Typically, opioids are omitted before cord clamping due to concern over transplacental drug transfer resulting in respiratory depression in the newborn. A recently published survey of RSI for CD in England showed that only 9% of anesthesiologists routinely administer opioids at induction of general anesthesia.5 This practice has been questioned, especially with the increased availability of short-acting opioids, such as remifentanil. As for the potential adverse fetal consequences, there is a paucity of studies adequately powered to investigate the effect of opioid administration prior to delivery on neonatal outcomes.6,7. Obesity is frequent during pregnancy, increasing the risk for AAGA. Traditionally, lower MACs of volatile agents are used during CD to avoid neonatal depression and uterine atony and considering lower anesthetic requirements during pregnancy. However, the optimal concentration of volatile anesthetics for maintenance of anesthesia after delivery of a newborn has not been fully ...
The study was having been conducted in Wad Medani Teaching Hospital from March to September 2016. The objective of the study was to estimate the incidence of awareness under general anesthesia in Wad Medani Teaching Hospital for patients undergoing general anesthesia. The study designed as observational cross-sectional study; the data was collected by structural questionnaire filled during interview with the patients after their agreement. Cases of awareness and recall have been observed in 13.3 % among the study population. The study concluded that incidence of awareness under general anesthesia in Wad Medani Teaching Hospital is highly significant and more than the international practice and literature. It is also concluded that many factors caused this incidence of awareness include the decreased number of anesthesiologists , lack of proper funding for the hospital and anesthesia department , absence of anesthetic gases monitor sensors in the used anesthesia machine , and absence of ...
According to Steyn-Ross et al. , when the anesthetic concentration is gradually increased or decreased, the equilibrium solution of the model suddenly jumps from one stable branch to another, and this can cause sudden transition between awareness and unconsciousness or vice versa . Because phase transitions make a hysteresis path, emergence and induction phases of anesthesia occur at different drug concentrations.179 In addition, the discontinuous step change in cortical entropy suggests that the cortical phase transition is analogous to a first-order thermodynamic transition in which the comatose-quiescent (unaware) state is strongly ordered, while the active cortical (aware) state is relatively disordered.172 Experimental support for the hysteresis hypothesis was provided by Kelz et al. , who showed in a narcoleptic murine model that the endogenous orexin system affects emergence from, but not entry into, the anesthetized state. In doing so, they suggested that induction of anesthesia and ...
The amygdala is strongly implicated in learning under emotionally charged settings such as fear. An inhibitory avoidance paradigm was used by Alkire and Nathan in which rats were placed in a lighted chamber facing a dark tunnel, which they normally prefer to enter. However, entrance into the dark area was negatively reinforced by electrical shock; rats quickly learned to avoid entering the dark tunnel and remained in the nonpreferred but "safe" lighted environment. When retested the next day, the rats continued to avoid entering the dark tunnel; their memory retention latency (i.e. , time to enter the dark tunnel) was very long, indicating that they remember being shocked. If a low concentration of sevoflurane was administered during the initial training period, the animals quickly entered the dark tunnel the following day, i.e. , sevoflurane prevented avoidance learning. However, after bilateral lesion of the basolateral amygdala, rats exhibited equally long memory retention latencies ...
The experiment was conducted in three phases on a total of 48 native English-speaking students of the University of Oregon (female, 26; mean age, 22.2 years). All subjects were paid for their participation, reported normal or corrected-to-normal vision, and were right-handed as assessed by Edinburgh handedness inventory questionnaires (Raczkowski et al., 1974).. In the first phase, ERPs were recorded in 24 subjects and grand averaged waveform generated. In the second phase, this experiment was repeated on 16 subjects to confirm replicability of the results. In the first two phases, subjects were tested first for implicit recall and then for explicit recall on similar paradigms. In the third phase, eight subjects were tested only for explicit recall to verify that previous exposure to the implicit task did not affect the activity during the explicit task.. For this experiment, a list of 450 words was prepared in such a way that no two words had similar first three letters (stem) and each stem ...
Bispectral index (BIS) is one of several recently developed technologies which purport to monitor depth of anesthesia. BIS monitors can replace or supplement Guedels classification system for determining depth of anesthesia. Titrating anesthetic agents to a specific bispectral index during general anesthesia in adults (and children over 1 year old) allows the anesthetist to adjust the amount of anesthetic agent to the needs of the patient, possibly resulting in a more rapid emergence from anesthesia. Use of the BIS monitor may reduce the incidence of intraoperative awareness in high risk procedures or patients[1] and may also have a role in predicting recovery from severe brain injury ...
Among 2060 CA events, 140 survivors completed stage 1 interviews, while 101 of 140 patients completed stage 2 interviews. 46% had memories with 7 major cognitive themes: fear; animals/plants; bright light; violence/persecution; deja-vu; family; recalling events post-CA and 9% had NDEs, while 2% described awareness with explicit recall of seeing and hearing actual events related to their resuscitation. One had a verifiable period of conscious awareness during which time cerebral function was not expected ...
This study presents the first evidence that TMR during sleep facilitates the emergence of explicit knowledge. Explicit recall of a sequence cued during sleep was significantly greater than recall of an un-cued sequence. Response speed was also influenced by TMR during sleep, with significantly more overnight RT improvement for the cued than uncued sequence. This bias was predicted by fast spindles at motor regions in the learning hemisphere.. Explicit awareness of implicitly learned SRTT sequences can emerge spontaneously after nocturnal sleep (Fischer et al., 2006). The gradual transition between implicit and explicit knowledge facilitates adaptation to a changing environment, and our data suggest that TMR can bias this otherwise spontaneous process. This result suggests that memory reactivation may underpin the emergence of explicit memory during offline consolidation and lends strong support to the active systems consolidation model (Diekelmann and Born, 2010), which proposes that memories ...
A recent article in the November 11 issue of Scientific American claims that alien abductions might be nothing more than "accidental awareness" caused by psychological damage inflicted upon a patient who awakens too soon while undergoing surgery. This hypothesis is presented by Anne Skomorowsky, Assistant Professor of Psychiatry at Columbia University and Psychosomatic Medicine Specialist at the New York-Presbyterian Hospital. I received the Dr. Skomorowskys article from a colleague who questioned if it was joke of some kind. I did as well, until I discovered that it has been printed in a prestigious journal. The article was loosely based upon the largest collection of data on "accidental awareness" ever assembled. It had been released by the Royal College of Anaesthetists. No mention was made of alien abduction. However, Dr. Skomorowskys article was linked to one written in 2008 by David V. Forrest, M.D., a Clinical Professor of Psychiatry at Columbia. Dr. Forrest noted that frequently ...
Large trials have demonstrated that around 1 to 2 per 1000 patients experience some form of awareness. The majority of these do not feel pain although around one third did. The most traumatic case of anesthesia awareness is full consciousness during surgery with pain and explicit recall of intra-operative events. The patient may feel the pain or pressure of surgery, hear conversations, or feel as if they cannot breathe. The patient may be unable to communicate any distress because they have been given a paralytic/muscle relaxant. A fully paralyzed patient is unable to move, speak, blink the eyes, or otherwise respond to the pain. If anesthesia awareness does occur, about 42% feel the pain of the operation, 94% experience panic/anxiety (sometimes because they cannot breathe), and 70% experience lasting psychological symptoms. Currently, the anesthesia provider community accepts that anesthesia awareness occurs yet is in denial about its frequency. Most studies show its incidence is 1-2 ...

Martin LJ, Oh GH, Orser BA. (2009) Anesthesiology. 111(5):1025-35.. Abstract. BACKGROUND: The memory-blocking properties of general anesthetics have recently received considerable attention because of concerns related to intraoperative awareness and postoperative cognitive dysfunction. The goal of this study was to identify the mechanisms by which γ-aminobutyric acid subtype A receptors that contain the α5 subunit (α5GABAARs) induce memory-blockade by etomidate and a pharmacologic strategy to reverse this impairment.. METHODS: The effects of etomidate and the α5GABAAR-preferring inverse agonist L-655,708 on the plasticity of glutamatergic excitatory transmission in hippocampal slices and behavioral memory for spatial navigational and fear-associated memory tasks were studied in wild-type and null mutant mice for the gene that encodes the α5 subunit (Gabra5-/- mice). Long-term potentiation of field excitatory postsynaptic potentials was induced in CA1 pyramidal neurons following ...
