Subcutaneous carbon dioxide insufflations popular in Central und Eastern Europe are a safe and inexpensive treatment modality in complementary medicine and are used mainly in musculoskeletal pain and vascular conditions. However, no rigorous trial exists on their efficacy.. The study is designed as a double blind sham controlled randomized trial to evaluate whether patients with acute non specific neck pain get pain free sooner, if treated with subcutaneous carbon dioxide insufflations compared to sham ultrasound.Since acute non specific neck pain is often self limiting, speed of recovery rather than outcome at fixed points in time is evaluated.. Participants receive either a maximum number of 9 subcutaneous carbon dioxide insufflations or a maximum number of 9 sham ultrasound administered by 4 therapists in a randomized order, thrice weekly. Carbon dioxide gas is insufflated subcutaneously at the locations of neck muscle tenderness. Per tender location 25 ml carbon dioxide gas is ...
During procedure, surgeon (who was blinded to random assignment) could request that unblinded anesthetist change the randomized treatment conditions (called a rescue intervention), if surgeon considered surgical conditions to be unacceptable. This was to be done systematically as follows: If the participant is on standard NMB, the preferred rescue intervention should be to increase the NMB to a depth of 1-2 PTCs; for such a participant the second option (if participant is also on low insufflation pressure) should be the increase of insufflation pressure by 4 mm Hg. If the participant is already on deep NMB, the preferred option should be (if participant is also on low insufflation pressure) the increase of insufflation pressure by 4 mm Hg. The unblinded anesthetist recorded any rescue actions performed. This measure presents the number of participants: with any rescue action performed, with rescue change in depth of NMB, with rescue change in insufflation pressure level ...

In a laparoscopic surgery or the like, access is provided to an opening in a body cavity that contains insufflation gas at an insufflation pressure. The flow of insufflation gas from the body cavity through the opening is limited in response to the sensed insufflation pressure in the cavity.
Prior to the introduction of vaccination with material from cases of cowpox (heterotypic immunisation), smallpox could be prevented by deliberate inoculation of smallpox virus, later referred to as variolation to distinguish it from smallpox vaccination. The earliest hints of the practice of inoculation for smallpox in China come during the 10th century.[46] The Chinese also practiced the oldest documented use of variolation, dating back to the fifteenth century. They implemented a method of "nasal insufflation" administered by blowing powdered smallpox material, usually scabs, up the nostrils. Various insufflation techniques have been recorded throughout the sixteenth and seventeenth centuries within China.[47]:60 Two reports on the Chinese practice of inoculation were received by the Royal Society in London in 1700; one by Dr. Martin Lister who received a report by an employee of the East India Company stationed in China and another by Clopton Havers.[48]. Sometime during the late 1760s whilst ...
We have presented three patients with CC and endoscopic haemorrhagic lacerations on insufflation. Literature review revealed a case report of a patient with CC who showed multiple small haemorrhagic lacerations in the right colon.3 However, no case report has described the large longitudinal colonic mucosal lacerations seen in our cases. To our knowledge, similar haemorrhagic lacerations in the colon have not been described in any other gastrointestinal diseases. Gastrointestinal lacerations have been seen in the oesophageal and gastric mucosa only after cardiopulmonary resuscitation.4 From 1996 to 2000, approximately 100 patients with CC were seen at our institution, and only three cases presented these findings.. A postulated mechanism for the mucosal lacerations in CC might be the rigid character of the collagen containing subepithelium, possibly in combination with endoscopic insufflation, leading to stretching of the mucosa and subsequent tearing. All three patients in our series presented ...
This months Editors Choice, by Sahetya et al, is a small, prospective, physiologic study adjusting driving pressurethrough manipulation of positive end-expiratory pressure(PEEP). When the lowest driving pressure was reached, the stability over time was assessed. Compared to PEEPset using the low PEEP/FIO2 table from the ARDSnet, adecrease in driving pressure was associated with an increasein PEEP in 6 subjects and decrease in 4. Importantly, afterthe change in PEEP, driving pressure stabilized in less than5 minutes. Dries and Marini provide comment, noting thatthe best PEEP is tidal volume dependent. They argue for anempiric approach to PEEP, as the impact of PEEP on drivingpressure is related to lung recruitability.. MI-E operation has remained fairly constant since its inception. Recently, MI-E systemshave added oscillations in an effort to improve ...
We agree that noninvasive positive-pressure ventilation should not be restricted to nighttime. Although we emphasized nocturnal use, we also encouraged daytime use. However, tolerance of this treatment was strictly defined as the ability to sleep nightly for at least 4 consecutive hours with the device, regardless of daytime use. We did use other interfaces, including mouth pieces for daytime ventilation. Every attempt was made to control mucus plugging and secretions by using suction devices, medications, and assisted coughing. Mechanical insufflation-exsufflation was used, but only when it became available at the end of our study ...
In reply:. We thank Antonio M. Esquinas and Giuseppe Fiorentino for their acknowledgments and for their summary of some of the findings from our investigation1 on this understudied topic of laryngeal response patterns to mechanical insufflation-exsufflation.2-4. We certainly agree that the larynx must not be seen merely as an opening at the top of the airway tree. As we have described in our work, the larynx is a highly complex valve that needs to adjust, adapt, and actively respond to a wide range of physiological situations and stressors. Basic features such as structure, function, and innervation are not fully understood in health or in disease. We know that the application of positive airway pressures can lead to laryngeal adduction, even in healthy individuals.2 Moreover, laryngeal collapse due to high ventilatory volumes during ongoing exercise was recently defined and described as an independent disease entity labeled "exercise-induced laryngeal obstruction.5,6 Thus, it should not come as ...
Laparoscopic surgery is currently accepted as the standard of care for the management of adnexal pathology. The safety of laparoscopy during pregnancy has not been fully evaluated. Potential advantages of this approach for the pregnant women include less postoperative pain and more rapid recovery, which in turn may reduce the danger of thromboembolism. The concerns include potential direct trauma to the uterus or fetus, compromise of the uteroplacental perfusion due to increased intra-abdominal pressure associated with carbon dioxide insufflation, and carbon monoxide (CO) poisoning as a result of exposure to smoke generated by thermal energy in the form of laser surgery or bipolar desiccation. The aim of this study was to evaluate the outcome of pregnancy in a series of 29 pregnant women undergoing laparoscopic surgery for the management of an adnexal mass in the first 4 months of gestation. Of the 29 women 1 had a first trimester abortion, and 1 delivered prematurely (32w, 2126g). Of the 24 ...
A 25 years old male patient was admitted to our hospital for laparoscopic cholecystectomy. The patient pre-operative assessment in the anesthesia clinic was normal, no medical history, no previous surgeries, smoker, pre-operative fasting blood sugar, serum electrolytes, urea, and creatinine were all within normal limits. He had a normal chest x-ray. In the operating room standard monitors (ECG, pulse oximetry, non invasive blood pressure (NIBP)) were placed. Initial blood pressure BP and heart rate were 128/65mmHg, 74 bpm respectively. Induction through a proper IV line started with fentanyl (100mcg), propofol (2mg/kg), and rocuronium (0,6mg/kg), intubation done with 8,0mm endotracheal tube. Ventilator settings: pressure controlled ventilation 15mmHg, fresh gas flow rate of 2liters/min, and respiratory rate was regulated to maintain a normal end-tidal CO2 (EtCO2).. The primary trocar inserted and gas (carbon dioxide) insufflation started in the peritoneal cavity. Sudden sinus Bradycardia was ...

Intrabronchial insufflation of acid causes immediate necrosis of the walls of many alveoli. Thrombosis of the alveolar vessels is an associated phenomenon. When a large number of vessels becomes affected, a clot propagates rapidly into the larger supplying vessels. The resulting lesion is indistinguishable from a hemorrhagic infarct.. The infarct-like areas so frequently encountered in influenzal pneumonia, it is not unlikely, have their origin in a similar process.. Infarction depends not only upon thrombosis or embolism of the large vessels, but may be initiated by extensive damage to the capillary bed. By this process infarcts may form in organs which are normally protected by collateral circulation.. ...

We describe the case of an infant undergoing endoscopic repair of a laryngeal cleft where the combination of dexmedetomidine and propofol infusions was used as the anesthetic technique. With this regimen, endotracheal intubation was unnecessary during the perioperative period, the procedure lasted approximately 3h, and the child recovered uneventfully. Historically, the techniques used for microlaryngeal surgery involve the use of intermittent endotracheal intubation and insufflation of halogenated anesthetics to the oropharynx. Given the potential benefits of a technique that obviates the need for endotracheal intubation during microlaryngeal surgery and prevents insufflation of halogenated anesthetics in an open environment, the combination of propofol and dexmedetomidine should be considered as a viable and desirable anesthetic option for infants undergoing complex microlaryngeal surgery Artí-culo en PDF ...
Bryant created an "auto-insufflator, consisting of a bent tube, having at one point an opening, which may be closed by a stopper. Through this opening the powder is introduced; one end is placed in the nose, the other in the mouth, and gentle blowing is performed. A simple rubber tube answers the same purpose. For insufflation into the larynx and trachea one uses special curved instruments. (Figs. 114, 115.) For insufflation into the pharynx and upon the tonsils any tube may be employed, such as a reed, quill, glass or rubber tube, or a special instrument," according to Tissier. (Fig. 116). ...

Carbon dioxide insufflation in esophageal endoscopic submucosal dissection reduces mediastinal emphysema: A randomized, double-blind, controlled trial
Aims and Objective: The aim of this study was to conduct a meta-analysis and systematic review of randomized controlled trials (RCTs) comparing 2 methods of colonic insufflation for elective colonoscopy, that is, carbon dioxide (CO2) or air, and to evaluate their efficiency, safety, and side effects. Materials and Methods: Prospective RCTs comparing CO2 versus air insufflation for colonic distension during colonoscopy were selected by searching PubMed, Medline, Embase, Science Citation Index, Current Contents, and the Cochrane Central Register of Controlled Trials published between January 1980 and October 2014.The outcome variables analyzed included procedural and immediate postprocedural pain (during, end, or within 15 min after procedure), early postprocedural pain (between 30 and 120 min), intermediate postprocedural pain (360 min) and late postprocedural pain (720 to 1140 min), cecal/ileal intubation rate, cecal/ileal intubation time, and total colonoscopy examination time. These outcomes were

article{d69173db-9180-4395-8614-5dd71b3a01f2, abstract = {The aim of the present study was to characterize the airway effects of U46619, a stable thromboxane A2 mimetic, instilled into the trachea of guinea pigs in vivo, and to investigate the role of different mediators in these effects. The airflow obstruction was evaluated by measurement of airway insufflation pressure (P1) and plasma extravasation by quantification of Evans Blue dye (EBD) in airways. U46619, given as a single dose to each animal (1 pmol-10 nmol), caused a dose-dependent increase in P1 and extravasation of EBD. The threshold dose required to induce an increase in P1 was 30 times lower than the threshold dose necessary to evoke EBD extravasation. The role of inflammatory mediators was studied when 10 pmol (inducing only the increase in P1) or 10 nmol (inducing the increase in both P1 and EBD extravasation) of U46619 was administered. The effects of both doses of U46619 were abolished by ICI192,605, an antagonist of prostanoid ...

A 2 cm incision is made just lateral and inferior to the umbilicus on the same side as the hernia. The muscle is retracted laterally so as to expose the posterior rectus fascia. Blunt dissection with the s-retractors or finger opens the preperitoneal space. (Figure 1B). The dissection of this space is facilitated by the use of a three-component dissecting balloon. This is inserted into the space via the umbilical incision. The bulb insufflator device is used to expand the balloon. During the insufflation, the surgeon monitors the dissection process with the laparoscope (Figures 2A and B). The expansion is gradual. It is important to have all the creases in the dissecting balloon flatten out. The balloon is desufflated and removed. The smaller stay balloon is then inserted (Figure 2C) and filled with 40 mL of air. It is used to hold traction on the fascia by being retracted back and locked. This is attached to the CO2 insufflator, which is set to a pressure of 15 mmHg. The patient is placed in a ...

Cardiac arrest is one of the most challenging presentations managed by emergency physicians. Stress, diagnostic uncertainty, and the time-sensitive nature of the presentation lead to challenging management decisions. However, following the wide dissemination of bedside transthoracic ultrasound, emergency physicians have used transthoracic echocardiography (TTE) for diagnosis, procedural guidance, and prognosis in patients with cardiac arrest [1-5]. While TTE has many benefits in the management of cardiac arrest, it is not without shortcomings. It has been associated with prolonged interruptions in chest compressions [6,7], provides inadequate imaging in up to half of critically ill patients [8], and may be obscured by body habitus, gastric insufflation, and defibrillator pad placement. Conversely, transesophageal echocardiography (TEE) has been shown to provide adequate images in nearly all patients [9], and is easily performed by inserting the ultrasound probe into the esophagus to a typical ...
