OBJECTIVE To describe the characteristics and outcomes of mechanical ventilation in pediatric intensive care units during the season of acute lower respiratory infections. DESIGN Prospective cohort of infants and children receiving mechanical ventilation for at least 12 hrs. SETTING Sixty medical-surgical pediatric intensive care units. PATIENTS All consecutive patients admitted to participating pediatric intensive care units during a 28-day period. MEASUREMENTS AND MAIN RESULTS Of 2,156 patients admitted to pediatric intensive care units, 1185 (55%) received mechanical ventilation for a median of 5 days (interquartile range 2-8). Median age was 7 months (interquartile range 2-25). Main indications for mechanical ventilation were acute respiratory failure in 78% of the patients, altered mental status in 15%, and acute on chronic pulmonary disease in 6%. Sepsis is one of the main reasons for non-elective admission to pediatric intensive care units (PICUs), but little is known about determinants influencing outcome. We characterized children admitted with community-acquired sepsis to European PICUs and studied risk factors for mortality and disability. Data were collected within the collaborative Seventh Framework Programme (FP7)-funded EUCLIDS study, which is a prospective multicenter cohort study aiming to evaluate genetic determinants of susceptibility and/or severity in sepsis. This report includes 795 children admitted with community-acquired sepsis to 52 PICUs from seven European countries between July 2012 and January 2016. Vancomycin trough (Vt) concentrations of 15-20 mcg/mL have been associated with an increased rate of renal injury in adults. Current data in pediatrics suggests Vts of 15-20 mcg/mL do not increase the risk of renal injury in children admitted to a pediatric intensive care unit (PICU). The primary objective was to determine if a difference exists in the incidence of renal injury in PICU patients receiving a β-lactam as compared with vancomycin therapy with Vts of 15-20 mcg/mL. In this retrospective study, we report that major complications of HFNC were relatively rare (0.9 per 100 HFNC treatment days), despite an increasing number of subjects being treated with HFNC in our pediatric ICU. HFNC has become an interesting alternative for delivery of noninvasive respiratory support in neonates and children because it is simple to use and well tolerated. However, there is little evidence in the literature regarding the safety of HFNC use in a pediatric ICU.. Over 1 y, we observed a low rate of barotrauma (2 episodes, 1%) and of clinically important epistaxis (1 episode, 0.6%). The role of HFNC in the development of these pneumothoraces is difficult to ascertain. In the first subject, severe bronchospasm alone may have led to a pneumothorax through hyperinflation.
