM, the conventional practice is to specify turning points a priori. However, the true turning points are often unknown and misspecifications of turning points may occur. The study examined the consequences of turning point misspecifications on growth parameter estimates and evaluated the performance of commonly used fit indices in detecting model misspecification due to mis-specified locations of turning points. In addition, this study introduced and evaluated a newly developed PGCM which allows unknown turning points to be freely estimated. This article systematically reviews the literature on the effects of adverse life events or trauma on people with intellectual disabilities. It is important to systematically examine empirical evidence of the effects of trauma in people with intellectual disabilities as to date the number of studies in this area is not substantial, and the effects of trauma seen in the predominately general population literature are not necessarily transferable to the intellectual disability population. Identification of the effects of trauma on people with intellectual disabilities facilitates case recognition and appropriate treatment. Objectives: Stressful life events (SLEs) have been associated with adjustment problems in ado- lescence (APA) in cross-sectional studies. Using a longitudinal cohort, we examined the influence of these events and predefined covariates on APA and compared internalizing and externalizing trajectories among children with many versus few SLEs. Methods: Data were obtained from the Study of Early Child Care and Youth Development. One thousand three hundred sixty-four children and their families were followed from childs birth until age 15 years. Adjustment problems at age 15 years were defined as high ( > 60 T-score) internalizing and/or externalizing problems on the Youth Self-Report and Child Behavior Checklist. Stressful life events were evaluated at 54 months, and third and fifth grade. Categories created by mixture model analyses for covariates were used in logistic regressions to predict adjustment problems. Stressful events have been investigated in various immune-mediated diseases but not in celiac disease. Our aim was to examine the relationship of stressful events assessed by the standardized interview of Paykel with the diagnosis of celiac disease in comparison to patients, with a diagnosis of gastroesophageal reflux disease used as the control group. Adults with celiac disease (n = 186) reported more frequent and more severe life events in the years prior to the diagnosis than control patients (n = 96) (67.2% vs. 37.5%, p < 0.001, mean Paykel score 11.5 vs. 13.4, p = 0.001, respectively). Findings were not significantly different between celiac disease and control patients for the time lapse between the event and the diagnosis (mean 5.5 vs. 5.7 months). Pregnancy was defined as a negative event by 20.3% of celiac women, but never by control women. Findings were confirmed when analyses were repeated in the subgroup of patients of both groups with diagnosis made within one year of onset of symptoms.

Recovery from serious psychiatric disabilities is a multidimensional, nonlinear process that consists, in part, of a transformation from illness-dominated identities to identities centered on empowerment and agency. This transformation may be punctuated by critical turning points. Analysis of these turning points may provide insight into the recovery process and how recovery can be initiated and advanced. In this qualitative study, in-depth personal interviews were conducted with 16 psychiatric survivors who were also providers of consumer-operated services. Participants were asked to discuss the critical turning points instrumental in their recovery processes. Analysis of these turning points using grounded theory revealed that participants played an active role with the help of supportive allies in initiating and sustaining their recoveries and that recovery consisted of a complex and ongoing struggle against multiple constraints to establish more positive identities.
