I read with interest the review article of Martherus and co-workers entitled Supporting breathing of preterm infants at birth: a narrative review. Despite the ongoing focus on the respiratory transition at birth, clinical strategies that improve outcome are lacking. As such the authors should be congratulated on this timely review. As they emphasise, there is still a dearth of human data, and much of the literature regarding pressure strategies at birth remains in the domain of preclinical studies. Previous research investigating structural neurodevelopmental alterations in individuals who were born very preterm demonstrated a complex pattern of grey matter changes that defy straightforward summary. Here we addressed this problem by characterising volumetric brain alterations in individuals who were born very preterm from adolescence to adulthood at three hierarchically related levels - global, modular and regional. We demarcated structural components that were either particularly resilient or vulnerable to the impact of very preterm birth. We showed that individuals who were born very preterm had smaller global grey matter volume compared to controls, with subcortical and medial temporal regions being particularly affected. Several small trials have suggested that supplementing the enteral diet of very preterm infants with lactoferrin, an antimicrobial protein processed from cows milk, prevents infections and associated complications. The aim of this large randomised controlled trial was to collect data to enhance the validity and applicability of the evidence from previous trials to inform practice. METHODS: In this randomised placebo-controlled trial, we recruited very preterm infants born before 32 weeks gestation in 37 UK hospitals and younger than 72 h at randomisation. An interesting study by Tulaimat and Mokhlesi1 regarding the accuracy and reliability of extubation decisions that recently appeared in the Journal merits additional comment. The implicit study question is whether an informed decision to extubate following a successful spontaneous breathing trial is any better than random chance. By study design, the clinical vignettes were selected so that, if a decision to extubate was made by coin flip, without any clinical information, the sensitivity and specificity (as defined in the study) would be expected to reach 50%. It was disappointing that, overall, experienced clinicians performed marginally better than a coin flip in predicting extubation success (ie, 57% sensitivity), but they were highly inaccurate in predicting weaning failure (ie, 31% specificity)..
