The Federated Council for Internal Medicine (FCIM) desires that the specialty continue to improve the quality of its trainees, the residency training experience, the competence of practitioners, and patient care. To accomplish these goals, the internal medicine community must:. 1. Provide a clearer definition of internal medicine. The distinction of internists from other primary care specialists can be described in part by their training, depth of knowledge, capacity to provide tertiary care, humanistic qualities, and potential for lifelong scholarship.. 2. Continue to attract the most talented medical students into internal medicine residency programs.. 3. Improve the standards of residency programs ...
Primary care tracks in internal medicine residency programs were initiated in the late 1970s in response to the need for more primary care physicians. These programs have shown that internists can be trained effectively in ambulatory settings. Graduates of primary care tracks are more likely to choose careers in general internal medicine than are other internal medicine graduates. Primary care internist training has been accompanied by the growth of academic general internal medicine divisions and by the enhancement of the scientific base for ambulatory care practice and medical education. Expanded ambulatory training opportunities, modeled after current primary care tracks, should be offered to all residents in internal medicine. At the same time, primary care tracks should be revised to address deficiencies in current ambulatory training. These tracks should be designed to commit most resident time to ambulatory care settings, to involve residents in community-based and interdisciplinary ...

N2 - Procalcitonin (PCT) is a circulating polypeptide produced in response to bacterial infections. Studies conducted in the Intensive Care Unit (ICU) setting have demonstrated its utility as a biomarker of bacterial infection and sepsis. Thus, PCT is widely used to distinguish between sepsis and SIRS, and to guide antibiotic therapy. At present sepsis represents a frequent diagnosis among patients admitted to Internal Medicine (IM) departments. Basing on the knowledge derived from ICU studies, the use of PCT has become routine in non-intensive wards, contributing to improve the management of sepsis. However, some differences between the two ...

Antibiotic resistance among pathogenic microorganisms is a matter of worldwide concern. Selective pressure by antimicrobial drugs is by far the most important driving force for the development of such resistance. Antibiotics are among the most commonly prescribed drugs in hospitals and in developed countries around 30% of the hospitalized patients are treated with these drugs [10]. The present study documents that 29.5%of the patients were prescribed antibiotics. This is very similar to the reports from developed countries [10]. Antibiotic prescription was studied over 3 months, the major disadvantage being that seasonal variations in antibiotic prescribing could not be taken into consideration over this short period. The number of samples which were sent for culture and sensitivity testing were small which may also affect the validity of the conclusions drawn about antibiotic resistance.. Among patients admitted to the Internal Medicine ward, there was a preponderance of those above the age of ...
Background. Detecting pulmonary embolism (PE) is a major problem at the ED. Final diagnosis often is made by CT-scan. Due to specialisation in emergency medicine emergency physicians might perform better than internal medicine physicians in detecting PE. To confirm this hypothesis, a single center retrospective cohort-study was performed.. Methods. During the three-month periods (march till may) 2007 and 2008 the findings in all patients undergoing pulmonary CT at our emergency department were reviewed. The investigations were attributed to emergency physician (EP) or internal medicine physician (IP). Negative and positive investigations were evaluated, and the number of patients treated by the respective group were calculated. Statistical analysis was performed by the Students-T-test, and probability levels of 5% were accepted as significant.. Results. In 2007, a total of 2847 patients attended for medical problems. 576 were treated by EP (20.23%). In 2008, 2408 patients searched for medical ...
There is a small Surgery Service at the VA, but for the most part this 60-bed hospital is run by the Internal Medicine Service. This allows for our residents to see what it is really like to work in a community hospital and see virtually everything that comes their way. Add to this the wonderful VA patients, with their "bread and butter" medical problems and their great spirit, and this becomes a great rotation ...

Bedside point-of-care ultrasound (POCUS) is increasingly used to assess medical patients. At present, no consensus exists for what POCUS curriculum is appropriate for internal medicine residency training programs. This document details the consensus-based recommendations by the Canadian Internal Medicine Ultrasound (CIMUS) group, comprising 39 members, representing 14 institutions across Canada. Guiding principles for selecting curricular content were determined a priori. Consensus was defined as agreement by at least 80% of the members on POCUS applications deemed appropriate for teaching and assessment of trainees in the core (internal medicine postgraduate years [PGY] 1-3) and expanded (general internal medicine PGY 4-5) training programs ...

Internal medicine or general medicine (in Commonwealth nations) is the medical specialty dealing with the prevention, diagnosis, and treatment of adult diseases. Physicians specializing in internal medicine are called internists, or physicians (without a modifier) in Commonwealth nations. Internists are skilled in the management of patients who have undifferentiated or multi-system disease processes. Internists care for hospitalized and ambulatory patients and may play a major role in teaching and research. Because internal medicine patients are often seriously ill or require complex investigations, internists do much of their work in hospitals. Internists often have subspecialty interests in diseases affecting particular organs or organ systems. Internal medicine is also a specialty within clinical pharmacy and veterinary medicine. Historically, some of the oldest traces of internal medicine can be traced from Ancient India and Ancient China. Earliest texts about internal medicine are the ...
Documentation must be about the care of the patient; advancing the patients care from one visit to the next, ACP believes. Clinicians should be able to capitalize on the advancing technology of electronic health records (EHRs) without having to duplicate documentation previously captured elsewhere within the patient record.. Value-Based Payment Modifier (VM): CMS proposes to reduce the potential penalties associated with the VM program for the 2016 performance period and hold harmless from downward adjustments those physicians who satisfy quality reporting requirements. This is in line with previous requests from ACP that CMS reduce the penalties associated with the legacy reporting programs and align them with the first year of the Merit-Based Incentive Payment System (MIPS) under the Quality Payment Program (QPP). Physician Quality Reporting System (PQRS): CMS proposes to change the satisfactory reporting criteria for the 2016 PQRS program to be in line with the six-measure quality reporting ...

Modification or prevention of the risk factors for CVD that often coexist with diabetes, such as obesity, hypertension, and high cholesterol, is an important goal for diabetic women. Other lifestyle risk factors that need to be discussed with patients and modified when present include physical inactivity and cigarette smoking.. Today, when health care providers should be more aware than ever of the increased risk for CVD in women with diabetes, it is clear that differences in treatment between the sexes still exist. A study recently published in Diabetes Care3 reported on a cross-sectional analysis that included 3,849 patients with diabetes treated in five academic internal medicine practices between 2000 and 2003. Researchers reviewed medical records and compared the treatment for conditions such as hypertension, dyslipidemia, and CHD in women with the treatment of the same conditions in a group of men with similar risk. They found that women were less likely to be treated for high blood ...

Point-of-care (POC) ultrasound has emerged as a noninvasive, inexpensive and portable diagnostic tool to aide in clinical decision-making as well as the gold standard to procedural guidance. Currently, multiple medical specialties and even medical school curriculum committees have begun to integrate POC ultrasound into training. Internal medicine residents at Highland General Hospital have had opportunities use ultrasound in the ICU and during their Emergency Medicine rotations (without formal training). Recognizing that POC ultrasound is becoming a fundamental skill, in 2012, the Departments of Internal Medicine and Emergency Medicine partnered to develop a new formal ultrasound curriculum with the goal of offering both the didactics and hands-on training needed to become competent and credentialed in POC ultrasound by residency graduation. The Department of Emergency Medicine at Highland Hospital has been a leader nationally and internationally in innovative ultrasound applications, research, ...
