Downloadable! This study measures the impact of information technology (IT) use on hospital operating costs during the late 1980s and early 1990s. Using a proprietary eight-year panel dataset (1987-1994) that catalogues application-level automation for the complete census of the 3,000 U.S. hospitals with more than 100 beds, this study finds that both financial/administrative and clinical IT systems at the most thoroughly automated hospitals are associated with declining costs three and five years after adoption. At the application level, declining costs are associated with the adoption of some of the newest technologies, including systems designed for cost management, the administration of managed care contracts, and for both financial and clinical decision support. The association of cost declines with lagged IT as well as the cost patterns at the less automated hospitals both provide evidence of learning effects.
Monitoring the quality of health care by means of performance indicator scores is part and parcel of national health care systems. Performance indicators (PIs) are used to monitor and improve quality and patient safety and to stimulate accountability and market processes in countries worldwide (e.g. USA (http://www.ahrq.com), UK (http://www.hqip.org.uk), and Denmark (http://www.ikas.dk). To play this role effectively, performance indicators need to be reliable and valid measures of health care quality [1-3] particularly when hospitals performances are ranked and published in the lay press [4] and/or used to link reimbursement to indicator results [4, 5].. National hospital performance indicator programs commonly use PIs that are selected on basis of expert judgment (e.g. medical doctors, patient organizations) and existing scientific evidence [6] about valid relations between health care processes and outcome indicator (e.g. [7]). These PIs have often been successfully implemented in other ...

A hospital information system comprises a data processing system including a plurality of terminals having display means and data entry means. Patient information is entered into the system via the terminals, is organized hierarchically in the system, and may be displayed to users having proper access to the system. The system provides a time-oriented task list, which is automatically generated from data which has been entered from physicians and nursing orders. Tasks may be charted by a system user without exiting from the task list, and all associated form(s) are automatically updated.

Healthcare work is, to a considerable extent, cognitive. Subsequently, the analysis and the design of supporting technology must be sensitive to the cognitive and adaptive demands of the work and to the cognitive strategies employed by healthcare practitioners. Despite the vital role that cognition plays in healthcare work, current technocentric design approaches for healthcare technology do not account for it, failing to observe it during analysis and failing to develop support for it during design. By review and analysis of case studies, we show that healthcare systems developed without input from cognitive analysis and cognitive design fail to take account of important healthcare work processes and workflows. In contrast, systems developed with a cognitively-focused design strategy demonstrate how it is possible to introduce technology that supports and enhances the work strategies of those engaged in patient care. Significant problems emerge when technological support systems are developed without

OBJECTIVE: The ability to accurately and efficiently identify patient cases of interest in a hospital information system has many important clinical, research, educational and administrative uses. The identification of cases of interest sometimes can be difficult. This paper describes a two-stage method for searching for cases ofinterest.. DESIGN: First, a Boolean search is performed using coded database variables. The user classifies the retrieved cases as being of interest or not. Second, based on the user-classified cases, a computer model of the patient cases of interest is constructed. The model is then used to help locate additional cases. These cases provide an augmented training set for constructing a new computer model of the cases of interest. This cycle of modeling and user classification continues until halted by the user.. MEASUREMENTS: This paper describes a pilot study in which this method is used to identify the records ofpatients who have venous thrombosis.. RESULTS: The results ...

In the medical fields, many efforts have been made to develop and improve Hospital Information System (HIS) including Electronic Medical Record (EMR), Order Communication System (OCS), and Picture Archiving and Communication System (PACS). However, materials generated and used in medical fields have various types and forms. The current HISs separately store and manage them by different systems, even though they relate to each other and contain redundant data. These systems are not helpful particularly in emergency where medical experts cannot check all of clinical materials in the golden time. Therefore, in this paper, we propose a process to build an integrated data model for medical information currently stored in various HISs. The proposed data model integrates vast information by focusing on medical images since they are most important materials for the diagnosis and treatment. Moreover, the model is disease-specific to consider that medical information and clinical materials including ...
Medical systems including Hospital Information System (HIS) are constantly evolving for digitalizing and efficiently managing medical information. Nowadays most of medical institutions like hospitals naturally use HIS. Recently, many studies [1-10] have tried to efficiently store, transfer, and manage clinical materials. However, HIS consists of several systems such as Order Communication System (OCS), Electronic Medical Record (EMR), and Picture Archiving and Communication System (PACS) and they generate clinical materials of different types. Those systems tend to separately and redundantly include one patients information. In other words, since information about a patients condition is contained in various materials, medical experts should read all of charts or reports and interpret all of images or videos before diagnosing. Therefore, systems should be able to provide and manage essential data required in emergency so that medical experts can make a quick decision.. Therefore, one of the ...
Mentor: Jennifer F. Tseng, MD, MPH, University of Massachusetts Medical School. The importance of an electronic medical record has been highlighted for both clinical care and research. The aim of this study was to compare a newly developed institutional clinical data warehouse, linked with the hospital information system (HIS), to a prospectively maintained departmental database. A novel HIS-linked institutional clinical data warehouse was queried for 9 primary and secondary ICD-9-CM discharge diagnosis codes for pancreatic cancer. This was compared to a manually entered and prospectively maintained surgical oncology database at the same institution. 1107 patients were identified from the HIS-linked dataset with pancreatic neoplasm-associated diagnosis codes dating from 1999 to 2009. Of these, 254 (22.9%) were captured in both datasets, while 853 (77.1%) were only in the HIS-linked dataset. Manual review of the 10% subset of the HIS-only group demonstrated that 55.6% of patients were without ...
Definition : Computed tomography (CT) study software designed to acquire, process, and archive data and images obtained during blood flow studies. This software usually works with CT scanning systems and post-imaging workstations; it may also interact with devices used in the procedures, such as monitors, printers, contrast media injectors, and recorders. The software is intended to work only with one or a few models of CT scanning systems and related devices, usually supplied by the same manufacturer. This also facilitates the processing and/or sending of data to and from hospital information systems and other (e.g., picture archiving and communication) workstations. Blood flow studies typically use CT images to analyze blood vessel lumen and stenosis and/or thrombus characteristics; dedicated software for assessment of the aorta and cerebral blood flow is also available. The software may or may not be available separate from the CT equipment and may be updated and/or upgraded (i.e., through ...
My background is completely healthcare. I started at the lowest level cleaning utility rooms and taking bodies to the morgue. And somehow ended up in the arena of systems and information technology, where I installed and managed applications. I have worked for healthcare providers, software & hardware vendors, and consulting firms focused on defining systems needs and implementing information solutions in healthcare environments. These solutions ranged from the general accounting areas to hospital information systems to ancillary systems such PACs, pharmacy, and laboratory. I have seen systems evolve from just feeding the billing system to todays valuable decision support systems and evolving medical records for clinicians.. My work in the healthcare field has also given me the great opportunity to work in the U.S., Europe, Middle East, Africa, and Latin America, where I have had the chance to learn about numerous healthcare systems and see many providers at work in various ...

Despite all the hoopla about clinical and business intelligence (C&BI) applications, the use of these tools by hospitals and healthcare systems is still in an early phase, a new report indicates. Only 46% of the 529 respondents to a HIMSS Analytics survey said they were using C&BI, and the majority of those indicated they were still learning how to use these analytic tools. Also revealing was the fact that more than half of C&BI users said they were using the analytic modules imbedded in their electronic health record/hospital information system (EHR/HIS). In contrast, less than a quarter of the C&BI users had purchased best of breed solutions, which tend to be more robust than those in EHR/HIS products.

Another method that we found works at Motorola, is to keep DHCP, and using the following in a batch script, on Windows OS machines. We do not have a problem with our Macintosh systems and DHCP and the Retrospect Client. net stop Retrospect (Whatever it is called in your Services Control Panel) ipconfig /release ipconfig /renew net start Retrospect This causes the DHCP renewal to take place, when the client has been stopped and then starts it again, once the new IP address has been obtained. We put a shortcut of this batch file on their desktops, and when they come in and dock, the double-click this icon, and it take 10 seconds. It also resolves any network problems they maybe having with expired DHCP leases, that are not automatically renewed. ..Greg At 6:56 AM -0700 4/18/00, Ronnie Livingston wrote: ,In my experience with Retrospect, it does not do a very good job handling ,clients that change their IP addresses. Presents both basic and advanced methods for statistical modeling of ICU data. The intensive care unit (ICU) is one of the major components of the current health care system. The complex tasks of collecting and analyzing data on performance measures are made easier when clinical information systems are available, but few clinical information systems have attempted to gather clinical information generating full reports that provide a panorama of the ICU performance and detailed data on several domains.. This book presents statistical methods - with special focus on including innovative approaches - that allow handling the specificities of ICU data, enabling practitioners to conduct appropriate analyses of their own data.. Organized in distinct sections which will provide both introduction and advanced understanding according to the level of the reader, this book will prove a valuable resource to either statisticians involved in ICU studies, or ICU physicians who need to model statistical data. ...

We have now released ISC DHCP 4.3.0a1. The theme for this release is "v6 uplift." We are upgrading some of the features youve used for DHCPv4 to make them available for DHCPv6. In general weve tried to use the same or similar commands so that moving from a DHCPv4 feature to DHCPv6 is as painless as possible.. Over time we plan to add several articles to our Knowledgebase to describe some of the new features and how you might configure them. You can also review them by watching a webinar we presented on the new features: ISC YouTube - ISC DHCP 4.3.. As always, wed appreciate anyone who can test out the new (and old) features and report any problems back to us. Please remember this is an alpha release - while we have done our own testing, the software is not suitable for production purposes quite yet. If all goes well we should be releasing a beta version in early January.. The tarball and release notes for this release can be found on our FTP site: ftp://ftp.isc.org/isc/dhcp/.. ISCs Release ...
This is quite a bit more complicated than for services running on the router itself, but still not too much of a challenge. First you have to set up the pc in the lan that runs the server (referred to from here on as the host) with a static ip address. Because the ip addresses are allocated with DHCP, a host might be given a different ip address each time it connects to the network. You can easily give it a static ip address by telling the DHCP server to give it the same ip address each time ...
