This study extends the examination of the relationship between nurse staffing and patient outcomes to the less researched area of critical care. The methodological approach was appropriate for the study purpose, but the design would have been strengthened if methods were used to account for the impact of patient characteristics on outcomes. There is considerable evidence suggesting that the vast amount of variance in outcomes, particularly mortality, arises from the risk associated with patients own characteristics. In addition, nursing costs were determined by using hospital accounting data rather than hours in the hospital roster. The yearly average cost was divided by 12 to determine the monthly costs. Seasonal staffing variations therefore would not be captured, and may have masked possible relationships with the patient outcomes under examination.. The vibrating assembly (14) is impedance matched to the frequency of a handpiece employing a straight connecting body of substantially equal length and dimension whereby straight and angulated handpieces may be interchangeably used in an ultrasonic surgical system employing a common drive and common operative tips. In a preferred embodiment, the ultrasonic tranducer has a nodal plane midway between its ends and is supported at this nodal plane.
A multiple stage anti-microbial apparatus and methods are disclosed, which provide capacity to process a handpiece between patients including soaking, de-overburdening via use of ultrasonic energy, treating with exposing to anti-microbial heat, accommodating coordinated though separate autoclaving, and accommodating contiguously packaging in a heat shrunk sheath after multiple anti-microbial treatment to prevent later contamination through the air. The distal portion of the heat shrunk sheath is initially removed along weakened paths to expose the operative head of the handpiece, accommodating grasping of the handpiece handle through the remainder of the sheath during use of the handpiece on the patient. After such use, the remainder of the sheath is removed along a weakened path and discarded. The anti-microbial apparatus comprises a portable casing with one or more exterior lids or covers. This was a difficult week to get through, mainly because the cold that has been running rampant through the school population finally got me. Even after taking a day off, I havent had as much energy. Our neck of the country was also settling in for a cold spell when the boiler gave out in the main building. It made the classrooms quite cold. You just never know what teaching environment you will have in these older buildings. I am looking forward to the three day weekend..
