Inpatient psychiatric facilities across the country face $1.7 billion a year in compliance costs stemming from outdated regulations and inconsistent surveys from private and state agencies, according to a new industry report released Tuesday. The biggest driver of regulatory costs for facilities is meeting requirements to lower ligature-risk points, which are areas where a patient can try to strangle themselves. Ligature-risk point regulatory compliance costs inpatient facilities nationwide an average $880 million a year, according to the National Association for Behavioral Health, which represents the facilities. ...
To identify the pattern and determinants of psychiatric illness and the predictors of long stay among long-stay patients at the Mental Health Hospital, Taif, Saudi Arabia, we examined a total of 430 records of patients who had been admitted during the period January 1999-January 2009 and had stayed for , 9 months. Black people are three times more likely to be admitted to mental health hospitals than the rest of the population, a health watchdog has said.. The Healthcare Commission carried out a census of 34,000 mental health hospital patients in England and Wales.. It said that finding the reason for its results should be a matter of urgency.. Mental health campaigners said the findings were shocking but the government said they would be used to measure progress in ending inequality.. It has long been known that black people were more likely to end up in mental health hospitals, but the census gives the clearest indication yet of the true figures.. The census was announced after the government pledged to eradicate discrimination in NHS mental health care following a report into the death of patient David Bennett which said there was institutional racism in the NHS.. Mr Bennett died after being restrained at a Norfolk clinic in 1998.. The Healthcare Commission, in conjunction with the Mental Health ...

A proposed rule issued Friday by the Centers for Medicare & Medicaid Services (CMS) that would update Medicare payment policies and rates for inpatient rehabilitation facilities (IRFs) in Fiscal Year (FY) 2012 seeks to establish a new quality reporting system authorized by the Patient Protection and Affordable Care Act (PPACA). The rule proposes to increase payment rates under the IRF Prospective Payment System (PPS) by a projected 1.5%-an estimated $120 million nationwide. The projected update reflects a rebased and revised market basket specific to IRFs, inpatient psychiatric facilities, and long-term care hospitals (the RPL market basket)-currently estimated at 2.8% for FY 2012, less a 1.3 percentage point reduction mandated by PPACA. The proposed quality reporting system is aligned with the goals of the Partnership for Patients, a new public-private partnership that aims to help improve the quality, safety, and affordability of health care. Initially, IRFs would submit data on 2 quality ...
Young people with learning disabilities are being driven to self-harm after being prevented from seeing their families during the coronavirus lockdown in breach of their human rights, a new report finds. The Joint Committee on Human Rights warned that the situation for children and young people in mental health hospitals had reached the point of severe crisis during the pandemic due to unlawful blanket bans on visits, the suspension of routine inspections and the increased use of restraint and solitary confinement. The report concluded that while young inpatients human rights were already being breached before the pandemic, the coronavirus lockdown has put them at greater risk - and called on the NHS to instruct mental health hospitals to resume visits. It highlighted cases in which young people had been driven to self-harm, including Eddie, a young man with a learning disability whose mother, Adele Green, had not been able to visit him since 14 March. When the lockdown came, it was quite ...

PUBLIC CONFIDENCE IN THE INTEGRITY OF THE FEDERAL GOVERNMENT is alarmingly low. While numerous factors contribute to this phenomenon, one of the most potent is the widespread belief that government has been taken over by powerful special interests. Such a belief is not unfounded. Special interests-which these days mainly mean large corporations and their trade associations- spend huge sums on campaign contributions and lobbying. Yet money is not the only way business exercises its influence; it also relies on the movement of certain people into and out of key policymaking posts in the executive and legislative branches. This movement, known as the revolving door, increases the likelihood that those making policies are sympathetic to the needs of business-either because they come from that world or they plan to move to the private sector after finishing a stint with government. The revolving door is not new, but it seems to have become much more common. Recent administrations have appointed unprecedented
Rogers Behavioral Health System will open a hospital in Brown Deer next week that eventually could have 56 beds for patients who require acute care.. We know the demand is there based on the number of calls we get to our intake staff, said Jim Kubicek, vice president of operations for the new hospital at 4600 W. Schroeder Drive just off Brown Deer Road.. Rogers Behavioral Health System also operates a hospital with 80 beds in West Allis.. The new hospital initially will employ about 120 people and is projected to employ 200 people when at capacity.. In 2013, Rogers bought the building, where it had provided outpatient care, for $2.15 million. It has since spent an estimated $13 million to convert it into a psychiatric hospital.. The health system bought a nearby building at 4555 W. Schroeder Drive for its outpatient services last year.. The hospital will have a mental health unit and a drug and alcohol unit. It also will have a child and adolescent unit.. It will provide intensive, short-term ...
Smokers with mental health disorders are as motivated to quit smoking as the general population,1 and quitting has been shown to improve mental as well as physical health. Quitting also enables smokers to take lower doses of neuroleptic drugs and hence experience fewer side effects.1 Despite all this, smokers in psychiatric hospitals are less, rather than more, likely than other smokers to be offered help to quit.1 In the past two decades, during which the prevalence of smoking throughout society has fallen by a quarter, smoking rates among people with serious mental illness have barely changed.1. Prohibiting smoking indoors is clearly justified to protect non-smoking patients and staff, but is it a step too far to prevent inpatients from smoking outdoors as well, and in so doing effectively force them to quit smoking? The legal position was tested in the English courts when Rampton secure hospital went … ...
This retrospective study examined health care utilization, clinical profiles, and outcomes of patients admitted to the 23-hour observation unit within the emergency room at the Institute of Mental Health, the only tertiary psychiatric hospital in Singapore. The authors hypothesized that a specific clinical profile-greater clinical severity and lower psychosocial functioning-predicted subsequent inpatient hospitalization.
BACKGROUND: Patient satisfaction surveys are commonly conducted to evaluate health care quality. However, little is known about the impact of the time point of survey administration on the level of satisfaction, questionnaire acceptability, and costs, especially for inpatient psychiatric care. AIMS: To assess whether inpatient satisfaction, questionnaire acceptability, and total costs of study differ according to the time point of questionnaire administration for inpatient psychiatric care. METHOD AND SAMPLE: Inpatients completed the ©Saphora-Psy, a French validated tool measuring satisfaction with care with 35 items assessing 7 dimensions of care, 2-3 days before leaving the ward (first phase). Four to eight weeks after discharge, patients received the same instrument at home (second phase). Time needed to fill the questionnaire and items assessing its acceptability were requested. RESULTS: Only fifty of 104 inpatients in the acute psychiatric hospital, aged |18, who completed the fir

Psychiatric patients present unique challenges for emergency care, with one of the foremost problems involving the optimal way to assess whether these patients are suitable for admission to psychiatric facilities. Prior literature regarding this clearance process consists largely of expert opinion and retrospective analysis. Our study advances understanding in this area by providing prospective data to this body of literature, and specifically focuses on whether mandatory test panels change disposition after EPs have cleared a psychiatric patient for admission by history and physical exam.. This multi-center study overwhelmingly demonstrates that routine or mandatory studies do not change the disposition of psychiatric patients after EPs have cleared them for admission. Of the 191 patients evaluated in this study, only one (0.52%) patients disposition was changed by additional tests.. With only 1 exception, none of the studies performed on medically cleared patients, including complete blood ...

Foster Home Crunch Strands Children in Psychiatric Facilities By Terri Langford April 8, 2016 Abused children in Texas are being left in psychiatric facilities longer than they were six years ago as the states child protective services system grapples with federal court scrutiny and diminishing options, according to data obtained by The Texas Tribune. Last year, 17,151 Texas children were removed from abusive homes. While the agency could not say exactly how many were placed in private and state psychiatric hospitals, data from 2009 to 2015 shows roughly 4,000 psychiatric admissions for foster care children each year. But the number of total days foster care children together spend in psychiatric facilities past their initial 8 to 10 days of treatment covered under Medicaid has risen. In June 2009, children taken together spent a total of 10 extra days in the facilities. By August 2015 that number had grown to 768 days. I can say the capacity issue is part of the problem, said Julie Moody, a ...

In response, the DOH cited a lack of oversight and enforcement tools over the hospitals. As a result, Governor Inslee requested that the legislature address this in the 2020 legislative session. Objective: To examine the rise in the rate of involuntary admissions for mental illness in England that has occurred as community alternatives to hospital admission have been introduced. Design: Ecological analysis. Setting: England, 1988-2008. Data source: Publicly available data on provision of beds for people with mental illness in the National Health Service from Hospital Activity Statistics and involuntary admission rates from the NHS Information Centre. Main outcome measures: Association between annual changes in provision of mental illness beds in the NHS and involuntary admission rates, using cross correlation. Partial correlation coefficients were calculated and regression analysis carried out for the time lag (interval) over which the largest association between these variables was identified. Results: The rate of involuntary admissions per annum in the NHS increased by more than 60%, whereas the provision of mental illness beds decreased by more than 60% over the same period; these changes

This hospital rises above state-run facilities as it was the first psychiatric hospital in the United States to be federally funded. United States Congress funded the construction for the hospital in 1852 and construction began a year later. The main building would follow the Kirkbride plan, but the campus itself would end up being enormous. The hospital opened its doors to patients in January of 1855. The US Navy used one building on campus as a general hospital during the Civil War. The US Army used a wing of the Kirkbride as their general hospital during the same time period. In the late 19th century, animals that needed temporary housing before the National Zoo was constructed were held at the hospital. In the 1940s, the hospital was housing an average of 8,000 patients and employed around 4,000 people. The hospital saw many strides and firsts in the treatment of mental disabilities. In 1963, Dr. Luther D Robinson founded a program for treating mentally ill patients who were deaf and became an

Between 1955 and 1994, state-run psychiatric hospitals across the country discharged most of their patients in a social experiment called deinstitutionalization. It coincided with three factors: greater availability of effective psychiatric medications, the creation of Medicaid and Medicare, and a societal movement toward treating people in their communities instead of institutions.. Despite positive intentions, deinstitutionalization helped fuel a new crisis: mass incarceration. Jhilam Biswas, MD, a forensic psychiatrist who directs the Brighams Psychiatry, Law, and Society Program, says nearly 50% of the nations prison population suffers from mental illnesses.2 In fact, jails in New York, Los Angeles, and Chicago are now the three largest psychiatric institutions in the country.. Our goal is to address the serious need for high-quality psychiatric treatment in the correctional setting and educate social systems outside of medicine and healthcare on psychiatric issues, she says. We ...
In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market and cases of patient exploitation in return for cigarettes. This article surveys the literature dealing with smoking among psychiatric patients, the role of smoking in patients and the moral dilemmas of taking steps to prevent smoking in psychiatric hospitals. It addresses the need for public discussion on professional caregivers dilemmas between their commitment to uphold the law and their duty to act as advocates for their patients rights and welfare. ...
Identifying people at high risk of suicide and reducing the numbers at risk are high priorities for mental health professionals in many countries. This case control study by Appleby et al permits examination of the relation between mental health service provision and suicide.. The sample of people who committed suicide and their matched controls were drawn from discharged psychiatric inpatients in Greater Manchester, UK. In a 30 month period, 708 overall confirmed or probable deaths occurred from suicide; of these, 149 suicides occurred in people who had had psychiatric hospital admissions within the previous 5 years. The authors used a model that linked risk of suicide to specific clinical predictive factors. The predictable risk of suicide, defined as risk as it would have been perceived by a clinician at the last contact before suicide, was independently assessed by review of clinical notes by 2 psychiatrists who were unaware of which patient was a case and which was a control. The authors ...

The Divisions clinical service platform is based at two regional Psychiatric Hospitals, Alexandra Hospital in Maitland and Lentegeur Psychiatric Hospital in Mitchells Plain. The Groote Schuur UCT Department accommodates the Vera Grover Chair, non-patient related research activities and access to general Departmental administrative support. Alexandra Hospital provides for people with intellectual disability and the Lentegeur Intellectual Disability Service functions within the general Lentegeur Psychiatric Hospital. OBJECTIVE: The present study was designed to (1) estimate the frequency of benzodiazepine use in psychiatric practice, (2) investigate factors associated with use, (3) establish whether a relationship exists between benzodiazepine dose and length of use, and (4) investigate factors associated with time to discontinuation. METHODS: This study was conducted in South Verona, Italy. All individuals who were exposed to benzodiazepines during 2005 were extracted from the local Psychiatric Case Register, and the longitudinal history of benzodiazepine exposure was retrospectively described. RESULTS: In 2005, a total of 1,771 individuals were in contact with at least one of the psychiatric facilities of the South Verona catchment area. Of these, 535 were benzodiazepine users, yielding a frequency of use of 30.2% [95% confidence intervals (CI) 28.0, 32.4]. In multivariate logistic regression analysis, lower level of education, diagnosis of affective illness, longer length of illness and higher service use were

The opening article of the Journals special section (Osher & Drake,1996) which traces the history of separating mental health and addictive services, is an important contribution to the literature on dual disorders. Since we are now in the early stages of the evolution of dual diagnosis services, however, I believe it is important to provide a more complete and detailed account of the previous initiatives in this field. In response to the article by Drake, Mueser, Clark and Wallach (1996) which states Ten years ago the only treatment options available for people with co-occurring substance abuse and severe mental illness were parallel treatments in separate programs (p.49), the record requires correction. Dual diagnosis treatment interventions and integrated programs that truly adapted to the needs of severely mentally ill chemical abusers (MICA) began in 1984 (Gigliotti,1986, Sciacca 1987a,1987b) in a New York State outpatient psychiatric facility. In 1985 these integrated treatment ...

A blistering review of Virginia's mental facilities, which was belatedly released last week by state officials, should tug at the conscience of every Virginia taxpayer. In a situation that has gone on for centuries, but which seems to have been exacerbated this decade, Virginia has relegated those citizens least able to fend for themselves to perpetual substandard care. From day one - Virginia is after all the home of the oldest state mental hospital in the country - mental health programs have been underfunded.

Having a primary care service for an acute psychiatric unit appears to help meet the numerous acute and long-standing physical health needs of patients. Previous work by Fisher & Roberts (1998) has highlighted the important role for a primary care doctor among long-stay patients. Our study suggests that patients in acute psychiatric wards can also benefit from primary care expertise. It also suggests that it is feasible to establish a primary care service, and that such a service usefully supplements the physical health care already provided and is welcomed by patients. Potential problems considered when the service was established did not really materialise. Some duplication of work occurred initially when patients were referred to specialist on-call services as well as to the primary care service, but feedback to staff minimised such events. An initial concern of staff was that the service would introduce an extra layer of complexity in the care of patients. In practice, this did not appear to ...
Whenever I want a read that takes me out away from super-sweet, I dive into a novel by Cynthia A. Rodriguez. Her latest novel, Folie A Deux did not disappoint! Its a mind f*ck of a romance; Girl, Interrupted meets Romeo and Juliet. Rose and Abel are wards of a psychiatric facility who find a connection when everyone says they should stay away from one another. I ran away with these characters and was with them as they fell hard, made bad decisions, and *almost* threw my kindle against the wall at that ending! Here is your SPOTLIGHT with Cynthia A. Rodriguez on Folie A Deux!. Jeannine Colette: This is a maddening romance that can only come from a maddening place. Where did you get the inspiration for this love story?. Cynthia A. Rodriguez: I actually started this story years ago, when I was going through separating from my husband. I hadnt realized just how angry I was over the disintegration of my marriage, but I was. And when I started writing it, it was becoming this sort of monster of a ...

This paper summarises longitudinal data based on repeat assessments of the behavioural functioning of 90 long-stay psychiatric patients. Data were gathered over a period of 4-5 years that spanned a hospital closure and the resettlement of patients in various alternative settings. Time-series analyses of individual data sets showed that resettlement led to no significant behavioural change in the majority of patients. However, some significant behavioural improvements were evidenced for approximately 20% of the group, while behavioural deterioration occurred in approximately 12%. Supplementary findings showed that following resettlement the quality of care provided significantly improved in many respects. However, access to social, recreational or vocational activities was reduced. In addition, the level of physical health symptomatology was shown to increase for this group of patients ...
GENEVA (2 December 2016) - Four United Nations human rights experts* today called on the South African authorities to establish a clear and sustainable deinstitutionalization policy and plan of action to avoid another tragedy, after a flawed process led to the death of at least 37 persons with psychosocial and intellectual disabilities. Unconfirmed reports indicate the casualties could be as high as 60.. Following a badly planned relocation process of 2,300 persons from Life Healthcare Esidimeni hospitals, the Gauteng Department of Health has transferred more than half of them under the care of NGOs with inadequate capacity and resources to assist people requiring high-level, specialized and intensive non-stop care. South Africa must set up a policy framework to guide its deinstitutionalization process, inclusive of a plan of action with timelines and benchmarks, the redistribution of public funds from institutions to community services, and the development of adequate housing and community ...
The major findings of this study were that patients for whom court-ordered medication was sought (MOO group), and, with few exceptions, obtained, were more likely to be transferred to state hospitals for long-term inpatient care and less likely to link to the next level of care after discharge, compared with similar patients who accepted treatment. These findings were essentially the same when MOO group patients were compared with patients who initially refused but ultimately accepted medication.. The relatively high rate of state hospital transfers in the MOO group strongly suggests that persistent medication refusal in a hospital setting may be a risk factor for treatment refractory status. Patients are transferred to a state hospital when discharge to the community is considered unsafe and they have not responded to many months of aggressive inpatient treatment, typically four to six months in our setting. It is interesting to speculate that there may have been an interaction between ...
TY - JOUR. T1 - Length of psychiatric hospitalization and prediction of antipsychotic response. AU - Zemian, Frank P.. AU - Thienhaus, Ole J.. AU - Garver, David L.. PY - 1990. Y1 - 1990. N2 - 1. 1. Clinical variables determining length of psychiatric hospitalization for psychotic inpatients were explored. Forty psychotic inpatients received a 14 day fixed dose neuroleptic trial. 2. 2. Neuroleptic responders ( 25 40) were discharged 15 ± 2 days after initiation of phannacotherapy. For neuroleptic non-responders ( 15 40) antipsychotic medication was then altered as clinically indicated. Patients requiring one change in medication (N = 8) were discharged after 27 ± 5 days; those requiring two medication adjustments (N = 4) were discharged after 33 ± 3 days and those requiring three alterations in phannacotherapy (N = 3) were discharged after 42 ± 12 days. 3. 3. Statistical analysis of clinical and diagnostic variables indicated that 84% of the variation in length of hospitalization was ...
Introduction: Metabolic Syndrome (MetS) is one of the most common syndromes that include a set of serious cardiovascular risk factors. MetS is highly prevalent in the high risk population and contributes to reduced lifespan. The primary goal of the study is to assess the prevalence of MetS and its relation to the treatment modalities and the psychiatric diagnosis. Methods: A cross-sectional study for the hospitalized patients in the Mental Health hospital in Taif city, Saudi Arabia that was conducted between Dec 2013 - Nov 2015. We included adult patients who were admitted to the inpatient service. We diagnosed MetS according to the new International Diabetes Federation (IDF) definition. Results: A total of 313 patients with mean age 48.4+10.8 years old, the majority were male and single, 68.3% have high school degree or less, mean BMI in the overweight range, and mean waist circumference 96.5+14.0 cm. Compared to these without MetS, those with MetS more likely to have BMI and waist ...
