Downloadable! This study measures the impact of information technology (IT) use on hospital operating costs during the late 1980s and early 1990s. Using a proprietary eight-year panel dataset (1987-1994) that catalogues application-level automation for the complete census of the 3,000 U.S. hospitals with more than 100 beds, this study finds that both financial/administrative and clinical IT systems at the most thoroughly automated hospitals are associated with declining costs three and five years after adoption. At the application level, declining costs are associated with the adoption of some of the newest technologies, including systems designed for cost management, the administration of managed care contracts, and for both financial and clinical decision support. The association of cost declines with lagged IT as well as the cost patterns at the less automated hospitals both provide evidence of learning effects.

Healthcare work is, to a considerable extent, cognitive. Subsequently, the analysis and the design of supporting technology must be sensitive to the cognitive and adaptive demands of the work and to the cognitive strategies employed by healthcare practitioners. Despite the vital role that cognition plays in healthcare work, current technocentric design approaches for healthcare technology do not account for it, failing to observe it during analysis and failing to develop support for it during design. By review and analysis of case studies, we show that healthcare systems developed without input from cognitive analysis and cognitive design fail to take account of important healthcare work processes and workflows. In contrast, systems developed with a cognitively-focused design strategy demonstrate how it is possible to introduce technology that supports and enhances the work strategies of those engaged in patient care. OBJECTIVE: The ability to accurately and efficiently identify patient cases of interest in a hospital information system has many important clinical, research, educational and administrative uses. The identification of cases of interest sometimes can be difficult. This paper describes a two-stage method for searching for cases ofinterest.. DESIGN: First, a Boolean search is performed using coded database variables. The user classifies the retrieved cases as being of interest or not. Second, based on the user-classified cases, a computer model of the patient cases of interest is constructed. The model is then used to help locate additional cases. The aim of this study was to compare a newly developed institutional clinical data warehouse, linked with the hospital information system (HIS), to a prospectively maintained departmental database. A novel HIS-linked institutional clinical data warehouse was queried for 9 primary and secondary ICD-9-CM discharge diagnosis codes for pancreatic cancer. This was compared to a manually entered and prospectively maintained surgical oncology database at the same institution. 1107 patients were identified from the HIS-linked dataset with pancreatic neoplasm-associated diagnosis codes dating from 1999 to 2009. Of these, 254 (22.9%) were captured in both datasets, while 853 (77.1%) were only in the HIS-linked dataset. Despite all the hoopla about clinical and business intelligence (C&BI) applications, the use of these tools by hospitals and healthcare systems is still in an early phase, a new report indicates. Only 46% of the 529 respondents to a HIMSS Analytics survey said they were using C&BI, and the majority of those indicated they were still learning how to use these analytic tools. Also revealing was the fact that more than half of C&BI users said they were using the analytic modules imbedded in their electronic health record/hospital information system (EHR/HIS). In contrast, less than a quarter of the C&BI users had purchased best of breed solutions, which tend to be more robust than those in EHR/HIS products.
