Patients who undergo elective orthopedic surgeries at high-volume, regional hospitals have better surgical outcomes and experience fewer complications than those who undergo those surgeries at local hospitals., High-Volume Hospitals Improve Orthopedic Outcomes
Results-A total of 118 institutions with 8533 patients were included. Mortality rate (14.9% versus 18.6%; P=0.049) and mortality index (1.1 versus 1.6; P=0.048) were significantly lower among directly admitted relative to transferred patients. For all patients, there were significant differences in institutional mortality rate (low: 19.7%, medium: 14.9%, high: 9.8%; P=0.003) and mortality index (low: 1.5, medium: 1.1, high: 0.8; P=0.004) between low-, medium-, and high-volume hospitals. For transferred patients to high-volume centers, both mortality rate (high: 10.0% versus low: 20.4%; P=0.005) and mortality index (high: 0.8 versus low: 1.5; P=0.034) were significantly lower than that observed for directly admitted patients to low-volume hospitals. ...
This study explored the association of mortality with patient factors, physician factors and healthcare organisation factors after admission to hospital with CAP. Mortality rates of 11% occurred in hospital and 26% at 1 yr 3. As expected, severity of illness and comorbidity had the strongest association with mortality 22. This paper is the first study associating high-volume physician care with decreased in-hospital mortality for CAP. The presence of an internal medicine specialist/subspecialist was not consistently associated with decreased hospital mortality except when a pre-admission consultation occurred. Healthcare organisation factors were weakly associated with in-hospital mortality for the acute episode and mortality in those that were discharged after the initial index episode of pneumonia. One-year mortality was decreased in high-volume hospitals, where the original admission occurred.. The relationships between specialists/subspecialists, high-volume practice and more complex ...

Perhaps the single most important thing you can do is seek care at a cancer center where providers have experience treating ovarian cancer. These centers, often referred to as "high-volume" centers because they treat many patients with the disease, are staffed with physicians who have extensive experience in treating ovarian cancer and provide the expertise you need.. High-volume hospitals and high volume surgeons are more likely to practice and adhere to National Comprehensive Cancer Network (NCCN) Ovarian Cancer Treatment Guidelines; improved survival rates have been associated with patients treated according to NCCN guidelines.. Seeking care with a high-volume provider will also help ensure that you receive an accurate diagnosis, which is incredibly important. In some cases, advanced cancers of the gastrointestinal tract (gastric cancer, colorectal cancer, hepato-biliary cancer, pancreatic cancer) can mimic ovarian cancer, so receiving an accurate diagnosis that will lead to the most ...

Commenting on the study finding that most women with ovarian cancer are not being treated with the recommended standard of care, a New York Times editorial noted, "One of the surest ways to improve performance would be to analyze and make public how well individual doctors and hospitals do in treating various diseases."1. Robert E. Bristow, MD, MBA, lead author of the study and Director, Division of Gynecologic Oncology, University of California, Irvine, considered that a fair approach. "I think we are going to see a time when we have to not only publish our data, but publish our cost of care so that consumers will be able to search for the best value. It may be that to get care at a high-volume center by a high-volume surgeon ends up costing more upfront, but if your payoff is an additional 4 or 5 or 6 years of survival time, with a good quality of life, then that may very well be interpreted as a good health-care investment," he said. "So I think we will see a time when these risk-adjusted ...

Joynt and colleagues (1) analyzed Medicare fee-for-service patients with a primary discharge diagnosis of congestive heart failure in the United States and found that in the low-volume group, being admitted to a hospital with a higher case volume was associated with lower mortality, lower readmission, and higher costs. In the report, patients with congestive heart failure receiving intensive cardiac care accounted for 16% in the low-volume hospitals compared with 37% and 69% in the medium- and high-volume hospitals, respectively. As we know, patients with congestive heart failure who have many comorbid conditions and severe symptoms will need intensive cardiac care. Therefore, a higher 30-day mortality rate and readmission rate in the low-volume group seems unreasonable. We noticed that this occurred in only 61% of low-volume hospitals in the urban locations ...
In this population based cohort, we found small but significant differences in postoperative complications (mortality, complications, and readmission) between patients treated by male and female surgeons, using an α criterion of 0.05. Patients treated by female surgeons had lower rates of 30 day mortality. We matched patients for age, sex, and general comorbidity that had the same surgical procedure by a female or male surgeon of the same age with comparable annual surgical volume in the same hospital. Male surgeons in this cohort had higher average procedural volumes, which mitigates some of the effect of surgeon sex. Outcomes adjusted for case-mix (without adjustment for surgical volume) were comparable for patients treated by female and male surgeons. Retrospective analyses showed that the effect of surgeon sex was restricted to patients who had elective operations; outcomes did not differ among patients who had emergent operations. This finding must be interpreted within the limitations of ...

A study published in February reported that patients who took Pradaxa (dabigatran) following radiofrequency ablation to treat atrial fibrillation experienced more bleeding and blood clots compared to people who were treated with warfarin post-procedure. The authors of the study, published in the Journal of the American College of Cardiology, noted that the lack of antidote to reverse life-threatening Pradaxa bleeding side effects "makes the risk of excessive bleeding complications all the more important.". The study looked at 290 patients who had undergone atrial fibrillation ablation procedures performed at eight high-volume hospitals from January 2010 to July 2011. Roughly half were treated with 150 mg of Pradaxa twice daily for at least 30 days before AF ablation. Pradaxa was withheld on the day of surgery and restarted 3 hours after ablation. The remaining patients were assigned uninterrupted periprocedural warfarin.. Overall, 2.1% of patients assigned Pradaxa experienced thromboembolic ...
Frailty is an aging-associated syndrome resulting from diminished capacity to respond to stressors and is a significant risk factor for disability and mortality. Although frailty is usually studied in old age, it is present in mid-life. Given the increases in mortality statistics among middle-aged Americans, understanding molecular drivers of frailty in a younger, diverse cohort may facilitate identifying pathways for early intervention. We analyzed frailty-associated, genome-wide transcriptional changes in middle-aged blacks and whites. Next generation RNA sequencing was completed using total RNA from peripheral blood mononuclear cells (n = 16). We analyzed differential gene expression patterns and completed a parametric analysis of gene set enrichment (PAGE). Differential gene expression was validated using RT-qPCR (n = 52). We identified 5,082 genes differentially expressed with frailty. Frailty altered gene expression patterns and biological pathways differently in blacks and whites, including
Factors associated with variation in outcome after surgery have been a topic of major interest for the past 20 years. By far the most explored is the relation between case volume and outcome. Hospital and surgeon volumes have been examined in most surgical specialties and related to many short and long term outcomes.123 Certainly for more complex procedures, higher institutional volumes correlate with better outcomes. The evidence for individual surgeons is less clear, often reflecting the outcome. Low volume cardiac surgeons, for example, have only slightly higher rates of complications but far higher rates of death in hospital than high volume surgeons.4. Virtually all studies that examine factors related to surgical outcome are observational. Causal relations are difficult to prove. As a result, programmes … ...

WASHINGTON, DC-A high-volume center accessing the coronary arteries via the radial artery safely discharged PCI patients home the same-day and significantly lowered the costs of the procedure when compared with an overnight stay, new research shows.. Presenting data at CRT 2017 in Washington, DC, William Rollefson, MD (Arkansas Heart Hospital, Little Rock) reported equivalent 30-day clinical outcomes among transradial PCI patients sent home the day of the procedure and patients who stayed overnight.. "When we looked at the cumulative costs in US dollars of transradial same-day discharge versus an overnight stay, the total cost in the same-day arm was $3,346," said Rollefson. "On average, the cost of the overnight stay was $4,682, which was an initial cost savings of $1,336, and this was slightly increased at 30-day follow-up.". Previous studies have shown patients can be safely discharged the same day after an uncomplicated PCI. For example, in a meta-analysis published in 2013, Olivier ...

One of the most important changes Dr. DeCook and his colleagues made at their institution was to do away with NPO before surgery. A 1993 study showed that preoperative drinking does not affect gastric contents [2]. Residual gastric volume and pH remained the same, and there were no problems with aspiration or regurgitation. In contrast, dehydration enhances pain-evoked activation in the human brain compared with rehydration. [3]. Dr. DeCook said that doing away with NPO has made a big difference in the ability to get patients discharged the same day as the surgery. In fact, he encourages his patients to drink clear liquids until they arrive at the hospital. Then, his patients receive 1 liter of fluids in the hospital preoperatively, 1 liter of fluids in the PACU, and 300 mL of fluids per hour for 4 hours postoperatively or until they leave the hospital.. One of the reasons patients are not able to be discharged the same day as surgery are the unwanted side effects of anesthesia, [4] including ...
