Discussion: Pain relief is very often not given due importance in our (Indian) circumstances. The importance of keeping the patient free of pain, during the entire length of treatment cannot be overemphasized. This is even more vital in the early post-op period, because the patient with pain is not going to be very cooperative in the physiotherapy and mobilization. Pain also will cause the adoption of protective postures such as flexion of the knee, plantarflexion of the ankle, which later develop into contractures one of the commonest complications of the Ilizarov method. Adequate doses of non-steroidal anti-inflammatory and or narcotics must be used. The use of NSAIDs is generally discouraged because of possible, unproved deleterious effects on regenerate formation; but for a short while they can be used ...

Aim and objective: Ilizarov hip reconstruction (IHR) is a traditional method of salvaging chronic adolescent problem hips but faces practical problems from external fixators leading to reduced compliance. We present the same reconstruction utilising only internal devices with a modification in technique and aim to review early results. Materials and methods: We retrospectively evaluated eight patients between 2014 and 2017 with chronic painful hips treated by a two-stage reconstruction; stage 1 included femoral head resection and pelvic support osteotomy using double plating, while stage 2 comprised distal femoral osteotomy avoiding varus followed by insertion of retrograde magnetic nail for postoperative lengthening. Patients continued physiotherapy postoperatively while protecting from early weight-bearing. Results: At mean follow-up of 19 months (range 6-36), all osteotomies healed with bone healing index of 47 days/cm (range 30-72). Pain improved from 8.3 (range 7-9) to 2 (range 0-6), while ...

Ilizarov has been named after the orthopaedic surgeon Gavril Abramovich Ilizarov from the Soviet Union, who pioneered the technique.1 The apparatus is based on the principle which Ilizarov called as the theory of tensions.2 It can be used for the treatment of infected non-union and mal-union of bones, limb lengthening, height increase and badly comminuted fractures (multiple fragments) in the limbs, even with skin loss.3-5 It is used in surgical procedures that are not amenable with other techniques.6 In addition to being used to support a fractured limb, the Ilizarov apparatus is also commonly used to correct deformity through distraction osteogenesis.7 This method is preferred over conventional treatment options such as internal fixation or cast where there is a high risk of infection or the fracture is of such severity that internal fixators are unworkable.8 The circular construction and tensioned wires of the Ilizarov apparatus provide far more structural support than the traditional ...

The surgical approach is required for the cleaning of necrotic bone tissues and infected soft tissues.. The resection of the infected area and the artholiths lead to a physiological shortening of the limb. The procedure that is universally recognised as being the most effective for restoring normal anatomic function is the one theorised and perfected by Ilizarov.. Thanks to a frame made up of metal rings and secured to the bone, the Ilizarov apparatus is able to carry out the bone transport of separated tissues and encourage their elongation.. The two bone segments in question can thus rely on a fixed support to guide them in the process of recomposition and subsequent recalcification.. Treatment of osseous infection using the Ilizarov method. Classifiable as acute and chronic osteomyelitis, depending on whether the symptoms persist for more than six weeks, or where there is the presence of arthroliths, osteomyelitis is an infection of the osteoarticular apparatus caused by bacteria - the most ...

I was granted membership of the KCH Limb Reconstruction Club in 2009. I arrived at Kings as a military patient, having been transferred from Selly Oak Hospital, Birmingham. I had been injured whilst serving on operations the year before. Unfortunately, the vehicle that my team and I were in hit an improvised explosive device (IED) which destroyed the vehicle and left three of us badly injured. Having received emergency medical treatment on the ground and been extracted back to one of the main NATO bases, we were all flown to the UK for further treatment. It was the end of our tour.. Its an odd thing, coming round a number of days later, not realising where you are or whats happened to you, with nurses asking you if you are aware of your own current whereabouts. Those seriously injured were moved to Selly Oak and, although a NHS hospital, certain wards housed injured military personnel. During that period, the number of soldiers treated at the hospital was significant.. Those who have been ...

Fibrous dysplasia (FD) is a rare bone disorder in which normal medullary bone is replaced by fibro-osseous tissue. It typically presents in childhood with pain, skeletal deformities, gait abnormalities and occasionally, fatigue fractures. The management of FD remains a challenge. Surgical procedures have been developed to provide symptom relief, correct skeletal deformity and offer mechanical support in cases at risk of fracture. However, there is a paucity of data on the management of acute femoral neck fractures in the adult population with FD. We report the case of a 23-year-old man with a shepherd\s crook deformity secondary to FD, who sustained an intra-capsular femoral neck fracture whilst playing football. The patient initially underwent closed reduction and internal fixation with cannulated screws. However, during the procedure, a guide wire broke whilst inside the femoral head. The patient was referred to the senior author, who undertook a second operation to remove the metalwork and ...
title:Comparison of Early Management of Compound Grade 2 and 3A (Gustilo-Anderson) Fractures of Tibia Shaft using Primary Intramedullary Nailing and Placement of Antibiotic Bone-Cement Beads, with that of External Fixator Application. Author:Santosh S Borkar, Darpan Maheshgauri, Viraj Bhoir. Keywords:Compound Fractures, Tibial shaft, Intramedullary Nailing, Antibiotic Bone-cement Beads. Type:Original Article. Abstract:Introduction: Compound fractures of tibia have been conventionally managed by debridement and external fixation, followed by intra medullary nailing or Ilizarov technique after few months. Definitive surgery is delayed in the early stages, as it may cause infections and additional soft tissue damage. But external fixators have their own set of complications. Hence a technique of antibiotic coated intramedullary nail (locked or unlocked) has been advocated. Methodology: Total 30 patients with grade 2 & 3 A (Gustilo-Anderson) compound tibia fractures were selected for the study. ...

Discussion. Osteomyelitis causes bone necrosis, soft tissue compromise, functional impairment, systemic illness and considerable morbidity.5 Chronic osteomyelitis may begin as acute haematogenous or contiguous disease, the latter typically being introduced by open fracture or surgery. In 1983 Cierny and Mader classified chronic osteomyelitis based on four anatomical types and three physiological groups of patients (A, B & C).14. Chronic osteomyelitis, as is the case with muscu-loskeletal tumours, can only be eradicated through complete resection of all necrotic tissue. Chemotherapy, in the form of antibiotics, only plays an adjuvant role.15. A curative approach resulted in a 12 cm subtrochanteric bone defect in our case. Bone defects greater than 4-6 cm are clinically challenging to treat and require specialised reconstructive techniques.1,2,16 Bone transport according to the Ilizarov method and bone graft into an induced membrane as described by Masquelet are two of the common techniques ...

Discussion. Circular external fixators are increasingly being used for orthopaedic trauma and post-traumatic reconstruction.18-20 These fixators exhibit a unique ability to eliminate bending and translational shear while maintaining a degree of axial micromotion.19,21-25 This three-dimensional stability translates into a biomechanical environment that is conducive to bone healing and regenerate formation and is often exploited for limb salvage and reconstruction.18, 24, 26-28. The hexapod fixator has been a recent modification of the traditional Ilizarov-type fine wire circular external fixator.29-30 It consists of two rings connected with six oblique struts in an octahedral configuration. Complex mathematical algorithms calculate strut length adjustments in order to manipulate the orientations of the two rings to each other.31-32 By attaching each of these rings to a bone segment, their position and orientation can be altered, thereby facilitating the reduction of complex multiplanar ...

The Ilizarov fixator is indicated for trauma induced conditions involving the upper and lower limbs and in all conditions in which mechanical trans-skeletal traction is necessary, such as in comminuted and multi-fragmented fractures, even with loss of bone substance. By avoiding long hospital stays and joint complications resulting from immobilisation, this technique allows a patient to resume his or her usual activities within a few days, with minimum impediment due in the main to the size of the fixator.. ...

Generally, surgeon-driven musculoskeletal evaluation systems are used for evaluating outcomes of patients who are treated with high tibial osteotomy. In this study, we investigated the effects of high tibial osteotomy using circular external fixator on quality of life. Twenty-one high tibial osteotomy of 19 patients were evaluated. Quality of life assessment was made using Short Form -36 at preoperative, before fixator removal and 6 months after fixator removal. After applying fixator, the physical function and physical role scores of Short Form-36 decreased and emotional role score did not improve. In other categories, significant improvements were observed when fixator in place. At the sixth month after fixator removal, significant improvements were dedected in all categories of Short Form-36. Although there was a decrease in physical functions after fixator application in patients who were treated with high tibial osteotomy using circular external fixator, significant improvement occurred in quality

Increasing numbers of patients are being diagnosed with diabetes, and they are living longer because of improvements in treatment [1-3]. Ankle fractures in patients with diabetes mellitus have long been recognized as a challenge to orthopedic surgeons. Nonunion and lengthy wound healing in high-risk patients with diabetes, particularly in those with peripheral arterial disease and renal failure, as in the present case, are often fraught with complications [1-3]. Whether diabetic ankle fractures are best treated noninvasively or surgically is controversial [2]. Some previous studies have shown that nondisplaced fractures in high-risk patients can be managed nonsurgically in a cast [5, 6]. Treatment entails casting with non-weight-bearing restriction until fracture healing is demonstrated.. McCormack et al. described 26 ankle fractures in patients with diabetes; 19 were treated surgically, and seven were immobilized in casts. The surgical group included one wound complication (5%), four infections ...

Limb-length discrepancy following total hip arthroplasty (THA) is often cited as a reason for patient dissatisfaction and for hip instability. Various intraoperative techniques have been described to help restore normal limb length after THA. The purpose of this study was to assess whether a computer-navigated surgical technique would help restore limb-length equality following THA. A retrospective study of 150 consecutive patients compared a free-hand (non-navigated) THA technique vs a computer-navigated THA technique. Each group contained 75 patients. The primary outcome measurement was limb-length discrepancy, which was evaluated using a digital anteroposterior pelvic radiograph. Secondary outcome measurements included a Harris Hip Score questionnaire and a single question evaluating the subjective feeling of the operative limb (longer, shorter, or equal). At a minimum 1-year follow-up, results showed that computer-navigated THA helped restore limb-length equality. An average leg-length ...
Given the common presentation of limb length discrepancy, this author reviews the historical methods for diagnosis as well as an objective method for dynamic assessment. He also addresses the clinical significance of limb asymmetry and how computer-assisted gait analysis plays a key role in assessing treatment.
high strength aluminum rings connected with threaded rods, hinges or distraction motors. A CESF is secured to bone using 1-1.6mm highly tensioned wires, or positive threaded fixation pins. Small bone fragments, less than 1cm in length can be adequately stabilized with a CESF.. A CESF can be used to gradually move bone segments, and grow bone, a process known as distraction osteogenesis. The system has allowed surgeons to successfully manage very complicated juvenile angular bone deformities and shortened bones. CESF is also used to manage fractures with extremely small (,1cm) bone fragments. Other potential uses include ligament reconstruction, carpal and tarsal fusions.. The Dallas Veterinary Surgical Center has become one of the leading referral centers in North America using the CESF and the Ilizarov method to manage difficult fractures and bone deformities. Postoperative care: Distraction is commenced at day 10. 0,25 mm of lengthening is perfomed 4 times daily (every 6 hours). Hip and knee joint motion exercises begin at 1-2. day postoperatively. For many years, the treatment of lower extremity shortness has been very hard for the patient and the surgeon. The techniques developed after the introduction of Ilizarovs distraction osteogenesis concept have pioneered a new era, and have been considered to be procedures in which previous complications are encountered much less frequently.. Today the most important properties that an extemity lengthening method shall possess are application time, ability to correct alignment (anatomic and mechanical axis of the extremity), number of total operations required, ability to protect the regenerated bone from refracture, minimum rate of pin tract site infection and possible complications, preservation of joint motion, healing time, comfortability in daily life and application cost..
