Of 239 intensive care units in the UK in January 2002, 177 were contributing to the case mix programme. Of these, 92 units participated in our study, together with the three high dependency units. Data from the case mix programme showed that participating and non-participating units were similar in type, size, affiliation, overall percentage of admissions potentially eligible for the study (3.9 v 3.2%), acute physiology scores for COPD,9 and survival in intensive care for potentially eligible admissions. We recruited 832 patients (724 from intensive care and 108 from high dependency). In intensive care units taking part in the case mix programme there were no differences between the 648 patients recruited to our study and the 996 not recruited in terms of age (z test P=0.49), acute physiology score (z test P=0.96), or survival in intensive care (χ2 0.77, P=0.38).. Intensive insulin therapy (IIT) to control blood glucose has been suggested and used in neurointensive care with conflicting results. This review covers the studies reporting on monitoring of cerebral glucose with microdialysis in patients with traumatic brain injury (TBI), subarachnoid hemorrhage (SAH) and ischemic stroke. The major finding of the study is that deliberate hyperventilation did not influence the ICU outcome of brain injury patients. The mean age of patients in this study was 41.3, a little higher compared to a study by Muizelaar et al., but very similar to a study published from Australia and New Zealand (7, 9). Patients with traumatic brain injury are often male young adults (9, 10). There was male preponderance in the present study also. The mean ICU length of stay was 10.5 days in the present study, which was longer than the 3 days reported in a previous study (11). Patients in the previous study were discharged from the ICU to a high dependency unit (HDU). The study hospital does not have a HDU; hence these patients were retained in the ICU for longer time. The mean GCS on admission was 6.3 in the present study, which is similar to a report by Schreiber et al. (12). In another study on those with LBP, 59.5% had more than five years of nursing experience and another 12% had more than 20 years nursing experience [7]. Findings from this study indicate that the nurses working in Intensive Care Unit, ICU (29.1%) and High Dependency Unit, HDU (20.1%) were more likely to report current back pain compared to other units. In both ICU and HDU, most patients are usually dependent, frail and need more help from nurses for their daily activities and transfer compared to those in other wards [9]. Nurses who are working in ICU experienced an increased rate of LBP compared with other CCUs. Similar results were obtained in 65 ICU in 22 South Korean hospitals and among 1345 subjects where 90.3% had back pain [10]. This is particularly relevant in an intensive care unit (ICU), as the family provides a key communicative and practical link between patient and clinician. Family members have been deemed a positive beneficial influence on ICU care and recovery processes, yet they themselves are often emotionally affected after discharge. The SICU usually averages fewer patients than a medical ICU service. The patients come in several flavors. Some undergo complex surgeries where an ICU stay is expected. We see all the post-operative liver transplants, and for me, it is great continuity as I see them pre-operatively in the medical ICU. Sometimes, patients undergoing large abdominal and vascular cases come to the ICU after their surgery. For the most part, these patients have scripted courses; we wake them up, take out the breathing tube, wean the blood pressure medicines, start their rehabilitation, and send them to the floor. These can be pretty satisfying as everyone is getting better. The second set of patients are those who have setbacks after their surgeries. They usually arent doing so great on the floor, with persistent infections or complications. These patients have the courses of our medical ICU patients; they can be long, protracted ICU stays where we battle each complication one by one, trying to stave off setbacks. ...
