Twenty patients (16 from 2-16 years old, mean 10.4 years; 4 from 17-69, mean 47.5 years) with an encephalitis lethargica (EL) syndrome resembling that reported in the 1916-1927 epidemic, and presenting between 1999 and 2002, are reported from Great Ormond Street Hospital, London, and other tertiary neurology centers in the UK. The EL syndrome consisted of a sleep disorder (somnolence, sleep inversion, or insomnia), lethargy, parkinsonism, dyskinesia (dystonia, oculogyric crises, hemiballism, chorea or tics), and neuropsychiatric symptoms (mutism (10 patients), anxiety, depression (6), agitation, catatonia, obsessive-compulsive disorder, ophthalmoplegia or pupillary abnormalities (7), headache (6), and seizures (3). Encephalitis lethargica (EL) describes an encephalitis with psychiatric, sleep, and extrapyramidal movement disorders. Dyskinetic and parkinsonian forms have been described. EL shares clinical features with the anti-N-methyl-D-aspartate receptor (NMDAR-Ab) encephalitis. We studied 20 sera from pediatric patients with contemporary EL. Ten sera (from 2 males and 8 females, aged 1.3-13 years) and 6/6 cerebrospinal fluid samples were positive for NMDAR-Ab. NMDAR-Ab-positive patients had dyskinesias, agitation, seizures, and insomnia, whereas parkinsonism and somnolence dominated in the NMDAR-Ab-negative children. We were unable to identify any tumors. The dyskinetic form of EL is an NMDAR-Ab encephalitis and can affect very young children.

Although ferrets produced a neutralizing humoral response to challenge, we did not observe disease or viremia in any animal. Residents approached him with worries over the citys beaches and shellfish beds after the sewage treatment plant received a notice of violation from the DEEP for releasing 43 million gallons of partially treated water into Long Island Sound last October. Heavy rainfall increases the flow of water into sewer lines, and sometimes leads to bypasses as treatment plants become overwhelmed by the sharp spike in volume. Under the recently approved legislation, the DEEP must show where sewage spills are expected during heavy rain and how far resulting contamination will reach. Fishing dredges, which had been invented in Europe in the 14th century, were repurposed for oyster harvesting in Long Island Sound. Peter Decker, a Norwalk oysterman, in 1874 introduced the steam-powered dredger, which increased the rate of harvest from about 25 bushels per day by sail to 20 times that much. Not only did these iron mesh contraptions dramatically increase the number of harvested oysters, they also had environmental consequences for the sea floor because they disrupted the habitat for other organisms and stirred up sediment that could choke remaining oysters..
