Traumatic amputation is one of the major injuries seen as a result of Operation Iraqi and Enduring Freedom with the majority being single trans-tibial amputations (TTA). TTA experience significant reductions in thigh muscle strength of the amputated limb during the first year after amputation. The residual limb is less active in daily functions of standing and walking, resulting in progressive atrophy of the quadriceps muscles in terms of a decrease in thigh diameter which is estimated at 25% of pre-amputation diameter. An intervention that may be useful in TTA rehabilitation is neuromuscular electrical stimulation (NMES). The overall objectives of this study is to test two different approaches to prosthetic rehabilitation as potential treatments for improving muscle strength, pain and functional performance of daily activities in military personnel with TTA. The purpose of the present study was to investigate the neuropsychological functioning of Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) combat veterans with Posttraumatic Stress Disorder (PTSD). More specifically, this study sought to examine whether the neuropsychological functioning of OEF/OIF combat veterans with PTSD and a comorbid mild traumatic brain injury (TBI) significantly differed from those with uncomplicated PTSD across multiple tests of cognitive functioning. To accomplish this objective, the medical records and neuropsychological assessment data of 59 OEF/OIF combat veterans with PTSD and 66 OEF/OIF combat veterans with PTSD and comorbid mild TBI were examined and included for statistical analysis. Results of this study yielded three main conclusions. First, neither group of OEF/OIF combat veterans demonstrated notable performance deficits across the battery of cognitive tests examined in this study. Many service members returning from Iraq and Afghanistan have difficulties adjusting back to civilian life. Research shows that symptoms of posttraumatic stress disorder (PTSD) are particularly high in returning Operation Enduring Freedom (OEF)/Operation Iraqi Freedom (OIF) veterans, and that combat-related nightmares and sleep disturbances are common in veterans with PTSD. This is of concern because people with these problems will often use unhealthy ways of coping. Although combat-related nightmares and difficulty sleeping are highly distressing, there are helpful treatments that do not involve taking medication. One of these treatments teaches specific skills to help people improve their sleep habits and to change their nightmares so that they are less upsetting. While the 9/11 GI Bill gives OEF/OIF veterans access to educational benefits, many veterans will return from deployment with physical and psychological challenges that may impede their academic success. Reagan National Defense Authorization Act for Fiscal Year 2005, (1) describes DODs extended health care benefit and VAs health care services for OEF/OIF veterans; (2) analyzes DOD data to determine the number of OEF/OIF servicemembers who may be at risk for PTSD and the number referred for further mental health evaluations; and (3) examines whether DOD can provide reasonable assurance that OEF/OIF servicemembers who need further mental health evaluations receive referrals.DOD offers an extended health care benefit to some OEF/OIF veterans for a specified time period, and VA offers health care services that include specialized PTSD services.
