BACKGROUND AND PURPOSE: Isolated Horner syndrome without associated cranial nerve palsies or ischemic symptoms is an important presentation of spontaneous internal carotid artery dissection (sICAD). Ultrasound is often used as a screening method in these patients because cervical MRI is not always available on an emergency basis. Current knowledge on ultrasound findings in patients with sICAD presenting with isolated Horner syndrome is limited. METHODS: Patients were recruited from prospective cervical artery dissection databases of 3 tertiary care centers. Diagnosis of sICAD was confirmed by cervical MRI and MR angiography or digital subtraction angiography in all patients. Pharmacological testing in Horners syndrome involves the use of cocaine to confirm the diagnosis and hydroxyamphetamine to localise the lesion to the post-ganglionic (third order) or non-postganglionic neuron. However, hydroxyamphetamine bromide 1% (Paredrine) is not always readily available to the ophthalmologist. An alternative drug for localising the site of the lesion is phenylephrine 1% which can easily be prepared by dilution of stronger concentrations (2.5% or 10%) and which is almost universally available in most ophthalmologists offices. Because of the principle of denervation supersensitivity, a Horners syndrome produced by a lesion interrupting the postganglionic fibres should dilate the pupil when phenylephrine 1% is placed in the conjunctival sac. Hes practically had to swat me away a few times when Ive tried to help him too much. He has handled being off his feet and not skating so much better than I would have guessed. Watching him struggle to get used to the cast was hard. Weve had lots of conversations about risk taking. My dad contributes to that conversation like this: He has to stop that skateboarding business! I know where hes coming from. Its a conflict. Nicolas does not dig team sports. Nicolas is strong, creative and very, very coordinated, despite what these portraits evidence. And the thing is, is that he has an unbelievably high tolerance for pain. I love and hate that. Web-based information from state departments of education from all 50 states and from the District of Columbia were reviewed in the fall of 2004. Results indicated that only 16 states (31%) include a focus on student social behavior in their mission statements. Individual student behavior support practices were identified in the curriculum for general educators in 30 (59%) states and for special educators in 39 (76%) states. A current topic in paleontology that has received much popular press is the question of whether T.rex (or other Tyrannosauridae in general) were predators or scavengers. Lets explore this issue.. Paleontologist Jack Horner of the Museum of the Rockies (Bozeman, MT) has proposed that T.rex could not have been a predator. His arguments against predation include its small eyes (needed to see prey), small arms (needed to hold prey), huge legs (meaning slow speed) and that there is no evidence for predation bones have been found with tyrannosaur teeth embedded in them or scratched by them, but so far no study has shown that tyrannosaurs killed other dinosaurs for food (a bone showing tyrannosaur tooth marks that had healed would be strong evidence for predation)..
