Hemidesmosomes have been extensively studied by immunofluorescence microscopy, but due to its limited resolution, their precise organization remained poorly understood. We studied hemidesmosome organization in cultured keratinocytes by 2- and 3-color super-resolution microscopy. We observed that in the cell periphery, nascent hemidesmosomes are associated with individual keratin filaments and that β4 is distributed along rather than under keratin filaments. By applying innovative methods to quantify molecular distances, we demonstrate that the hemidesmosomal plaque protein plectin interacts simultaneously and asymmetrically with β4 and keratin. Furthermore, we show that BP180 and BP230 are characteristically arranged within hemidesmosomes with BP180 surrounding a central core of BP230 molecules. In skin cross-sections, hemidesmosomes of variable sizes could be distinguished with BP230 and plectin occupying a position in between β4 and BP180, and the IF system. Purpose : Whether ocular cicatricial pemphigoid (OCP) treated with stepladder immunomodulatory therapy (IMT) can achieve complete remission is not clear. We performed a retrospective, observational clinical study to evaluate the possibility of long-term complete remission of OCP after withdrawal of IMT. Methods : The authors studied the long-term remission off IMT in 34 patients with biopsy proven OCP between January 2005 and October 2015, 25 men and 9 women . The mean age at onset for OCP was 67.0 years. The long-term complete remission off IMT for OCP was defined as patients with OCP who were withdrawn off IMT for at least 1 year with absence of progressive scarring and active ocular inflammation. The average follow up was 5 years. Demographic, treatment regimen, conjunctival inflammation, stage of OCP (Foster), and best corrected visual acuity were recorded. All patients were on systemic immunosuppression at the time of surgery (dapsone, azathioprine, cyclophsophamide, or combinations) and were treated with peri-operative oral corticosteroids. Patients were evaluated pre- and postoperatively for conjunctival inflammation, conjunctival cicatrization, degree of keratopathy, and disease stage. No patient progressed in disease stage. Vison improved an average of 3.5 Snellen lines (-3 to +8). Worse outcome was associated with chemotherapy intolerance or the presence of any preoperative conjunctival inflammation. Thirteen patients remained on immunosuppressives for the entire study. Corneal ulcers developed postoperatively in three patients in whom continued immunosuppression was not tolerated. We present information on the synthesis of mutant gene products in junctional epidermolysis bullosa patients with laminin-5 defects, coupled with mutation site mapping data, to provide insight into ways that genetic lesions effect gene product function. We found that clinical severity corresponded to the extent of laminin-5 heterotrimer assembly. The hallmark of the most severe form of JEB, H-JEB, is the lack of one of the protein chains needed to assemble laminin-5, causing an absence of the heterotrimer, as shown here for patients G, M, J, and L. The lack of expression could often be traced to the near absence of RNA transcripts (Fig. 3) in the patients we analyzed, several of whom had PTCs. A small sample of tissue (biopsy) is taken from the eyes and usually also the mouth. These biopsies are evaluated under a fluorescence microscope and tested for antibodies associated with MMP. Whilst biopsies from skin and mouth MMP lesions are less than 10% likely to give false negative results, unfortunately biopsies from the eye can give a false negative result in 30-50% of cases. So a positive result from the eye confirms the diagnosis, but a negative result from the eye does not necessarily mean that the diagnosis is not MMP.
