The male predominance of the two main histologic malignancies of the small bowel cancer may reflect a role of sex hormones which will be examined in this study. This was a nationwide population-based nested case-control study, based on a cohort of subjects born between 1932 and 2008, as identified in the Swedish Multi-Generation Register. For each case of small bowel cancer, 10 age- and sex-matched controls were randomly selected. Number of children and age at having the first child were analyzed in relation to the risk of small bowel cancer using conditional logistic regression, providing odds ratios (ORs) and 95 % confidence intervals (CIs). A total of 632 female cases and 894 male cases of small bowel cancer were included. No overall increased risk of small bowel cancer was found in parous compared to non-parous women (OR = 1.02, 95 % CI 0.67-1.54). Pembrolizumab is an immune checkpoint inhibitor and is an anti-human programmed cell death-1 (PD-1) monoclonal antibody. Pembrolizumab is used for non-small cell lung carcinoma with high programmed cell death ligand-1 (PD-L1) expression. It has been found that better overall survival can be obtained using pembrolizumab compared to the existing chemotherapy. We report a case of perforation of small intestinal metastasis after pembrolizumab treatment. A 62-year-old man was treated by pembrolizumab for PD-L1 highly expressed lung adenocarcinoma, with multiple metastasis (small intestinal, lymph nodes, and bone). The treatment was stopped owing to drug-induced pneumonitis. One month after drug withdrawal, the patient visited the emergency department of our hospital with the complaint of severe stomachache. Results 24 patients were found to have a small bowel tumour on CE. This represented 1.5% of all patients that underwent CE over seven years. The indications included iron deficiency anaemia (n=9), overt bleeding (n=11) and investigation of complications of coeliac disease (n=4). Patients had a mean number of 4.7 tests prior to CE which included gastroscopy, colonoscopy, small bowel radiology, computed tomographic scanning, mesenteric angiograms and red cell labelled scans. The mean age in the group was 55 years (range 34-83 years). The endoscopic appearances on CE included a mass or ulcerated mass lesion (n=20) or isolated ulceration of mucosa with fresh bleeding (n=3) and multiple ulcers with abnormal underlying mucosa (n=1).. Cyberknife people said it is too large for them to do now but will review post chemo to see if it has shrunk enough. Otherwise they will consider standard radiotherapy but the risks are higher as I have had it before and so the other stuctures in the area are at risk eg small bowel perforation.. Have started to get our heads around the concept of it being palliative and about controlling it rather than curative which is a huge shift from last time but still hold out hope for a possible operation being offered if the response to CRT is ...

They usually present with small bowel obstruction, perforation, bleeding, or, rarely, intestinal intussusception. Only a few case reports have mentioned bowel symptoms due to metastatic malignancies. We report a seldom encountered clinical condition of intestinal intussusception from metastatic lung malignancy. Pathology demonstrated both epithelial and mesenchymal content, and the final diagnosis was carcinosarcoma. This case report indicates that intestinal metastases should be considered in the differential diagnosis for patients with lung malignancy and abdominal symptoms.Keywords:
