Objectives. This study examined socioeconomic differentials in risk of death from a number of specific causes in a large cohort of White men in the United States. Methods. For 300 685 White men screened for the multiple Risk Factors Intervention Trial between 1973 and 1975, data were collected on median income of White households in the zip...
MRFIT). Known by its acronym, this large randomized trial of preventive interventions initially was thought to demonstrate the inadequacy of preventive interventions, such as cholesterol-lowering regimens, because death rates were not significantly different between the groups with and without intervention. However, more detailed analysis revealed that apparent similarity of intervention and control groups was due, in part at least, to contamination of the control group by publicized intervention methods that many adopted, although it was not meant to be part of their regimen; and that intervention did significantly reduce death rates from coronary heart disease, but the death rates due to violent causes, such as traffic crashes and suicide, were significantly elevated in the intervention group, raising the possibility that lower cholesterol levels were associated with adverse modification of mood. A brief description and formalities for access to data sets from the MRFIT studies are at ...

The Yobe Government has appealed to the Federal Government to float an intervention fund to cushion the effects of the Boko Haram insurgency on affected states.. The Special Adviser to the state Governor on Media and Information, Alhaji Abdullahi Bego, made the appeal in a chat with the News Agency of Nigeria (NAN) in Damaturu on Sunday.. Bego said that while the Yobe government was totally in support of the declaration of emergency in the state, it would also like the Federal Government to set up a fund to assist people affected by the insurgency. ...
Kidney cancer incidence is increasing globally. Reasons for this rise are unclear but could relate to obesity and hypertension. We analyzed longitudinal relationships between hypertension and obesity and kidney cancer incidence in 156 774 participants of the Womens Health Initiative clinical trials and observational studies over 10.8 years. In addition, we examined the effect of blood pressure (BP) on kidney cancer deaths for over 25 years among the 353 340 men screened for the Multiple Risk Factor Intervention Trial (MRFIT). In the Womens Health Initiative, systolic BP (SBP) was categorized in 6 groups from ,120 to ,160 mm Hg, and body mass index was categorized using standard criteria. In age-adjusted analyses, kidney cancer risk increased across SBP categories (P value for trend ,0.0001) and body mass index categories (P value for trend ,0.0001). In adjusted Cox proportional hazards models, both SBP levels and body mass index were predictors of kidney cancer. In the MRFIT sample, there were ...
METHODS. The relationship of cigarette smoking and smoking cessation to mortality was investigated among men screened for and also among those randomized to the Multiple Risk Factor Intervention Trial (MRFIT). RESULTS. Among the 361,662 men screened for the MRFIT, cigarette smoking was an important risk factor for all-cause, coronary heart disease (CHD), stroke, and cancer mortality. These risks, on the log relative scale, were strongest for cancers of the lung, mouth, and larynx. The excess risk associated with cigarette smoking was greatest for death from CHD. Overall, approximately one-half of all deaths were associated with cigarette smoking. Among the 12,866 randomized participants, weak positive associations with duration of cigarette smoking habit and tar and nicotine levels were found with all-cause mortality. For both SI and UC men, substantial differences in subsequent CHD (34-49%) and all-cause (35-47%) mortality were evident for men who reported cigarette smoking cessation by the end of the
To examine the relation between serum fatty acids and coronary heart disease CHD, the authors conducted a nested case-control study of 94 men with incident CHD and 94 men without incident CHD who were enrolled in the Usual Care group of the Multiple Risk Factor Intervention Trial between December 1973 and February 1976. To examine the relation between serum fatty acids and coronary heart disease (CHD), the authors conducted a nested case-control study of 94 men with incident CHD and 94 men without incident CHD who were enrolled in the Usual Care group of the Multiple Risk Factor Intervention Trial between December 1973 and February 1976. After confirming the stability of the stored serum samples, the authors measured serum fatty acid levels by gas-liquid chromatography and examined their association with CHD. In all multivariate models, levels of the cholesterol ester saturated fatty acid palmitic acid (16:0) were directly associated with CHD risk (standardized odds ratio = 1.68; 95% confidence interval 1.10-2.55 in the model that adjusted for total plasma cholesterol level ...
In the 3 major long-term statin trials involving patients with established coronary disease, stroke, or the combined end point of both, stroke and transient ischemic attack were significantly reduced after several years of active treatment.2-4⇓⇓ This finding is not surprising because the same mechanisms by which long-term statin treatment prevents plaque rupture in the coronary circulation are likely to apply in the cerebrovascular circulation, leading to a reduction in atherosclerotic events in both vascular beds. However, the risk of hemorrhagic stroke is significantly increased in subjects with spontaneously occurring low cholesterol levels; for example, the risk of death from hemorrhagic stroke was 3-fold higher in men with serum cholesterol levels ,160 mg/dL in the Multiple Risk Factor Intervention Trial.5 Whether reducing cholesterol to very low levels with drugs increases the risk of hemorrhagic stroke is unknown.. The Myocardial Ischemia Reduction with Aggressive Cholesterol Lowering ...

The analysis included 6,154 participants; 25% (95% confidence interval [CI], 22-28) of black men were at high risk (≥6.67% of fatal CVD in 10 years) compared with only 10% (95% CI, 8-12) of white men. This high-risk subgroup accounted for 55% (95% CI, 49-59%) of CVD deaths in black men compared with 30% (95% CI, 24-35%) in white men. For women, 12% (95% CI, 10-14%) of blacks versus 3% (95% CI, 2-4%) of whites at high risk accounted for 42% (95% CI, 35-46%) of CVD deaths in blacks versus 18% (95% CI, 13-22) in whites. The risk-based multiple risk factor intervention could reduce absolute disparities by 415 per 100,000 (25%) in men and 423 (32%) in women. ...

OBJECTIVE: To test the early effects of a novel one-to-one health professional-delivered intervention designed to increase awareness and thereby promote early presentation of breast cancer among older women. METHODS: Women attending their final routine appointment in the English NHS Breast Screening Programme received a booklet or a booklet supplemented by a brief interview, in addition to usual care. This was a within-group before-and-after evaluation of the two versions of the intervention. The primary outcome was change in the knowledge of breast cancer symptoms from baseline to 1 month postintervention. Secondary outcomes were knowledge of risk of developing breast cancer, confidence to detect a breast change, and likelihood of disclosure to someone close. Levels of cancer worry and any adverse effects caused by the intervention were also monitored. RESULTS: One hundred seventy-six women received the booklet and 116 received the booklet-plus-interview. At 1-month postintervention, the mean number of
A system providing for user intervention in a medical control arrangement may comprise a first user intervention mechanism responsive to user selection thereof to produce a first user intervention signal, a second user intervention mechanism responsive to user selection thereof to produce a second user intervention signal, and a processor executing a drug delivery algorithm forming part of the medical control arrangement. The processor may be responsive to the first user intervention signal to include an intervention therapy value in the execution of the drug delivery algorithm, and responsive to the second user intervention signal to exclude the intervention therapy value from the execution of the drug delivery algorithm. The medical control arrangement may be a diabetes control arrangement, the drug delivery algorithm may be an insulin delivery algorithm, and the intervention therapy value may be, for example, an intervention insulin quantity or an intervention carbohydrate quantity.
To the best of our knowledge, this study is among the first to describe the type of PKT interventions that could be employed in routine clinical encounters with arthritis or cancer patients during which treatment or management were discussed, their impact and the characteristics of PKT interventions associated with effectiveness. We observed that, while eligible studies were few, 9 of 16 had an unclear or high risk of bias, and only one employed theory in the intervention design, all achieved a positive impact on at least one of the outcomes measured. No studies specifically measured or reported patient harms associated with the intervention. We also observed that, although arthritis and cancer differ with respect to modes of treatment and types of clinical outcomes, and PKT interventions were investigated in more cancer studies, the types of PKT interventions were similar for both arthritis and cancer patients. No studies included interventions for clinicians or organizations as a means of ...

The purpose of this randomized controlled trial was to determine the impact of a multicomponent intervention on mammography rates, including a practice facilitator and "best practice" methods. The results suggest that this intervention can improve mammography rates in a range of practice settings. Given the low mammography rates in the state of Oklahoma, having proven strategies that increase the number of women who choose mammography screening could potentially reduce the burden of breast cancer within the state through early detection, when treatment is more likely to be efficacious. These findings are consistent with other studies that have tested multicomponent interventions that included modified academic detailing, patient education, physician reminders, audit and feedback, prompt and reminder systems;28 physician education, and a practice facilitator to help redesign office routines including flow sheets; physician education, provider prompts, patient reminders, printed patient education ...
The first aim of this review was to identify the frequency with which receipt, as defined in the BCC framework, is addressed in health intervention research. Only 19.6% of the studies identified from the forward citation search to report on fidelity were found to address receipt, compared with 33% in a recent review on clinical supervision [85]. Amongst the studies identified, 60.6% assessed receipt in relation to understanding (compared to 0-69% in other reviews [10, 14-17]) and 42.4% in relation to performance of skill (39-65% in other reviews [10, 14-17]). Strategies to enhance understanding were present in only 12.1% (0-79% in other reviews [10, 14-17]) and performance of skill in 21.1% of studies (50-69% in other reviews [10, 14-17]). These results suggest that there has been little improvement over time with regards to the frequency with which receipt is addressed in health intervention research and that there is a need to continue to advocate for better quality evaluations that focus and ...

A major problem with implementation research to date has been the limited understanding about what interventions contain and how they are meant to work. Contributing to this is the frequently scant, or absent, reporting of the process of intervention development. In addition, few studies provide a theoretical basis for the choice and design of interventions to change clinical practice. We have developed an intervention modelling process (IMP) that corresponds closely to the theoretical and early modelling phases of the MRC Framework [13] - explicit stages of development that are currently lacking in implementation research. The systematic approach we have used here in the development of the content of two theory-based behavioural interventions forms the initial part of the IMP.. The contents of the interventions were designed to differentially target specific "determinants of behaviour change" - theoretical constructs that were identified in a previous study as predictive of both the behaviour ...
The main trial will need to address the issues normally posed by randomised controlled trials, such as sample size, inclusion and exclusion criteria, and methods of randomisation, as well as the challenges of complex interventions. Individual randomisation may not always be feasible or appropriate. For example, cluster randomisation is often used for trials of interventions directed at a practice or hospital team. 10 11 Randomised incomplete block designs have also been used to evaluate different approaches to promoting change in professional behaviour.12. It is often not possible to conceal allocation of treatment from the patient, practitioner, and researcher in complex intervention trials. The potential biases of unblinded trials therefore have to be taken into account. Dissimilar levels of patient commitment between intervention and control groups may cause differential dropout, making interpretation of results difficult. When patients have strong preferences, a preference trial design may ...
Are your close friends or any family member struggling with addiction or is facing a problem that is having an adverse impact on his health? Are you brawling to find the best way to cure such addiction? The intervention becomes an important procedure to help your loved ones to take their foremost step towards recovery.. But, what an intervention is? The Family Intervention is basically a gathering where the close friends, relatives and colleagues come together for their loved ones, including a drug addict, substance or alcohol abuse and meet to persuade them to seek help for the problem that has raised. The reluctance of the close friends and family members surely help the addict to take his first step. Some of the tips you should consider while holding such interventions include:. ...
