The degree of the psychological and behavioural component of illness may at times be out of proportion with objective physical pathology. Delphi XE2 introduced namespaces across the runtime library. This stressed unit name lookup inside the compiler, and led to some severe performance regressions in certain cases. So during the runup to the XE2 release, I fired up a profiler and started investigating. It turns out there were numerous situations where lookups were being performed repeatedly with the same arguments, and logically the results should have been consistent across these repeated calls. A relatively cheap and easy fix seemed to be memoization. So I added a caching infrastructure to the compiler and used the profiler to guide me where to inject it.. Placental infection measured by placental smear at delivery is a standard indicator, widely used to characterize malaria infection in pregnant women. However, a single measure can hardly reflect the entire history of infection during pregnancy. To investigate the relation between this indicator and peripheral infection during pregnancy, we used data collected in a randomized trial of malaria prophylaxis in 928 pregnant women in Burkina Faso, 1987-1988, during which repeated measures of peripheral infection were taken. We analyzed placental infection using a logistic model, with two methods for handling missing data. Peripheral infection during two periods of pregnancy was significantly related to placental infection at delivery, before the fifth month: OR = 2.9 [1.3; 6.3]; after 7 months: OR = 4.9 [2.7; 8.8]). Therefore, an early peripheral infection may persist throughout gestation, and placental infection is a good indicator of the womens parasitological status during pregnancy.

In our study, we assessed the reproducibility (test-retest reliability) of activations in the Broca area and the left insula for an object naming and a combined naming/noun generation fMRI paradigm. Both tasks are often used in fMRI for the localization of language functions, and naming is the standard task in IOM. The aim of our study was to explore the reproducibility of activations in the Broca area and the insula within a clinical framework, in which fMRI data complement IOM or are to be validated with IOM. Psychological assessment of children has become increasingly common. One important aspect of an assessment is the feedback, when the result is communicated to the parents. The aim of the present study is to investigate parents experience of assessment feedback, with focus on the relation to the assessor, validation, invalidation and emotions in relationship to the experience of having obtained a new understanding of the child. The study is a cross-sectional study including 33 participants. The results show that positive emotions and invalidation are the two best predictors as to what degree the parents experience that they have been given a new understanding of their child. The study design prevents any conclusions regarding the directions of relationships. Elite overhead throwing athletes with rotator cuff tears represent a unique group of patients with an ultimate goal of returning to their previous level of competition. We hypothesized débridement of small partial-thickness rotator cuff tears would return the majority of elite overhead throwing athletes to their previous level of competition. Preoperative and intraoperative findings on 82 professional pitchers who had undergone débridement of partial-thickness rotator cuff tears were evaluated using our database. We obtained return to play data on 67 of the 82 players (82%); 51 (76%) were able to return to competitive pitching at the professional level and 37 (55%) were able to return to the same or higher level of competition. Of the 67 patients, 34 pitchers returned a questionnaire with a minimum followup of 18 months (mean 38 months; range 18 to 59 months). These perceptions can predict adjustment and quality of life in patients with single conditions. However, multimorbidity (i.e. patients with multiple long-term conditions) is increasingly prevalent and a key challenge for future health care delivery. The objective of this research was to develop a valid and reliable measure of illness perceptions for multimorbid patients. Methods Candidate items were derived from previous qualitative research with multimorbid patients. Questionnaires were posted to 1500 patients with two or more exemplar long-term conditions (depression, diabetes, osteoarthritis, coronary heart disease and chronic obstructive pulmonary disease). Data were analysed using factor analysis and Rasch analysis. Rasch analysis is a modern psychometric technique for deriving unidimensional and intervally-scaled questionnaires. Results

We used animal models for experimental brain ischemia as a paradigm of acute brain lesions and additionally investigated a large cohort of stroke patients. We analyzed release of HMGB1 isoforms by mass spectrometry and investigated its inflammatory potency and signaling pathways by immunological in vivo and in vitro techniques. Features of the complex behavioral sickness behavior syndrome were characterized by homecage behavior analysis. HMGB1 downstream signaling, particularly with RAGE, was studied in various transgenic animal models and by pharmacological blockade. As the first physician in my family, I had little background or advice for choosing a surgical residency. I ranked my match list based on perceived academic prestige first, and location second. I matched to my second choice - which turned out to be a brutal surgical program in a lovely city, where I still reside today. The programs cache opened a lot of doors for me, in terms of fellowship and job opportunities after training was completed. I met two very strong mentors in my surgical program, one a chief resident when I was an intern, and another was the program director. Somehow I managed to graduate as a chief resident, despite having horrible Absite scores a couple of years, some emotional and relationship problems, and probably some burn-out. We tested whether social status in house finches ( Haemorhous mexicanus) modulates the acute phase response or aggressive interactions with flockmates. We treated subordinate or dominant finches within captive flocks with lipopolysaccharide (LPS) to stimulate an acute phase response (APR), and quantified mass loss, activity, foraging behaviours, and agonistic interactions. Subordinate finches lost more mass than dominants in response to LPS, but social status did not influence the expression of sickness behaviours (activity and foraging) upon LPS injection. LPS-injected subordinate birds experienced reduced aggression from mid-ranking but not dominant flockmates, indicating status-mediated effects of sickness behaviour on agonistic interactions.
