A prospective study was undertaken from 1969 to 1974 to evaluate a program establishing coronary care units in small community hospitals. Crude mortality rates from acute myocardial infarction in these small hospitals were 14.5%, slightly higher but not statistically different from concurrently collected data in three larger comparison hospitals (11.5%). Multivariate discriminate analysis of clinical characteristics was used to calculate a risk score for each patient admitted. Observed deaths exceeded expected deaths (134 versus 119) (P , 0.30) in small hospitals but observed deaths were less than expected (55 versus 77) (P , 0.05) in comparison hospitals. Mortality was significantly greater in those units admitting fewer than 60 patients with infarctions yearly (20.9%) than in those admitting more than 60 (10.7%) (P , 0.001). Carotid stenting has become the treatment of choice in patients who need carotid endarterectomy (CEA) but are at high surgical risk (1,2). Our study also demonstrated that a substantial proportion of patients are at low risk for such events. Initial identification of substantial proportions of the patient population at either extreme of the risk spectrum could lead to more effective and efficient allocation of nursing staff and monitoring modalities that would match resources to patient need in tiered care units.. BACKGROUND: Adverse drug events (ADEs) occur often in hospitals, causing high morbidity and a longer length of stay (LOS), and are costly. However, most studies on the impact of ADEs have been conducted in tertiary referral centers, which are systematically different than community hospitals, where the bulk of care is delivered, and most available data about ADE costs in any setting are dated. Costs in community settings are generally lower than in academic hospitals, and the costs of ADEs might be as well. To assess the additional costs and LOS associated with patients with ADEs, a multicenter retrospective cohort study was conducted in six community hospitals with 100 to 300 beds in Massachusetts during a 20-month observation period (January 2005-August 2006). Because most community hospitals in Japan do not maintain 24-h availability of in-house anesthesiologists, surgeons, and interventional radiologists, staffing dramatically declines during off hours. It is unclear whether, in such under-resourced hospitals, trauma patients presenting during off hours and requiring subspecialty intervention have worse outcomes than those who present during business hours. This was a retrospective cohort study at a community hospital in Japan. Participants were all injured patients requiring emergency trauma surgery or transarterial embolization who presented from January 2002 to December 2013. We investigated whether outcomes of these patients differed between business hours (8:01 AM to 6:00 PM weekdays) and off hours (6:01 PM to 8:00 AM weekdays plus all weekend hours). October 4, 2013 - A telestroke service increases the rate of effective tissue plasminogen activator (tPA) therapy for patients with acute ischemic stroke treated at community hospitals, according to a report in the October issue of Neurosurgery. Stavropoula Tjoumakaris, M.D., and colleagues of Thomas Jefferson University Hospital evaluated the effects of a telestroke network that included 28 community hospitals in the Philadelphia area. In the network, stroke specialists used "remote presence" technology to consult on the care of stroke patients seen at hospitals that did not have a specialized stroke unit. The study evaluated the rate of appropriate treatment with tPA.. Over 18 months, the service provided nearly 1,650 remote stroke consultations.
