Provides clinical descriptions, diagnostic guidelines, and codes for all mental and behavioural disorders commonly encountered in clinical psychiatry. The book was developed from chapter V of the Tenth Revision of the International Statistical Classification of Diseases and Related Health Problems (‎ICD-10)‎. The clinical descriptions and diagnostic guidelines were finalized after extensive field testing by over 700 clinicians and researchers in 110 institutes in 40 countries, making this book the product of the largest ever research effort designed to improve psychiatric diagnosis. Every effort has been made to define categories whose existence is scientifically justifiable as well as clinically useful. The classification divides disorders into ten groups according to major common themes or descriptive likeness, a new feature which makes for increased convenience of use. For each disorder, the book provides a full description of the main clinical features and all other important but less ...

We used health administrative data from 2 Canadian provinces (Ontario and Alberta) and the Brigham and Womens Hospital in Boston. All data were linked and analyzed anonymously. The study was approved by the research ethics board at each study site (Sunnybrook and Womens College Health Sciences Centre Research Ethics Board, the University of Alberta Health Research Ethics Board and the Partners Human Research Committee).. All health care activities in Ontario and Alberta are publicly funded and are captured in population-based administrative datasets. These datasets include the Discharge Abstract Database, which captures all hospital admissions and, in the years we sampled, used diagnostic codes from the International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10); the Registered Persons Database, which captures each persons date of death (including deaths that occur out of province); and the National Ambulatory Care Reporting System, which captures ...
The authors of this article address an interesting question: the repercussion on mothers of the symptoms experienced by their children with atopic dermatitis. The objective of the study and the hypothesis advanced are interesting, although ultimately the results were not significant except in the case of physical functioning scores. This result may have been due to the fact that, rather than assessing mental suffering or anxiety, the authors chose to assess the symptoms of a specific psychiatric disorder-obsessive-compulsive syndrome. Obsessive-compulsive disorder is classified as a neurotic, stress-related and somatoform disorder in the 10th revision of the International Statistical Classification of Diseases (ICD-10)1 and is included in the section on obsessive compulsive and related disorders in the 5th revision of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5).2 It affects between 1% and 3% of the general population3 and is characterized by the presence of obsessions ...

All those who commented specifically on the interim final work RVUs for these codes objected to the interim final work RVUs - some citing potential access problems. Commenters suggested that we use more reliable time data. Commenters suggested that valuation should be based on actual time data, which demonstrates that the time for this code has not changed since the last valuation; and thus the work RVUs should not decrease from the CY 2013 values. Among the commenters suggestions were using data from the Function and Outcomes Research for Comparative Effectiveness in Total Joint Replacement (FORCE-TJR), which includes data on more than 15,000 total lower extremity joint arthroplasty procedures, including time in/time out data for at least half of the procedures, and working with the specialty societies to explore the best data collection methods. Background: Left bundle branch block (LBBB) is associated with increased risk of heart failure (HF). Whether a more pronounced widening of QRS duration in LBBB and in other BBBs modifies the risk of HF is unknown.. Methods and Results: This analysis included 14,522 participants from the Atherosclerosis Risk in Communities (ARIC) study (mean age 54 years, 55% women, and 26% blacks) free of HF at baseline. During an average 18 years follow-up, 1,772 incident hospitalized HF, defined by any listing of a HF hospital ICD code (428) or death certificate ICD code (428, I50), occurred. At baseline,377 participants had BBB, categorized as lone RBBB, LBBB, indetermined type BBB (IVCD), and RBBB with left anterior fascicular block (LAFB). Cox proportional hazard analysis was used to examine the association between different patterns of BBB with incident HF. Compared to QRS duration ,100 ms, LBBB, IVCD, and RBBB &LAFB were associated with increased risk of HF (HR=2.9, 1.8, and 2.1, respectively), and so ...

Background/Aim Water main breaks can occur due to the stresses of an aging infrastructure and changing climate. Water main breaks are a public health concern because they can cause pressure transients (specifically, abrupt decreases in water pressure/flow in the pipeline), which can lead to the intrusion of contaminants. In May 2010, a major water main break affected approximately 2 million residents in 30 Greater Boston communities and a boil order was issued. This study examines the association between the main break and subsequent hospital visits for gastrointestinal illness. Methods A case-crossover study was used to assess emergency room (ER) and hospital outpatient visits for gastrointestinal (GI) illness following the main break in 2010. ER and hospital outpatient data were obtained from the State of Massachusetts. GI illness was defined using ICD-9-CM diagnosis codes. Main break details were obtained from archived press releases. Controls were selected using a time-stratified ...

The Charlson Comorbidity Index (CCI) can be automatically calculated from the International Classification of Disease (ICD) code. However, the feasibility of this transformation has not been acknowledged, particularly in hospitals without a qualified ICD coding system. Here, we investigated the utility of coding-based CCI in China. A multi-center, population-based, retrospective observational study was conducted, using a dataset incorporating 2,464,395 adult subjects from 15 hospitals. CCI was calculated using both ICD-10-based and diagnosis-based method, according to the transformation rule reported previously and to the literal description from discharge diagnosis, respectively. A κ coefficient of variation was used as a measure of agreement between the above two methods for each hospital. The discriminative abilities of the two methods were compared using the receiver-of-operating characteristic curve (ROC) for prediction of in-hospital mortality. Total agreement between the ICD-based and diagnosis

Heart failure (HF) is the most common discharge diagnosis across the United States, and these patients are particularly vulnerable to readmissions, increasing attention to potential ways to address the problem. The study cohort was derived from the Healthcare Cost and Utilization Projects National Readmission Data 2013, sponsored by the Agency for Healthcare Research and Quality. HF was identified using appropriate International Classification of Diseases, Ninth Revision, Clinical Modification codes. Readmission was defined as a subsequent hospital admission within 30 days after discharge day of index admission. Readmission causes were identified using International Classification of Diseases, Ninth Revision, codes in primary diagnosis filed. The primary outcome was 30-day readmission. Hierarchical 2-level logistic models were used to evaluate study outcomes. From a total 301,892 principal admissions (73.4% age ≥65 years and 50.6% men), 55,857 (18.5%) patients were readmitted with a total of 64,264

Prognostic scores, and more specifically comorbidity scores, are important and widely used measures in the health care field and in health services research. A comorbidity is an existing disease an individual has in addition to a primary condition of interest, such as cancer. A comorbidity score is a summary score that can be created from these individual comorbidities for prognostic purposes, as well as for confounding adjustment. Despite their widespread use, the properties of and conditions under which comorbidity scores are valid dimension reduction tools in statistical models is largely unknown. This dissertation explores the use of summary comorbidity measures in statistical models. Three particular aspects are examined. First, it is shown that, under standard conditions, the predictive ability of these summary comorbidity measures remains as accurate as the individual comorbidities in regression models, which can include factors such as treatment variables and additional covariates. ...

Our objective was to estimate the cost of inpatient malnutrition conditional upon admitting diagnosis and recorded nutritional treatment. We analysed an anonymised administrative data set for inpatients treated in hospitals in Victoria, Australia 2003-2004. The data set included patient-level cost, clinical markers, demographic and episode variables. The data set was analysed to identify codes, which indicated the presence of malnutrition or its treatment. Linear regression was used to ascertain to what extent malnutrition affected the cost of admission. Controlling for the underlying condition and any treatment administered, recorded malnutrition is estimated to add AU $1,745 per admission. The total cost of coded malnutrition to the Victorian public hospital system in 2003-2004 was estimated to be least AU $10.7 million. Only 1.87% of inpatients were coded as malnourished. As administrative data are known to underreport the prevalence of malnutrition, our estimate represents a credible lower ...

The biological application of nanoparticles (NPs) is a rapidly developing area of nanotechnology that raises new possibilities in the treatment of human cancers. The cytotoxicity was evaluated by MTT and LDH assays. The apoptotic effect of free ICD-85 and ICD-85 NPs on HeLa cells was assessed using caspase-8 colorimetric assay. The MTT assay showed that ICD-85 NPs could enhance the in-vitro cytotoxicity against HeLa cellscompared to the free ICD-85. The IC50 value at 72 h was reduced from 25 ± 2.9 μg/mL for free ICD-85 to 15.5 ± 2.4 μg/mL for ICD-85 NPs. However, LDH assay demonstrated that ICD-85 has dose-dependent cytotoxicity on HeLa cells while ICD-85 NPs exhibited weaker cytotoxicity on same cells. The results also indicate that ICD-85-induced apoptosis on HeLa cells is associated with the activation of caspase-8. Moreover, caspase-8 assay analysis demonstrated that the ICD-85 NPs induced a higher apoptotic rate in HeLa cells compared to free ICD-85. Our results demonstrated that the
One year ago I came across a job I was not aware of: medical coder. In Switzerland, but also in the US, medical reports, written by medical doctors in natural medical language, are translated by medical coders into a series of diagnosis codes using the ICD (International Classification of Diseases). There are other databases of codes to delineate operations performed (e.g. during surgery); CHOP in Switzerland is one example of such a catalogue. Medical coders are highly skilled professionals; they need to understand medical terms and classify them into precise codes, like J13 for Pneumonia due to Streptococcus pneumoniae and J12 for Viral pneumonia, not elsewhere classified.. Medical coding is a highly skilled profession in part because the assignment of code has a deep economic impact: every code has a cost associated with it (cost of treatment, cost of the actions taken during a surgery etc.). It is therefore quite expensive for the clinics to convert reports into billing codes to be ...
