The Krukenberg procedure, also known as the Krukenberg operation, is a surgical technique that converts a forearm stump into a pincer. It was first described in 1917 by the German army surgeon Hermann Krukenberg. It remains in use today for certain special cases but is considered controversial and some surgeons refuse to perform it. The procedure involves separating the ulna and radius for below-elbow amputations, and in cases of congenital absence of the hand, to provide a pincerlike grasp that is motored by the pronator teres muscle. The prerequisites for the operation are a stump over 10 cm long from the tip of the olecranon, no elbow contracture, and good psychological preparation and acceptance. This is a case report of a 33-year-old woman who presented 5 days post-partum with abdominal distension, abdominal discomfort, increased bladder pressure and fatigue. These symptoms prompted a pelvic ultrasound and CT scan of the chest, abdomen and pelvis which showed a large 20 cm left-sided adnexal mass. Following this imaging, blood tests revealed elevated tumour markers CA-125 and lactate dehydrogenase, resulting in referral to and surgical intervention by the gynaecology oncology team. Pathology and immunohistochemistry indicated that the tumour was a metastasis from the colon, that prompted for colonoscopy which confirmed that the carcinoma was a metastases from the sigmoid.. ...
Currently, uveal melanoma is not considered within the Lynch syndrome tumor spectrum. However, there are studies suggesting a contribution of microsatellite instability in sporadic uveal melanoma tumorigenesis. We report a 45-year-old woman who was referred for genetic counseling due to a family history of Lynch syndrome caused by a MLH1 mutation. She originally underwent enucleation (removal) of the right eye secondary to a uveal spindle cell melanoma diagnosed at age 25. The tumor recurred 22 years later presenting as an ovarian metastasis and concurrently a microscopic endometrial endometrioid carcinoma, grade 1/3 was diagnosed. Subsequent studies highlighted that the uveal melanoma showed high microsatellite instability and loss of MLH1 and PMS2 protein expression, with no MLH1 promoter methylation or BRAF mutation. We studied a new cellular model for Blooms syndrome consisting of HeLa cells constitutively expressing a shRNA specific for BLM and transiently transfected with a pool of siRNAs directed against sequences other than that targeted by shBLM. These cells display the growth defect, cytogenetic features (high levels of SCE), and mitotic abnormalities (anaphase bridges and lagging chromosomes) typical of Blooms syndrome cells.. We found that SCE formation in response to BLM depletion was strictly dependent on RAD51, showing an epistatic interaction between the two genes. In mammalian cells, RAD51 involvement has been reported in the formation of induced, but not spontaneous, SCEs (32), suggesting that the SCEs in Blooms syndrome cells are induced. The case of adenocarcinoma with human chorionic gonadtropin (HCG), primary in the male gallbladder, is extremely rare. A Medline search has shown only a few similar cases reported. We herein describe a case of primary gallbladder adenocarcinoma associated by ectopic HCG positive tumor cells in a 79-year-old male. Pathological examination showed a mixture of moderately and poorly differentiated adenocarcinoma with ectopic HCG and placental alkaline phosphatase (PlAP) in tumor cells, though the increase of serum or urinary HCG secretion was not confirmed. The literatures were also reviewed. A case of gallbladder cancer with ectopic HCG production is quite rare in the literature, though many similar cases in other site, especially in GI tract, are reported. Embryological consideration suggests the increased frequency of similar cases more than being thought now.

Abstract:. Cutaneous metastasis is a rare manifestation of visceral malignancies, a phenomenon seen in 1% to 3% of all metastasizing tumors. A 62 years old female presented with multiple skin nodules in the neck and jaundice for 2 months. FNAC from the skin nodules revealed hypercellular smear composed of malignant epithelial cells arranged in clusters and glandular patterns. A provisional diagnosis of metastatic adenocarcinoma was given. USG and CT-scan of abdomen revealed a tumor in gallbladder fossa. USG guided FNAC and later on histopathological examination from the biopsy of the gallbladder tumor clinched the diagnosis of moderately differentiated gallbladder adenocarcinoma. Biopsy from the skin nodules revealed adenocarcinoma metastasis in skin. The patient was given postoperative chemotherapy and radiotherapy. Follow up period (3 months) was eventful.. ...
Epithelial ovarian cancer (EOC) is a devastating disease that affects 1 in 72 women in the United States. It is the fourth leading cause of cancer-related deaths among women in the United States and is the primary cause of all gynecologic cancer-related deaths worldwide. Approximately 70% of patients diagnosed with EOC present with stage III or IV disease in which the tumor has disseminated beyond the ovaries and pelvic organs to the peritoneum and mesothelial lining of abdominal organs, including the diaphragm, stomach, omentum, liver, and intestines. Despite current surgical and cytotoxic therapies, 80% of patients diagnosed with advanced EOC develop recurrent disease and only 30% of patients survive 5 years after diagnosis (1).
