This is a double blinded randomized controlled trial in patients undergoing colon open surgery. The purpose is to evaluate the effectiveness of two different analgesic techniques on functional recovery after surgery.. Twenty five patients will receive thoracic epidural analgesia plus patient controlled analgesia (PCA) (epidural analgesia group) and 25 patients wound infiltration of local anesthetic plus PCA (wound infusion group).. Hypothesis: the postoperative recovery of patients receiving local anesthetic wound infusion will be faster than patients receiving thoracic epidural analgesia.. Functional recovery, pain intensity, opioid consumption and side effects, length of hospital stay and biological markers of inflammation after surgery will be measured in both groups. ...

This phase I clinical trial will evaluate the safety and efficacy of intra-coronary injection of AC133 selected autologous marrow-derived stem cells in patients with chronic coronary artery occlusion. A clinical study to determine the therapeutic potential of marrow-derived stem cells as an adjunct therapy to current standard therapies for CAD is warranted. The current initiative is to investigate a model of chronic myocardial ischemia and (1) to determine whether intra-coronary injection of selected autologous marrow-derived AC133 stem cells is reasonably safe for use in humans and (2) if this treatment shows any improvement in coronary perfusion, as assessed using non-invasive imaging. This study is structured to evaluate the feasibility and safety of autologous AC133+ bone marrow-derived stem cell via intra-coronary injection into documented ischemic but viable myocardial zones via established collateral vessels. The epicardial vessel that normally supplies the ischemic zone must be 100% ...
|p|Continuous interscalene blocks involve the insertion of a perineural catheter along the brachial plexus to provide a continuous local anesthetic infusion with or without patient-controlled bolus dosing. Most commonly used to produce surgery-specific analgesia for major shoulder surgery, continuous interscalene blocks may also play a role in improving surgical outcomes by facilitating early physical therapy as a result of superior pain control. This article reviews the current evidence supporting the benefits of continuous interscalene blocks in improving the quality and duration of postsurgical recovery, as well as indications, contraindications, potential complications, and adverse effects. Although a limited number of studies on continuous interscalene blocks have been published, findings suggest improvements in the time to achieve set discharge criteria, pain control, and mobility that meet or exceed surgeon-defined physical therapy goals and patient satisfaction. While the benefits in terms of
Lidocaine may be absorbed following topical administration to mucous membranes, its rate and extent of absorption varies depending upon concentration and total dose administered, the specific site of application and duration of exposure. In general, the rate of absorption of local anesthetic agents following topical application occurs most rapidly after intratracheal administration. Lidocaine is also well-absorbed from the gastrointestinal tract, but little intact drug may appear in the circulation because of biotransformation in the liver. Lidocaine is metabolized rapidly by the liver, and metabolites and unchanged drug are excreted by the kidneys. Biotransformation includes oxidative N-dealkylation, ring hydroxylation, cleavage of the amide linkage, and conjugation. N-dealkylation, a major pathway of biotransformation, yields the metabolites mono-ethylglycinexylidide and glycinexylidide. The pharmacological / toxicological actions of these metabolites are similar to, but less potent than, ...

Dr. Vanden Berge has recently stared performing stem cell injections in his office here in Stephenville. Stem cell injections are being used in the treatment of many conditions including osteoarthritis. For patients who are not ready for surgical intervention, stem cell therapy may offer an alternative. At VB Orthopaedics, stem cells are harvested from your own bone marrow and injected into the affected joint or around ligaments and tendons. The process is completed in the office, and most patients report relief in joint pain, swelling, and stiffness.. Can stem cells improve your joint pain without surgery?. . As we age, our bodies can deteriorate leading to pain, stiffness, and swelling. Osteoarthritis is one of the most common chronic joint conditions, but there are numerous ailments that stem cells are used to treat. If you are suffering from joint pain, are stem cells an option for you?. . What are stem cells?. . Stem cells come from your own body, and they are found mainly in bone ...
A combined spinal-epidural anesthetic was performed with a 25-gauge Sprotte needle through an 18-gauge Tuohy needle. After injection of 12 mg hyperbaric bupivacaine, an epidural catheter was placed. A T4level was obtained and a cesarean section was performed. At amniotomy, the amniotic fluid was suctioned to the wall suction and then switched to suction connected to the cell saver. This was done to minimize the amniotic fluid contamination of the salvaged blood. A viable female infant was delivered who was subsequently given APGAR scores of 7 and 9 at 1 and 5 min, respectively. No meconium was noted at delivery. The placenta was delivered with little difficulty; however, attempts to externalize the uterus for repair were hindered by multiple adhesions of the bowel to the uterus. These adhesions led to continuous wound oozing. During the course of uterine repair, approximately 1,800 ml blood was lost and salvaged into a reservoir with a 40-[micro sign]m screen. This blood was washed with a ...
A new research study published online as an article in press on the Journal of the American College of Surgeons website in advance of print publication concluded that patients who participated in an enhanced recovery program following colorectal surgery left the hospital sooner and had a significantly lower hospital costs than those following a traditional pattern of care. The focus of the enhanced recovery program is to speed patients recovery by reducing the bodys stress responses to a surgical procedure and promoting earlier return of bodily functions. In this study patient stays were trimmed by nearly two days, on average, compared to patients receiving traditional care and there was no increase in readmissions in the first month following surgery. In addition, the rate of surgical site infections dropped by nearly 10 percent.. Enhanced recovery includes but goes beyond fast tract surgery in which there is minimal invasive techniques and encouragement of early mobilization and eating ...

My research group conducts bench to bedside translational studies directed at advancing our mechanistic understanding of cardiac pathophysiology as well as developing new diagnostic and therapeutic approaches for the management of patients with chronic ischemic heart disease. Our ongoing areas of preclinical investigation apply proteomic approaches to identify intrinsic adaptive responses of the heart to ischemia and studies examining the ability of intracoronary stem cell therapies to stimulate endogenous cardiomyocyte proliferation and improve heart function. We are particularly interested in understanding how elevations in heart filling pressures and reversible ischemia lead to cardiomyocyte cell death and heart dysfunction. We also conduct basic and patient-oriented research to understand how chronic ischemia modifies the cellular composition and sympathetic innervation of the heart to help develop new approaches to identify patients at risk of sudden cardiac arrest from ventricular ...
BACKGROUND: Lynch syndrome is a highly penetrant cancer predisposition syndrome caused by germline mutations in DNA mismatch repair (MMR) genes. We estimated the risks of primary cancers other than colorectal cancer following a diagnosis of colorectal cancer in mutation carriers. METHODS: We obtained data from the Colon Cancer Family Registry for 764 carriers of an MMR gene mutation (316 MLH1, 357 MSH2, 49 MSH6, and 42 PMS2), who had a previous diagnosis of colorectal cancer. The Kaplan-Meier method was used to estimate their cumulative risk of cancers 10 and 20 years after colorectal cancer. We estimated the age-, sex-, country- and calendar period-specific standardized incidence ratios (SIRs) of cancers following colorectal cancer, compared with the general population. RESULTS: Following colorectal cancer, carriers of MMR gene mutations had the following 10-year risk of cancers in other organs: kidney, renal pelvis, ureter, and bladder (2%, 95% confidence interval [CI] = 1% to 3%); small ...

Arthritis is very painful and the pain can keep getting worse if you dont do anything to treat your arthritis. Arthritis can be hard to treat and you often need to take painkillers on a long term basis which can be hard on your liver and stomach. Surgery is painful and you have to deal with long recovery times and you still might not be able to live pain free. A better alternative is to use stem cell injections for arthritis.. You get arthritis when your joints become inflamed. This is due to the cartilage between your bones beginning to break down. The cartilage can start to break down for many reasons. Age and being overweight can play a role and so can heredity. If there is a history of arthritis in your family, you are more likely to get it.. Once the cartilage begins to break down, you have bone rubbing on bone. This causes pain, swelling, and stiffness. It might take a long time to get going in the morning and you might have a hard time getting out of bed until you stretch. Your joints ...

In recent years, significant research efforts have been dedicated to improving the treatment options and visual outcomes of patients with age-related macular degeneration (AMD). The introduction of VEGF inhibitors offered a means of preventing permanent vision loss, and even improving vision. However, the necessity of monthly or bimonthly intravitreal injections still remains. Presentations at ARVO 2015 highlighted several novel delivery systems in development to address this limitation.. DeCogan et al presented a de novo-designed system of cell-penetrating peptide constructs fused to a therapeutic protein transduction domain (DeCogan F: 4147). In vivo, the system successfully transported microgram quantities of macromolecules, such as the large monoclonal VEGF inhibitors, in a topically administered drop that penetrated to the back of the eye.. Other presentations focused on slow-release depots of VEGF inhibitors. Initial in vitro pharmacokinetic work was presented on controlled-release polymer ...

The oncolytic mutant vesicular stomatitis virus VSVΔ51 achieves robust efficacy in multiple extracranial tumor models. Yet for malignancies of the brain, direct intratumoral infusion of VSVΔ51 causes lethal virus-induced neuropathology. Here, we have developed a novel therapeutic regime that uses peripheral immunization with a single sub-lethal dose of VSVΔ51 to establish an acute anti-viral state that enables the safe intracranial (IC) infusion of an otherwise lethal dose of VSVΔ51 within just 6 hr. Although type I interferons alone appeared insufficient to explain this protective phenotype, serum isolated at early time points from primed animals conferred protection against an IC dose of virus ...

DESCRIPTION (provided by applicant): With few exceptions, stem cell injections into the heart produce short-term improvement in cardiac function but a notable lack of integration and differentiation. This temporal improvement in cardiac function may be due to paracrine factors, but the process is not well understood. Recently, mitochondria have been observed to be transferred through tunneling nanotubes (TNTs) into myocytes, which might explain the transient effects of stem cell injections on improved cellular function. An understanding of mitochondrial transfer between stem cells and myocytes might enable rescue of failing cardiomyocytes by directed transfer of functional mitochondrial replacements. Significantly, there are pediatric genetic mitochondrial cardiomyopathies that in which such transfer could be particularly useful as a therapy. TNT formation has been observed in many settings; however, difficulty in capturing such a small structure using currently available tissue-sectioning ...

Consider the linear regression model, $$ y_i=f_i(\boldsymbol{x},\boldsymbol{\beta})+\varepsilon_i, $$ where $y_i$ is the response or the dependent variable at the $i$th case, $i=1,\cdots, N$ and the predictor or the independent variable is the $\boldsymbol{x}$ term defined in the mean function $f_i(\boldsymbol{x},\boldsymbol{\beta})$. For simplicity, consider the following simple linear regression (SLR) model, $$ y_i=\beta_0+\beta_1x_i+\varepsilon_i. $$ To obtain the (best) estimate of $\beta_0$ and $\beta_1$, we solve for the least residual sum of squares (RSS) given by, $$ S=\sum_{i=1}^{n}\varepsilon_i^2=\sum_{i=1}^{n}(y_i-\beta_0-\beta_1x_i)^2. $$ Now suppose we want to fit the model to the following data, Average Heights and Weights for American Women, where weight is the response and height is the predictor. The man or boy test was proposed by computer scientist Donald Knuth as a means of evaluating implementations of the ALGOL 60 programming language. The aim of the test was to distinguish compilers that correctly implemented recursion and non-local references from those that did not. There are quite a few ALGOL60 translators in existence which have been designed to handle recursion and non-local references properly, and I thought perhaps a little test-program may be of value. Hence I have written the following simple routine, which may separate the man-compilers from the boy-compilers. - Donald Knuth begin real procedure A(k, x1, x2, x3, x4, x5); value k; integer k; begin real procedure B; begin k := k - 1; B := A := A(k, B, x1, x2, x3, x4); end; if k

We saw in the preceding section that non-linearity was one of essential features of turbulence. When small disturbances grow large enough to interact with each other, we enter a whole new world of complex behavior. Most of the rules we learned for linear system do not apply. Since most of your mathematical training has been for linear equations, most of your mathematical intuition therefore will not apply either. On the other hand, you may surprise yourself by discovering how much your non-mathematical intuition already recognizes non-linear behavior and accounts for it. Considering the following simple example. Take a long stick with one person holding each end and stand at the corner of a building. Now place the middle of against the building and let each person apply pressure in the same direction so as to bend the stick. If the applied force is small, the stick deflects (or bends) a small amount. Double the force, and the deflection is approximately doubled. Quadruple the force and the ...
