Background: Acute kidney injury (AKI) is a common and serious complication in children admitted to the Pediatric Intensive Care Unit (PICU). The primary objective of our study was to estimate the incidence of AKI in children admitted to PICU. Secondary objective was to study the etiology ,short term outcome and determine the predictors of fatality in children with AKI. Methods: This was a prospective, cross sectional study conducted in children aged 1month to 18 years in the Pediatric Intensive Care Unit of Amrita Institute Of Medical Science, Kochi from Nov 2015 to Oct 2017 based on Acute Kidney Injury Network Criteria(AKIN). Results: The incidence of AKI was 18% .59.3% of the AKI patients were ≤5year of age. The use of health information technology (HIT) to improve patient safety is widely advocated by governmental and safety agencies. Electronic-prescribing and smart-pump technology are examples of HIT medication error reduction strategies. The introduction of new errors on HIT implementation is, however, also recognised. Problem Statement/Question: Severe pediatric Traumatic Brain Injury (TBI) remains a significant cause of morbidity and mortality. Evidence-based guidelines leave gaps in how to best implement and deliver standardized care on the local hospital level. A genome-wide study identified de novo DNA methylation alterations in leukocytes of children at paediatric intensive care unit (PICU) discharge, offering a biological basis for their impaired long-term development. Early parenteral nutrition (early-PN) in PICU, compared with omitting PN in the first week (late-PN), explained differential methylation of 23% of the affected CpG-sites. We documented the time course of altered DNA methylation in PICU and the impact hereon of early nutritional management. We selected 36 early-PN and 36 late-PN matched patients, and 42 matched healthy children. We quantified DNA methylation on days 3, 5 and 7 for the 147 CpG-sites of which methylation was normal upon PICU admission in this subset and altered by critical illness at PICU discharge. Acute kidney injury (AKI) is associated with poor outcome in critically ill children. The reported incidence rate of AKI in children admitted to pediatric intensive care units (PICUs) range from 8% and 89% [1-6]. AKI has been associated with prolonged hospital stay, progression to chronic kidney disease, and a significantly higher relative risk of in-hospital death [7-10]. The epidemiology and outcomes of adult AKI have been validated through large, multi-center studies describing over 20,000 adult patients [11-13]. Allocation of resources to the ODTI have improved, specific education sessions on organ donation for staff now occur, consultant staffing in PICUs has improved and awareness of organ donation among consultants has increased and finally there is an increased awareness among the public of the need for organ donation and transplantation as a result of advocacy groups and the support of the media. To obtain an overall score, each activity is given a cognitive complexity value based on a scale from 1 to 5. The total score is calculated across the 19 domains of care and then classified as I-V in overall complexity of care.. Several studies have described the stresses and responses of parents during a childs hospital stay in a PICU1-3; however, none has focused on parents anxiety when their child is transferred out of the PICU onto a general paediatric ward. This study by Bouvé et al is unique in that it tested a creative, cost effective, brief intervention aimed at helping to reduce parental transfer anxiety. A strength of the study is that it builds on the Lazarus Stress and Coping Model, a well known conceptual model. The sample size ensured adequate power, the participants were randomly assigned, and the outcome (state anxiety) was measured using an appropriate and valid tool.. Finding the ideal sedation regimen in children undergoing mechanical ventilation is challenging. On the one hand, the less sedation you receive, the less likelihood you are to obtain hospital acquired infections, and the earlier you are likely to be free from mechanical ventilation. Children presenting to intensive care with undiagnosed inherited metabolic diseases (IMDs) can pose a significant diagnostic and management challenge to intensivists. This chapter covers the range of presentations of IMDs in neonates in particular, addressing important points to be raised in the history and assessment of children suspected of these conditions-including perinatally. The main subgroups of IMD presenting in the neonatal period are explained: intoxication; energy deficiency; making and breaking complex molecules and seizures; and important differentiating features noted. Baseline investigations of a suspected IMD are listed, and the importance of considering and testing for hyperammonaemia discussed. The emergency treatment of acute crisis or first presentation of IMDs is set out, which includes calculating appropriate glucose intake. OBJECTIVE. Our goal was to determine if there were any changes in risk-adjusted mortality after the implementation of a computerized provider order entry system in our PICU. METHODS. Study was undertaken in a tertiary care PICU with 20 beds and 1100 annual admissions. Demographic, admission source, primary diagnosis, crude mortality, and Pediatric Risk of Mortality III risk-adjusted mortality were abstracted retrospectively on all admissions from the PICUEs database for the period October 1, 2002, to December 31, 2004. This time period reflects the 13 months before and 13 months after computerized provider order entry implementation. Pediatric Risk of Mortality III mortality risk adjustment was used to determine standardized mortality ratios. RESULTS. During the study period, 2533 patients were admitted to the PICU, of which 284 were transported from another facility.
