The organization of psychiatric services for general hospital departments might change in far-reaching ways in the coming decades. Whereas the focus was primarily on reactive services for inpatients on medical and surgical wards, the future should focus on more proactive integrated service delivery for the complex medically ill. The essential difference from other psychiatric services is that the population served is taken care of by medical specialists in the general health setting. Consequently services are delivered in the context of the medical-psychiatric interface. Consult requests are always formulated in this perspective: the patient is treated for a medical illness or physical complaints and there are signs of an interfering psychiatric disorder. Nowadays these patients are referred to as the complex medically ill. Therefore triage and treatment integrated in the medical context is the area of expertise of consultation-liaison (CL) psychiatrists. The development of this area of ...
In recent years, there has been a significant increase in the incidence of fungal infections attributed to Candida species worldwide, with a major shift toward non-albicans Candida (NAC). In this study, we have described the distribution of Candida species among different hospital departments and calculated the antifungal consumption in our facility. We also correlated the consumption of certain antifungals and the prevalence of specific Candida species. This was a retrospective review of all the Candida isolates recovered from the computerised microbiology laboratory database of Makassed General Hospital, a tertiary care centre in Beirut, Lebanon, between January 2010 and December 2015. Data on antifungal consumption between January 2008 and December 2015 were extracted from the hospital pharmacy electronic database. We used Spearmans coefficient to find a correlation between Candida species distribution and antifungal consumption. Between 2008 and 2015, we observed that the highest antifungal

In an effort to achieve site-neutrality in terms of Medicare reimbursement between certain off-campus provider-based departments of hospitals (PBDs) and free-standing physician clinics, the Centers for Medicare & Medicaid Services (CMS) has announced further reductions in reimbursement for certain items and services furnished by those PBDs that began billing under the Medicaire Outpatient Prospective Payment Sytem (OPPS) on or after November 2, 2015 (Non-Excepted PBDs).. Dalys family members joined about 150 people at the medical center Thursday for a dedication ceremony that named the emergency department The Michael J. Daly Center for Emergency and Trauma Care. Susan L. Davis, president and CEO of St. Vincents Medical Center, called Daly a gentle giant and the conscience of the board, and said giving approval for the use of his name was the most generous of gifts Daly gave the medical facility. There could be no greater honor for my father than to name this emergency department after him; this place of great caring, this safety net for the community, said Deirdre Daly, Dalys daughter, shortly before she and other family members unveiled the interior signs for the refurbished emergency center, which will open in January. Illuzzi said the new emergency center, when completed sometime next year, will have more than triple the space, 62 beds, specialized trauma and critical care suites, an expanded behavioral health area, and a reduction in the door-to-doctor

The effects of a renal unit culture over time on ANY young patient, over time is going to be considerable. As the staff are considered to be coming from a healing and therefore positive mission, their effect on the young patient over time and also on the patients family are often overlooked. They frequently build relationships over time with members of your family. I have heard of the horrific undermining of young adult women with consultants telling their boyfriend/ partner or husband that she wont be able to have children you know. In my experience I had a bad experience of a mentally abusive husband, getting in a rage and phoning my consultant and claiming Id given him permission to see him without me present He was told theres nothing to worry about - she is just a bit depressed My damagy ex then proceeded to bring in all sorts of odd people from the church to cheer me up and tell I ought t trust Jesus more!! The same husband came raged at me for dropping a tea cloth on the floor. ...

The World Health Organization (WHO) has stated that preparedness for effectively facing a major influenza epidemic should involve the training of physicians in the management of contagious diseases and upgrading hospital resources and procedures [1]. Children would be particularly vulnerable during an influenza pandemic and specific measures are needed to face the threat to them effectively. We performed a national survey to obtain information about the preparedness in facing a major influenza outbreak in Italian paediatric units. In Italy, paediatrics clinics are found in both paediatric wards and paediatric departments. Departments are more complex structures, containing several units with different specialisations and facilities. For this study, we interviewed heads of both departments and units. A structured questionnaire, including 30 items, was submitted to the heads of 150 paediatric hospital departments across the country. Responses were obtained from 123 units; 10% of these had rooms dedicated
The Philadelphia Department of Public Health (PDPH) is asking hospitals and urgent care facilities to administer COVID-19 Vaccine to patients upon discharging in emergency departments. Hospitals, ED, and UC facilities in the United States play an influential role in building confidence in and improving COVID-19 vaccine uptake, as healthcare providers are the most trusted source of health information.. Emergency departments serve as the primary, and at times only, health care access point for up to 20% of the US population, and urgent care clinics handle more than 29% of primary care visits in the country. We encourage hospitals to expand COVID-19 vaccination efforts to all hospital departments, including postpartum discharges and patients being discharged to long-term care of rehabilitation facilities. Expanding COVID-19 vaccine availability via these locations also makes vaccine more accessible to patients.. We hope this helps push Philly towards reaching the national goal of 70% vaccination ...

Being deeply entrenched in the tech industry and having spent most of my career practicing medicine or working as a hospital executive, I often ponder how advances in artificial intelligence, machine learning, machine vision, and robotics will impact health and healthcare delivery. I can tell you that I have seen very clear signs that these technologies will have an impact. In fact, Im sure some of you who work in healthcare have already experienced how technology is reshaping things, and in some cases eliminating jobs. The hospital lab is an easy example. I remember when highly educated lab workers peered into microscopes to manually count and differentiate blood cells. Today, this is mostly done by machines. In fact, many manual processes and procedures in the lab have been completely taken over by machines in the modern hospital. Im sure you can think of examples in other hospital departments as well, such as autonomous robotic carts that now travel the elevators and hallways delivering ...

A patients chance of surviving a heart attack dramatically increases if the patient or a bystander calls 911 and uses emergency medical services (EMS). Thats why Augusta University Health invests not only in early heart attack care but also in ensuring an efficient critical care process - and it starts with EMS.. This process, managed by Michael Willis, EMS coordinator at Augusta University Health, includes timely feedback to EMS, and between EMS and critical care areas.. My main focus is to provide EMS with the information that they need so we can work together to strengthen and improve the care our patients receive and their outcomes, Willis said. If EMS gets an early electrocardiogram reading, then we can activate critical resources faster. This partnership increases the chance of recovery dramatically.. Until Augusta University Health created the EMS coordinator role, each individual hospital department designated a staff member who took on the responsibility of reporting back to EMS. ...

It offers a potentially easy intervention to improve things such as infection of the fetus and mother, and also premature delivery. Both of these are problems that can cause later abnormalities in the infants brain development so treatment with vitamin D could have a really important effect on later neurologic impairment in children ...

The report estimates that on an average day in July, there are 502 hospital emergency department visits involving underage alcohol use. For the three day Fourth of July weekend however, the number of daily hospital emergency department visits jumps to 938.. Underage drinking is not a harmless right of passage. It has far-reaching consequences. Disinfectants and antiseptics of different preparations and concentrations have been used universally on animate and inanimate surfaces in hospitals (1 4). The observed increase in the resistance of a number of bacterial pathogens to some disinfectants and antiseptics has resulted in the development of new preparations as well as the introduction of various strategies to achieve optimal efficacy and effectiveness of disinfectants and antiseptics (5, 6).. It has been documented that stock solutions of disinfectants/antiseptics sold commercially, diluted disinfectants/antiseptics prepared in hospital pharmacies for distribution to the various hospital departments, and disinfectants/antiseptics that are in use in hospital departments have been exposed to contamination (7, 8). The levels of contamination by some of these bacteria have ranged from 102 to 108 colony-forming units per mL of disinfectant/antiseptic, with the possibility of attaining an infective dose at the site of application (8, 9). ...

High quantitative work demands (have to work in rapid pace, work piles up etc.) were seen in hospitals (somatic departments) as well as in practice areas outside hospitals (nursing homes and community nurse units). These work units are quite different from each other in patient turnover, organisational structure, and professional prestige. However, both somatic hospital departments and nursing homes serve patients with severe physical disorders, a fact that is likely to have a strong impact on the magnitude of the aides work tasks. In community nurse units, it may be difficult to limit the number of patients under treatment, and the distance from one patient to another may be long. Hence, the number of personnel that is needed in these work units may easily be underestimated by those responsible for the personnel situation. High physical demands were seen in both nursing homes and somatic hospital departments. The reason is obviously the type of patients that are served in these work ...

During daily rounds in internal medicine departments, it is a custom to encourage, both patients and families, to go home as soon as it is considered appropriate. However, many patients are staying in-hospital after discharge eligibility: some due to family incompetence or failure in reassurance by the attending physician, some due to objective obstacles in assuring appropriate treatment continuity in the setting of primary medicine and some due to lack of vacant beds at their next, rehabilitation or nursing facility. We wanted to make sure, that our stringent policy of advancing discharge due to fear of nosocomial infections is indeed justified. We did not find in the relevant literature, previous studies that fully answered our needs with regard to the population of elderly patients hospitalized in internal medicine departments. Harkanen and co. showed that prolonged hospital stay was associated with increased risk for adverse drug reactions [6]. In another study [7], prolonged hospitalization ...
That just couldnt happen here, but the sooner we develop written standards and best practices for the liberation procedure and observe them in practice, the safer the MS community will be, says Dr. Gupte. The way it is now is just madness. Everyone seems to be taking shortcuts. We know that it is expensive to keep patients in a clinical setting over a single night much less 10 days, but its quite absurd to release them the same day they have the procedure. We have always believed it to be unsafe and now it has proven to be unsafe. The thing is, are Synergy Health Concepts and other clinics doing the Liberation Treatment going to be changing their aftercare methods even though they know it is unsafe to release a patient on the same day? The answer is no, even after Mrs. Clarkes unfortunate and unnecessary death. Therefore, they are not focused on patient safety…its become about money only and lives are being put at risk as a result ...

Orderlies are often utilized in various hospital departments. Orderly duties can range in scope depending on the area of the health care facility they are employed. For that reason, duties can range from assisting in the physical restraint of combative patients, assisting physicians with the application of casts, transporting patients, shaving patients and providing other similar routine personal care to setting up specialised hospital equipment such as bed traction arrays. Orderlies are typically found in emergency departments, operating rooms, psychiatry, long term care, and orthopaedics. Orderlies are described as non licensed hospital assistants that are instructed to perform delegated functions under the direct supervision of a licensed practitioner in the health care setting. In the US, orderlies have been phased out of health care facilities in recent years and their functions are now replaced by the patient care assistant and Certified Nursing Assistant. They remain common in Canada and ...

Clinical shifts in our Emergency Department are dynamic, fast-paced and challenging. We are required to make many decisions about our patients in collaboration with inpatient specialist teams and the other hospital departments.. We manage on average 190 patients each day in Geelong ED with a seasonal variance, as Geelong is a popular tourist destination. The following cases are a selection of the patients I was involved with on a weekday shift recently. To put things in perspective, over this particular 24-hour period, there were 210 new ED attendances of which 73 were admitted (35%) and 137 reviewed, treated and discharged (65%). There were 3 trauma team calls, 3 stroke team calls and 1 code STEMI. There were 53 paediatric attendances. Our flight retrieval doctor was dispatched to regional Victoria to repatriate a critically unwell man to Melbourne.. First up in resus, my shift started with a young child with a small wound on his lip after a fall at home earlier that morning. Using ...
An upcoming ballot initiative in California includes a provision that would require physicians to be drug-tested prior to practicing at any hospital, or after their involvement in an adverse event. This is packaged with other measures that appear political and punitive towards all physicians. But the drug-testing provision bears scrutiny because, while drug testing is widespread in American business, and required of nurses and many hospital and medical workers, private doctors are not routinely tested.. Id like to tell you it isnt a problem for doctors, but unfortunately, Ive seen firsthand that there are physicians who practice while they are under the influence. As someone who oversaw doctors as chief of a hospital department charged with monitoring the quality of its members, the idea of a colleague practicing while impaired is terrifying. And while we physicians have a direct responsibility to protect patients, we often find it hard to speak up when we see something. The attitude is: ...
BACKGROUND CONTEXT: Recent studies suggest that prospective registration more accurately reflects the true incidence of adverse events (AEs). To our knowledge, no previous study has investigated prospectively registered AEs influence on hospital readmission following spine surgery.. PURPOSE: To determine the frequency and type of unplanned readmissions after complex spine surgery, and to investigate if prospectively registered AEs can predict readmissions.. DESIGN: This is a prospective, consecutive cohort study.. PATIENT SAMPLE: We conducted a single-center study of 679 consecutive patients who underwent complex spine surgery defined as conditions deemed too complicated for surgery at a secondary institute, or patients with severe comorbidities requiring multidisciplinary observation and treatment.. OUTCOME MEASURES: The outcomes in this study were (1) readmission to any hospital department within 30 days of discharge and (2) readmission to a surgical spine center at any time in ...

I also really cant talk about CPT without bringing up a little tiny thing from the hospital side called a charge description master (CDM), or as its more commonly called, the charge master. Its as masterful as it sounds: a line-item listing of everything a hospital department charges for. Each line item has a description of the charge, charge amount, and sometimes a CPT code. One of the most difficult transitions I see in pro-fee coders crossing over to the hospital side is not understanding that the coder doesnt code everything. There are many codes that are assigned automatically by the charge master when a charge is applied to the bill. This is the case when the CPT code doesnt require a lot of subjective reasoning (e.g., lab test or x-ray). For those procedures and services, such as operative procedures, that require subjective reasoning, a real-live coder will assign the code. It may sound counter intuitive, but this actually increases the amount of coding-related jobs in a hospital. ...

Many smokers believe they have tried and failed all treatments because they - and their physicians - have not used existing treatments in the most effective way, noted Nancy Rigotti, director of the Tobacco Research and Treatment Center of the Massachusetts General Hospital Department of Medicine. Outdated product labeling by the Food and Drug Administration for nicotine replacement therapy contributes to this problem, as does the fear of possible side effects of FDA-approved medications. As a result, some smokers consider using newer, untested and unregulated products like electronic cigarettes.. In her paper, based on a grand rounds presentation at Northwestern University Feinberg School of Medicine, Rigotti presents a hypothetical smoking patient: a 50-year old man who is discouraged because previous quit attempts - one with nicotine replacement, the other with the antidepressant bupropion - did not succeed. He is reluctant to try varenicline, another FDA-approved smoking cessation ...

Translated by Frederika Randall. 2. Destination, Hope. The ambulance is speeding towards Ciampino Airport, a police car escorting us. A jet owned by the private emergency service Rega is waiting to take me to Zurich, to the Balgrist University Hospital department specialising in treating spinal cord injuries. Im neatly packed into the gurney and the doctor travelling with me sits by the window, reading a newspaper. He gives me not a glance during the whole trip; he seems to think this boring task hes been assigned is a drag. And why should he care? In his eyes, Im just a parcel to be delivered. True, hes probably not much of a doctor if hes being used as a postman-no offence to postmen. The only one here who occasionally asks if Im okay is the nurse. The driver just curses the police in the car ahead, criminals, as he sees it, who are driving too fast.. The way these idiots are going, well be the ones needing an ambulance.. We arrive at Ciampino. For several minutes Im out on the ...
