The desire to find a more acceptable alternative to Lipiodol led, in the 1940s, to the introduction of the oil-ester based chemical iophendylate (Pantopaque, Myodil). With the assurance from the initial experimental studies that it was safe Steinhausen and associates, in 1942, advocated the routine use of iophendylate for spinal myelography. In sad truth iophendylate was never shown to be safe, particularly in experimental animals. The neurotoxicity of iophendylate turned out to be only slightly less than Lipiodol. It took a period of almost 50 years and about 5 million iophendylate myelograms before this fact even began to enter into the awareness of the medical profession at-large . Primary intradural extramedullary Ewing sarcoma (IEES) is extremely rare, and initial imaging and clinical findings for this tumor mimic those of benign intradural spinal tumors. The present report describes a case of a 35‑year‑old woman who presented with IEES with meningeal seeding, and the literature on the management and clinical course of these tumors was reviewed. An examination revealed right‑side leg dominant rapidly progressive hypoesthesia and motor deficits. MRI identified intradural tumors at the T12‑L1 and L4‑5 levels. Growth and an increase in the number of intradural tumors occurred over 3 weeks. While Im grumbling about draconian measures taken these days, perhaps I should point out something else the current government initiatives have caused. I have a very dear friend who suffers from Arachnoiditis, this is quite a rare condition, but they were retired from their work because of it. At one point they were hospitalised almost totally paralysed. This person has enough courage and determination to get them self mobile again, yet they have to take very strong painkillers to ease the condition. What is the latest indignity this person has been subjected to? They have had their Disability Living Allowance taken off them, which has meant losing their car, and this at a time when they have successfully overcome throat cancer for a second time, and keep ending up in hospital with vasovagal syndrome. Yet when we look at the adhesive arachnoiditis community, women with the disease are far more common than men. Why is that? Dr. Antonio Aldrete suggests that this is because 2/3 of women in childbirth opt to have spinal or epidural anesthesia, and he estimates that 4% develop adhesive arachnoiditis as a result of complications from epidural anesthesia.
