In this study, we aim to compare the serological responses to HAV vaccination between HIV-infected patients who receive 2 doses or 3 doses of HAV vaccine and HIV-uninfected persons who receive 2 doses of HAV vaccine. Persons who identify themselves as male homosexuals aged younger than 40 years and are seronegative for hepatitis A virus will be enrolled. HAV vaccination will be provided free-of-charge. AIMS--To assess the reliability of saliva samples as a means of testing for HIV-antibodies outside clinic settings. METHODS--Men taking part in a non-clinic longitudinal study of homosexually active men provided samples of saliva and blood. Sera were screened using a competitive ELISA (Wellcozyme) and positive sera were confirmed by an indirect ELISA (Abbott). Saliva samples were screened either using an IgG captive radioimmunoassay or an amplified ELISA. RESULTS--A total of 534 paired saliva and blood samples were tested. Overall sensitivity was 96.2% and specificity was 100%. None of the saliva tests were falsely positive for HIV-1 antibodies. CONCLUSIONS--HIV-1 saliva tests can reliably be used in a non-clinic or field setting. This chapter analyzes Werner Fassbinders 1978 film of Vladimir Nabokovs 1936 novel Despair. In light of Nabokovs own border crossing as a Russian immigrant in Berlin, Fassbinder draws out the implications of the German setting in the writers time. The chapter argues that by focusing on the homosexual and Jewish themes of the novel in light of Fassbinders own homosexuality and experience as a citizen of a nation that had carried out the Holocaust just before his birth in 1945, the director creates a complex cultural map of sexuality, religious identity, and the mental illness that plagues the protagonist, Hermann. Fassbinder also develops Nabokovs device of the double: in the film, Hermann, by murdering his stand-in Felix as a symbolic suicide, allows him to experience a rebirth through a new identity, away from Germany and his financial, marital, and social problems.
