A patient with a ruptured left sylvian fissure arteriovenous malformation (AVM) treated with primary transvenous Onyx embolization, and angiographically cured, is reported. The patient presented with a large intraparenchymal subopercular hematoma and there was angiographic demonstration of a pseudoaneurysm and single draining vein. Trans-arterial approaches were unsuccessful because of the en-passage nature of feeding arteries and hence a transvenous approach was used to embolize the pseudoaneurysm with Onyx. Follow-up angiography at 1 month revealed durable angiographic cure of the AVM. Our case illustrates that in patients with ruptured AVM secondary to pseudoaneurysm and single draining vein, transvenous treatment can be utilized to achieve occlusion of the pseudoaneurysm resulting in AVM cure. This pilot study will examine the use of two common antibiotics-minocycline and doxycycline-as medical therapy for inoperable or partially treated AVMs and giant aneurysms. These drugs, which are tetracycline derivatives, can reduce the levels of a family of enzymes, called matrix metalloproteases, that degrade tissue and thereby cause a reduction in the risk of spontaneous bleeding from AVMs or aneurysm-which is the main cause of stroke associated with these disorders. Surgery is considered curative once postoperative angiography confirms the absence of vessels. The preradiosurgical annual hemorrhage rate was calculated as the number of hemorrhages divided by the patient-years at risk. Because AVM are generally considered as congenital anomalies, we assumed that patients were at risk for hemorrhage from their dates of birth. What is the likelihood of rupture of a brain AVM? This review summarizes their natural history and assesses the risk of spontaneous hemorrhage.
Results Sixty-nine patients (mean age 34 years, male to female ratio: 2) were consecutively treated for an infratentorial AVM. Fifty-seven presented with haemorrhage, 6 with focal neurologic deficits, the 6 cases were diagnosed incidentally. The Spetzler-Martin grade was ,3 in 39 (56.5%) cases. Associated aneurysms were noted in 43.5% of cases. All patients had been treated by endovascular procedures, associated with microsurgical resection in 9 patients and with stereotactic radiosurgery in 6 cases. Our initial experience with use of Onyx for embolization of brain AVMs is encouraging, with an average size reduction of 75% and complete obliteration with embolization alone in 16%. In most patients, one embolization session was sufficient to obtain a complete obliteration or sufficient size reduction for subsequent surgery or radiosurgery. These results are comparable to other studies that have used Onyx.5,6. We think that Onyx has several advantages over n-BCA. The introduction of Onyx in most AVM embolizations in our practice has transformed the embolization of AVMs from a rather unpredictable intervention into a controlled procedure. Prolonged (,60 minutes) and controlled intranidal injection is possible with Onyx. When reflux in the feeding pedicle or early filling of draining veins is noted during injection, the injection can be paused for 1-2 minutes, thus allowing the Onyx to solidify. It usually happens when Im eating, and its scary not knowing how much Im going to have to keep swallowing before it either starts comming out my mouth or what! I dont want it to come out my mouth, because I cant open my mouth very wide right now (due to the avm, and not being able to work w/the therabite since my "wound" is too fresh from the procedure still)! I dont want it to come out my mouth, because I cant open my mouth very wide right now (due to the avm, and not being able to work w/the therabite since my "wound" is too fresh from the procedure still)! When I do bleed and it comes out my mouth, it is alot and globs - UGH! Poor Celia (and Mark), theyve both witnessed this and its not pretty!
