To evaluate the prognostic impact of monosomal karyotype on post-remission outcome in acute myeloid leukemia, we retrospectively analyzed 2,099 patients who had achieved complete remission. Monosomal karyotype was noted in 73 patients (4%). Of them, the probability of overall survival from first complete remission was 14% at 4 years, which was significantly inferior to that in patients without monosomal karyotype, primarily due to a high relapse rate of 86%. Monosomal karyotype remained significantly associated with worse overall survival among patients with unfavorable cytogenetics or complex karyotype, and even in patients who underwent allogeneic hematopoietic cell transplantation during first complete remission. They are collected as part of the standard of care in many diseases, particularly in pediatric and cancer medicine contexts. Karyotypes are represented in a unique text-based format, with a syntax defined by the International System for human Cytogenetic Nomenclature (ISCN). While human-readable, ISCN is not intrinsically machine-readable. This limitation has prevented the full use of complex karyotype data in discovery science use cases. To enhance the utility and value of karyotype data, we developed a tool named CytoGPS. CytoGPS first parses ISCN karyotypes into a machine-readable format. Abstract Objective: To investigate whether obstetrical complications are increased in pregnancies with a normal karyotype and an abnormal triple test (TT) (≥1:380 or more) where all anylates were within the normal range. Methods: A retrospective cohort study of women who underwent a TT and delivered in a tertiary medical center, was conducted. Patients were divided into a study group (pregnancies with abnormal TT) and a control group (normal TT). In both groups the anylate concentrations were within the normal range and the karyotype was normal. Demographic and clinical characteristics, antenatal factors, gestational complications and perinatal outcomes were compared between the groups. Results: An abnormal TT result in the presence of normal analytes and a normal karyotype, had no impact on obstetrical complications. We investigated whether octogenarian patients with acute myeloid leukemia enrolled onto Cooperative Group clinical trials and treated with intensive induction therapy could be cured, and whether karyotype and selected molecular markers had any prognostic significance in these patients. Among 138 patients with cytogenetic results, normal karyotype was the most common (47.1%) followed by complex karyotype (14.5%) and sole +8 (9.4%). Among these patients, the relapse-free survival (RFS) rate at 1 year was 37% and 13% at 3 years, and the respective overall survival (OS) rates were 24% and 8%. Whereas the 90 patients who survived beyond 30 days had the same RFS rates, their 1-year and 3-year OS rates were 36% and 11%, respectively.
