Carotid artery dissection is a significant cause of ischemic stroke in all age groups.Spontaneous internal carotid artery dissection is a common cause of ischemic stroke in patients younger than 50 years and accounts for up to 25% of ischemic strokes in young and middle-aged patients. Dissection of the internal carotid artery can occur intracranially or extracranially, with the latter being more frequent. Internal carotid artery dissection can be caused by major or minor trauma, or it can be spontaneous in which case genetic, familial, and/or heritable disorders are likely etiologies. Patients can present in a variety of settings, such as a trauma bay with multiple traumatic injuries; their physicians office with nonspecific head, neck, or face pain; or to the emergency department with a partial Horner syndrome. Web-based information from state departments of education from all 50 states and from the District of Columbia were reviewed in the fall of 2004. Results indicated that only 16 states (31%) include a focus on student social behavior in their mission statements. Individual student behavior support practices were identified in the curriculum for general educators in 30 (59%) states and for special educators in 39 (76%) states. A current topic in paleontology that has received much popular press is the question of whether T.rex (or other Tyrannosauridae in general) were predators or scavengers. Lets explore this issue.. Paleontologist Jack Horner of the Museum of the Rockies (Bozeman, MT) has proposed that T.rex could not have been a predator. This is a condition most common in young adult females, which usually begins in one eye. The pupil is sluggish to react to light. Many people with this condition will also have diminished deep tendon reflexes and they can have trouble focusing at near. The condition is usually not associated with any more serious conditions. Some eyedrops have a dilating effect on the pupil, so eyedrop use is another cause of a dilated pupil. This, the study authors report, is in contrast to previous studies that positioned chimpanzees as reluctant altruists and led to the widely held belief that human altruism evolved in the last six million years only after humans split from apes. The current study findings are available in the online edition of Proceedings of the National Academy of Sciences. Schwannomas are benign, well-differentiated tumors that originate from Schwann cells. Involvement of the cervical sympathetic nerve is relatively rare. Computed tomography is indispensable for the diagnosis. The treatment is surgical. Histological examination confirms the diagnosis. Horners syndrome postoperatively is supportive of the diagnosis. The rarity of giant cervical sympathetic chain schwannoma made the case of our patient interesting to report. Furthermore, our patients immense tumor size is very rare, and we could not find any similar report in the literature. Cervical sympathetic chain schwannoma is frequently confused with schwannoma of the vagus nerve on clinical and radiological examination, and its diagnosis can therefore be challenging for clinicians, radiologists, and pathologists. We present the genetic analyses conducted on a three-generation family (14 individuals) with three members affected with isolated-Hirschsprung disease (HSCR) and one with HSCR and heterochromia iridum (syndromic-HSCR), a phenotype reminiscent of Waardenburg-Shah syndrome (WS4). WS4 is characterized by pigmentary abnormalities of the skin, eyes and/or hair, sensorineural deafness and HSCR. None of the members had sensorineural deafness. The family was screened for copy number variations (CNVs) using Illumina-HumanOmni2.5-Beadchip and for coding sequence mutations in WS4 genes (EDN3, EDNRB, or SOX10) and in the main HSCR gene (RET). Assuming that the entire cervical region is to be exposed, make a ventral midline incision from the level of the larynx cranially to the level of the manubrium caudally (Fig. 40-6).
