OBJECTIVES Disabled, homebound individuals tend to suffer both chronic pain and depression; however, low-income, homebound older adults have been underexposed in pain and depression research. We examined the extent of pain frequency, intensity, and interference; the relationship between pain and depressive symptoms; and prescription analgesic use and its association with use of antidepressant and anxiolytic medications among these older adults. METHOD The data came from the baseline assessment of 215 homebound individuals aged 50+ who were referred to a clinical trial of depression treatment. Bivariate and multivariate analyses were used to examine the research questions. RESULTS Almost 87% (n = 186) of the participants reported having had chronic pain in the preceding three months. Of the pain reporters, the mean frequency, intensity, and interference of pain were 8.65 ± 2.05, 7.71 ± 2.10, and 7.80 ± 2.82, respectively, on a 1-10 scale, and 61% were taking a prescription analgesic. Food sources of vitamin D become increasingly important as the ability of older adults to activate vitamin D produced from sun exposure declines. Household food supplies of homebound older adults receiving home-delivered meals contained modest amounts of vitamin D with key sources being milk, fish and shellfish, eggs and egg substitutes, ready-to-eat cereals, and spreads. To improve access to this nutrient, an economical adjunct to home-delivered meals programs is providing an additional meal consisting of vitamin D-fortified milk and ready-to-eat cereal, pouches of tuna or salmon and vitamin D-fortified juice, and/or vitamin D supplements. Improving vitamin D status could help ameliorate cognitive decline, falls, and skeletal strength and decrease institutionalization of older adults and health care costs.

Jessica is 17 years old and before January 2008 she was a normal active teenager on the school dance team. In June of that year, she was diagnosed with CFS, after a sudden onset beginning with Epstein Barr Virus mono on January 6, 2008. She is now being treated by a doctor in Dallas. Her primary symptoms include sore throats, headaches, extreme fatigue, lack of focus, and short-term memory difficulties. Her symptoms and the severity of them vary. Home health care is the fastest-growing expense in the Medicare program because of the aging population, the increasing prevalence of chronic disease and increasing hospital costs. Patients and families are choosing the option of home care more frequently. Medicares regulations are often considered the standard of care for all home health agency interactions, even when a patient does not have Medicare insurance. These regulations require patients who receive home health care services to be under the care of a physician and to be homebound. The patient must have a documented need for skilled nursing care or physical, occupational or speech therapy. The care must be part time (28 hours or less per week, eight hours or less per day) and occur at least every 60 days except in special cases. A detailed referral and specific care plan maximize the care to the patient and the reimbursement received by the physician.
