Fianna Fáil Senator Dr. Keith Swanick has called on the Health Minister to seriously consider using the existing district hospital network to alleviate the pressure being experienced in our acute hospitals. Senator Swanick was speaking at the Institute of Community Health Nurses conference earlier today.. The Mayo based GP explained, "The district hospital network is a useful asset in our health service that is not being effectively utilised. We need to maximise the services provided in these hospitals so that patients have an alternative to already overcrowded Emergency Departments and acute hospitals. District hospitals can be used in the treatment of minor medical conditions thereby alleviating the pressure on bigger units.. "Almost every county has a district hospital making it an almost nationwide resource. These hospitals could be used for step down care, could accommodate certain outpatient clinics or be used as an intermediate care setting for older people who have been discharged from ...
Introduction. Recent advances in therapy make early identification of patients with Pulmonary Hypertension (PH) important. The 2008 consensus statement reported an average UK treatment rate of 24.9 per million.(1). Aim. Our institution is a District General Hospital serving a population of 330,000. Our aim was to compare our treatment rate to the national average since the appointment of a physician with an interest in PH.. Method. Retrospective casenote review of patients attending our monthly PH clinic since 2006.. Results. We identified 102 Outpatient and 35 Inpatient referrals. 28 went on to receive specialist treatment for PH; mean age 66 (range 36-85); 24 female 4 male. Diagnoses: Chronic thromboembolic 10, Idiopathic 8, Collagen vascular disease 5, Congenital left to right shunt 2, COPD 2 and Portal hypertension 1. Haemodynamics (mean): Cardiac catheter: mean Pulmonary Artery Pressure (PAP) 48mmHg (n=22, range 22-78), cardiac output 4.8 (n=21, range 2.5-7.7), cardiac index 2.83 (n=22, ...
Results 279 patients were referred to the HPBCS between 1 June 08 and 1 June 09: 175 for suspected pancreatic cancer (PC), 104 for hepatobiliary cancer (HC). This compares with 51 for suspected PC over a 6-month period pre-HPBCS and 26 for suspected HC a year pre-HPBCS, representing an annual increase of 72% and 300%, respectively. The outcomes of discussion at the RSC for suspected PC were: inoperable PC (59/175), operable PC (24/175), IPMN (45/175), benign lesions (22/175), pancreatitis (15/175) and pseudocysts (10/175), while for HC: hepatocellular carcinoma (34/104), gallbladder cancer (13/104), cholangiocarcinoma (9/104), liver metastases of unknown primary (2/104), benign liver lesions (23/104) and liver lesions of unknown significance (21/104). 23.7% of cases (45 IPMN, 21 liver lesions of unknown significance) were referred back for local surveillance contributing to an ever expanding pool of patients under the local HPB team. Nearly all cases were referred to the RSCs within 2 weeks of ...
Introduction: Severe malnutrition is a common cause of preventable morbidity and mortality among children aged 5 years in developing countries. The prevalence of bacteraemia, urinary tract infection, diarrhea and pneumonia among children with severe malnutrition is high. These, coupled with an atypical clinical presentation of sepsis, justify the routine use of empirical antibiotic treatment in the initial phase of inpatient management of SAM children1 2 .The choice of antibiotics should be guided by locally prevalent pathogens and their antibiotic susceptibility patterns. Objectives: The broad objective of this study was to determine the prevalence of bacteraemia in SAM children at Mbagathi District Hospital and its association with the history and physical examination characteristics of the enrolled children. The primary objectives of the study were: • To determine the blood bacterial isolates in SAM children in Mbagathi District Hospital. • To determine the sensitivities of the bacterial ...
33 and 23 patients were identified as undergoing high-risk surgery in 2013 and 2014, respectively.. NCEPOD data identified arterial lines; central lines and cardiac output monitoring were used in 27%, 14% and 5%, in high-risk patients, respectively. In this audit, these figures were 50%, 36.1% and 2.8%, in 2013, and 60.9%, 26.1% and 34.8%, in 2014, respectively.. In this audit, 69% and 65% of "high-risk" patients were referred to higher-level care in 2013 and 2014, respectively. Of these, 85% (2013) and 90% (2014) had a clearly documented plan from higher-level care, whether that be admission to HDU or outreach plan. The overall admission rates to higher level care were 47.2% and 65.2% in 2013 and 2014, respectively. This is comparable to the 22.1% seen in the NCEPOD data. ...
The average length of hospital stay is regarded as a key determinant of greater hospital costs. The objectives of this quantitative, retrospective, descriptive study were to determine the health status and medical conditions of patients with increased length of stay at a district hospital in Limpopo Province. A total of 609 patients' records with longer than average length of stay, from January to December 2007, were selected by stratified random sampling. The most prevalent diseases for staying longer were infectious and parasitic diseases, symptoms, signs etc, and injury, poisoning and consequences of causes yielding more than a third (36% and 12%, respectively, for each classification). Neoplasm-related admissions, in which cancer was identified as the most prevalent, stayed in hospital for an average of 14.6 days. The average length of stay for all disease classifications ranged from 7.1 days to 14.6 days. The length of stay at Elim Hospital is mostly influenced by the type of health conditions
Simon Francis is the kind of kid you want your children to befriend; his thoughtful consideration and unassuming nature are evidence of his kindness. In December the grade one student hosted his sixth birthday with his ten closest friends; rather than gifts he instructed his pals to bring a cash donation for the Neonatal Intensive Care Unit (NICU) at Nanaimo Regional General Hospital (NRGH). As a result, Simon was able to deliver a $130 donation to the Nanaimo & District Hospital Foundation to be given to NICU ...

I feel like youre not aware of the fact that the pellets required to 2-shot within optimal range with the JG only went down by one per shot, which is still 7/12 pellets. . . Given that the JG has under half the range such that it does minimum damage at a range where the old JG did full damage per pellet, the guns simply not as good anymore. Oh, and it only does 85 damage the first pellet. The JG hasnt been meta for ages, the PMG and OCA are better. I dont get how you think the JG takes zero skill to hit with and SMGs require some magical god level aim to beat the JG, as neither of these are true. If you do truly simply wait for the enemy to run into your crosshair youll get out TTKed virtually every time by a PMG or OCA, and I do mean. Oh, and the CSG is simply so bad now its not even part of the discussion TBH. AIMS: To determine the prevalence, clinical characteristics, and outcome of hypoglycaemia on admission in children at a rural Kenyan district hospital. METHODS: Observational study of 3742 children (including 280 neonates) in Kilifi District Hospital, Kenya. MAIN OUTCOME MEASURES: hypoglycaemia (blood glucose |2.2 mmol/l) and hyperglycaemia (blood glucose |10.0 mmol/l). RESULTS: Non-neonates: the prevalence of hypoglycaemia on admission was 7.3%. Severe illness, malnutrition, last meal |12 hours ago, and a positive malaria slide were independently associated with hypoglycaemia. Overall, mortality in hypoglycaemic children was 20.2% compared to 3.8% in normoglycaemic children (p | 0.001). The brunt of mortality in hypoglycaemic children was borne by those who were severely ill or malnourished (31.8%) as opposed to those who were neither severely ill nor malnourished (9.0%). Neonates: 23.0% of neonates were hypoglycaemic on admission. Inability to breast feed and weight |2500 g were independently
Cromer & District Hospital is due to be expanded as part of the future plans by the Norfolk and Norwich University Hospitals NHS Foundation Trust which runs the hospital. NNUH has agreed to redevelop one of the original buildings on the Cromer & District Hospital site and bring it up to modern standards.. Part of this building has been modernised already and contains the renal dialysis unit. An existing building on the hospital site will be redeveloped to create the new medical unit which will provide services such as chemotherapy, blood transfusion and rheumatology treatments. It will also free up space in the main Cromer Hospital building to deliver surgical treatments in dermatology, urology, vascular surgery and pain management.. Due to the previous generosity of local individuals and the community we are able to use our existing charitable funds to fund much of the scheme. The NHS pays for the staffing and running costs of Cromer & District Hospital and this will include the new unit. A new ...
Results A total of 203 referrals (n = 109 in 2013, n = 94 in 2014) were analysed: choledocholithiasis (n = 72), hepatobiliary cancers (n = 32), cholelithiasis (n = 36), liver disease (n = 23) and others (n = 40).. For all pathology, we managed to half the average time from referral to first decision by a consultant in 2014. The mean waiting time for ultrasound as the first choice of investigation improved by 3.2 days and for MRCP by 3.1 days in 2014. It took on average 3.2 days longer to obtain a CT in 2014.. For choledocholithiasis, the time interval from referral to ERCP in 2013 (2-49 days, mean = 18.1) were similar to 2014 (2-47 days, mean = 17.0) despite mean improvement of 2.2 days and 2.8 days in the waiting times for ultrasound and MRCP respectively. This was due to selected cases where CT was also required as it took on average 5.8 days longer to obtain a CT in 2014 in this group.. For cancers, the time from referral to biliary decompression (ERCP/PTC) improved by 3.5 days from 2013 ...
This is the first UK study to show that patients admitted to DGHs with unstable angina or NSTEMI and who are stratified at high risk wait significantly longer for angiography and revascularisation than similar patients admitted to a hospital containing a tertiary centre for cardiac services. Specifically, DGH patients were found to wait over two and a half times longer for angiography and over twice as long for revascularisation. The study hypothesis has therefore been proved.. The baseline characteristics of the MRI and DGH groups of patients were well matched except that more MRI than DGH patients had previously undergone coronary revascularisation. This is probably due to the fact that patients who have had a coronary procedure tend to represent to the hospital where they had it done and often remain under regular follow up at the tertiary centre. It is not clear why significantly more DGH patients had raised cholesterol.. If the "hit list" for invasive assessment and treatment of patients ...
University Hospital Crosshouse is a large district general hospital in Kilmarnock, Scotland, built to replace the old Kilmarnock Infirmary. It is run by NHS Ayrshire and Arran providing services to the North Ayrshire and East Ayrshire areas, including the Isle of Arran. It is located one mile outside the town in the village of Crosshouse. The hospital was completed by Melville Dundas & Whitson in 1978 and replaced the Kilmarnock Infirmary. It was opened as the North Ayrshire District General Hospital, but staff soon took to calling it "Nadge," hence the adoption of the local place name of Crosshouse. It was called Crosshouse Hospital until March 2012 when the board decided to change the name to University Hospital Crosshouse, as a result of a partnership with the University of the West of Scotland. In 2006, a new Maternity Unit was opened in the grounds of the hospital. The Ayrshire Maternity Unit replaced a similar unit at Ayrshire Central Hospital, Irvine. The hospital has a radio station, ...
What would be my advice to a young neurologist, boldly birthing from the womb of the neurosciences ivory tower training ground to practice in the reality of medicine that is found in the district general hospital (DGH)? How could it be possible to survive without the skills of the ever available super-expert neuroradiologist and the safety net of the all embracing specialist centre? The basic principles are of course simple common sense, and the specifics depend upon knowing and understanding radiologically useful neuroanatomy and the range of normal appearances. My remarks are directed primarily to the newly appointed consultant as she (or he) takes up a post which involves a mixed neurological practice of the hub and spoke variety or one based mainly in a district hospital. Established consultants will have either resolved the problems or alienated the local general department by insisting that all of her (or his) important examinations are referred up to the neuroradiology department for a ...

In recognition of Congenital Heart Defect Awareness Week (Feb. 7-14), a Harney County resident shares the story of dealing with her sons heart defect, and why she became involved with the LIFE (Local Individuals For Emergencies) Committee at Harney District Hospital (HDH).. Heart defects were never on my radar. As far as I knew, our family had no history of them (and by the size of my extended family, we have certainly had a chance to test these things out!) I had very limited awareness of the prevalence of congenital (present at birth) heart defects. But on Martin Luther King, Jr. Day in 2015, everything changed when Dr. Sharon King called and said: "It looks like theres something wrong with your babys heart." We were so excited to be adding to our family, but with those words, the second half of my pregnancy became a world of unknowns and required a whole lot of faith.. The next several weeks took us to Bend, and then to Portland. At each step, we were given a range of scenarios to expect, ...

Updated 27 January 2017. We inspected Eastbourne Hospital as part of the East Sussex Healthcare NHS Trust inspection on 4,5 and 6 October 2016. The trust had been previously inspected in September 2014 and March 2015. On both inspections we identified serious concerns and gave the hospital an overall rating of inadequate. The trust was rated inadequate overall because the two location reports and the concerns that we identified across the trust relating to culture and governance. A Quality Summit which included all key stakeholder organisations was held in September 2015 and, following that meeting, I recommended that the trust be placed into Special Measures. This meant that the trust was subject to additional scrutiny and support from the local clinical commissioning groups and NHSI who provided an improvement director to advise and to monitor the implementation of action plans to address the shortcomings identified. The commission also maintained a heightened programme of engagement and ...
Sembabule District is a district in the Central Region of Uganda. The town of Sembabule is the site of the district headquarters. Other urban centers in the district include Lwemiyaga and Ntuusi. Sembabule District is bordered by Mubende District to the north, Gomba District to the northeast, Bukomansimbi District to the east, Lwengo District to the south, Lyantonde District to the southwest, and Kiruhura District to the northwest. The town of Sembabule is approximately 48 kilometres (30 mi), by road, northwest of Masaka, the largest town in the sub-region. Sembabule District was carved out of Masaka District in 1997. The district is generally a rural district that receives relatively low rainfall and has long dry spells. This greatly affects agriculture, but cattle keeping is not seriously affected. Some cattle are reared on ranches. Ranching and dairy farming are the backbone of the districts economy. Administratively, the district has two counties (Lwemiyaga and Mawogola Counties) that are ...
Baseline LDL-C levels were similar in both centres. NICE criteria were met for 2.05 items in the whole study (UH patients 1.7 and DGH patients 2.7). District general hospital patients were more likely to have familial hypercholesterolaemia (89 vs 69%; P = .02); intolerance to statins (94 vs 52%; P < .001) and polyvascular disease (42% vs 17%; P = .005). Prescriptions (evolocumab 73%; alirocumab 23%) were collected by 76% of patients (UH 64% vs DGH 100%). Therapy was discontinued by time of review in 15% of patients (UH 7% vs DGH 25%; P = .02). In adherent patients PCSK-9 inhibitor treatment reduced TC by 28% (2.24 ± 2.39 mmol/L; P < .001) and LDL-C by 49% (2.10 ± 1.33 mmol/L; P < .001). A LDL-C < 2.5 mmol/L was achieved in 30% of patients and ...

Obstetrical and abdominal emergencies, and trauma, much of it affecting children, represent a major and neglected part of Africas burden of disease. Countries cannot train and retain sufficient specialist surgeons (doctors) to address these priorities. We propose a surgical training intervention targeted at Clinical officers (COs), who are trained non-physician clinicians that form the backbone of Africas district hospital services. Lacking medical degrees, COs have fewer opportunities for emigration. Surgical training of non-clinician physicians has been tried and reportedly worked well in African countries. It has never been rigorously evaluated, nor been subject to economic and population impact assessments. Two different 2 year training models are proposed: district hospital in-service training in Malawi and centralised training in Zambia. Before-and-after and randomized controlled trial evaluations are planned. The latter is the strongest study design for evaluating an intervention. ...

A service performing very early abortions in a district general hospital uses menstrual aspiration, which enables abortion to be carried out early, easily, safely, and simply. Menstrual aspiration avoids interference with routine gynaecological work and permits an operator to widen his criteria for accepting patients for termination. From September 1975 to April 1977 123 cases were referred and 104 aspirations were performed. Termination of pregnancy failed in only two cases, through inexperience, and infection occurred in only one case. Menstrual aspiration should be performed within 10-18 days of missing a period. ...
