Introduction. The use of outpatient parenteral antimicrobial therapy (OPAT) as a treatment strategy with the aim of de-hospitalizing patients has been growing since its advent during the 1970s.1 OPAT has become a safe and standardized practice for patients presenting with various infections who require long-term parenteral antimicrobial therapy. International consensus guidelines have determined that OPAT can be performed in physicians offices, clinics, specialized infusion centers or in patients homes.2,3. Patients should be selected for this type of treatment by physicians who are familiar with the infectious conditions that will be treated. They should also be evaluated by nurses who have experience in implementing and managing long-term venous access, and by social workers who will decide whether patients present social, economic and cultural conditions that allow them to be safely treated this way. If patient evaluation and selection are performed adequately, OPAT is acknowledged to be ...
Our results suggest that, for most of the clinical conditions we studied, there are no major differences in patient assessed health outcomes between hospital at home and hospital care. This is consistent with the results of other randomised controlled trials.15-19 Our trial did not have the power to detect differences in morbidity or mortality, and the number of patients recruited with chronic obstructive airways disease was small. However, our results suggest that patients recovering from a knee replacement are not suitable for hospital at home care: nearly a third of these patients experienced complications associated with their knee replacement that prevented early discharge. More elderly medical patients allocated to hospital at home care had to receive secondary care during the three month follow up than did those allocated to hospital.. Although there were few differences in outcome, most patients preferred hospital at home care. It is not clear how these preferences were formed, and ...

article{48809aad-3beb-4aaa-8407-8ff19b1fb3b7, abstract = {OBJECTIVE: To explore the characteristics and outcome of patients after stroke admitted to early supported discharge (ESD) services, and to investigate changes over time. STUDY POPULATIONS: Patients admitted between June 1997 and September 1998 and participating in a follow-up study (n = 87) and all patients admitted in 2005-06 (n = 226). Background populations: All stroke cases in Malmö alive 3 months after stroke in June 1997 to September 1998 (n = 514) and 2005-06 (n = 1353). RESULTS: There were no differences in age, gender, proportion living alone or Katz Index distribution between the 2 study populations. The Katz Index improved between start and end of ESD (p < 0.001). Patients admitted to ESD services did not differ from the background populations with regard to gender or age, but were less often living alone in 2005-06 (p = 0.002). The mean duration of the ESD input was shorter in 2005-06 (p < 0.001). In 1997-98 the ...
Methods Data collection took place from Jan to May 2013. Ten wards (mix of medical and surgical) were visited on five occasions over that period. Information was collected from patients prescribed IV antibiotics for 48 hours or longer, and included basic demographic data, diagnosis, antibiotic prescribed and dose, and whether a 48 hour review had taken place. The data was reviewed by a virtual AMS team (Infectious Diseases (ID) consultant and ID pharmacist) and each patient underwent a 48 hour Antimicrobial Prescribing Decision, which was compared to the clinical decision. In addition the team assessed the eligibility for the patient for Outpatient Parenteral Antimicrobial Therapy (OPAT).. ...

ESA (European Society of Anaesthesiology). A systematic review and meta-analysis of hospital data worldwide, presented as this years Euroanaesthesia meeting in Stockholm, adds further evidence that patients admitted to hospital at weekends have higher mortality than those admitted on weekdays. The study is by Dr Hiroshi Hoshijima, Tohoku University, Sendai, Japan, and colleagues.. The analysis included 72 studies from various world regions, covering 55,053,719 participants. The authors found that weekend admission was associated with increased morality of between 15% and 17% depending on the statistical technique used.. Subgroup analysis revealed that patients admitted during the weekend were at a higher risk of death than weekday admission in patients in almost all categories. There are at least two potential explanations for our results. First, these differences reflect poorer quality of care in hospital at the weekend, and second, patients admitted on at weekend could be more severely ill ...

The purpose of this journal entry is to let everyone know my exact treatment plan as it stands today because I think some people are under the impression that Im in the hospital at Sloan-Kettering or Greenwich, etc. I wanted to make sure that you all know the plan for the months ahead. Right now I have started my first 8 week round of chemotherapy called FOLFOX. This is a standard, aggressive chemo regimen that is recommended to treat patients in Stage 4 to prevent the cancer from spreading anywhere else, most importantly for me the lungs and the lymphnodes which thankfully, it hasnt gone to yet. Also, this treatment helps to kill or, zap out as my doctors like to say, the microparticles of cancer that may be in my colon and liver that the doctors cant see. At the end of this first 8 week stint, I will have a CT scan that will be reviewed together by my oncologists at both Greenwich Hospital and Sloan Kettering, as well as my liver surgeon. At that point they will see if the tumors have ...

The third form of home-based rehabilitation is providing home programs for people with stroke, so they may at least maintain, if not augment, gains made during formal rehabilitation and to reduce cardiovascular risk profile. American Heart Association recommend28 that people with stroke perform aerobic exercises 3 to 7 days per week, as well as strengthening, flexibility, and neuromuscular exercise 2 to 3 days a week, for their life time. Clearly institutionalizing this ongoing intervention is not feasible, and home-based programs, if successful, would be a solution.. Olney et al29 tested this by comparing the outcomes achieved by 2 groups of people with chronic stroke, 1 with 10 weeks of supervised training (n=38) and 1 group with 1 week of supervised training to learn the program followed by 9 weeks of unsupervised training carried out at home (n=36). Both groups made equally modest gains on indicators of motor impairment (gait speed, ,6 minutes; muscle strength) and cardiovascular risk ...

The cybersecurity architecture at home does not have the same levels of security as in organizations, even more if the house has been developed under a smart infrastructure (smart home) scheme, the cybersecurity attack surface expands. This context could be one of the reasons why the number of attacks has increased significantly to more than 35%. VPN solutions are uses to protect telecommuting communications but security attacks exploit vulnerabilities of home-equipment and people, so VPN solutions could reduce their efficiency. Attackers ...

I was just really down one day and my brother-in-law said, Millie, dont stop. Weve got the internet now. You can find the best doctor in the world. And that is what I did.. Millie found Neurosurgeon, Dr. Michael Kaiser from the The Spine Hospital at the Neurological Institute of New York.. I read that he only does surgery as a last resort. Hell do anything else first that might work, she says. After I sent him an email, he actually emailed me back and asked for my phone number. He himself personally, called me, and that blew my mind. He was this big New York doctor that Id read all kinds of stuff about, that everybody in the world knows him, and he was calling me.. I arrived at the hospital at 9am on Friday and requested to see a doctor who hopefully could give me the results of the Ultrasound. Two and a half hours later I met a doctor who was debating whether he should take responsibility for my wife or whether another chest specialist (who my wife had seen as an out-patient some 9 months previously) should be involved. This doctor said if my wife came under his care he could tell me anything from this point onwards but could not comment on any matters arising before this time. That afternoon my wife had a CT chest and abdomen Scan and I was told the results would not be known for 24 hours. I telephoned the hospital at 12 noon the following Monday and was told the results of the CT Scan were still not known. Five minutes after that call my wife telephoned to say she could come home. I went to collect her and again enquired about the CT Scan results. When I was told they were not known I asked whether it was right for my wife to be discharged without ...

After having lunch with them in the hospital cafeteria we were just getting ready to leave and spend the rest of the day exploring the town...but then it happened again....the foot started to twitch. Yes, it was another seizure coming on. Luckily we were still in the hospital. So we hurried back upstairs in the childrens hospital where they took her back to a room and went to call the doctor. Dr Kurtzberg (the dr for the infusion) then called Elisabeths neurosurgeon to give him a heads up on what was happening. He gave her the go ahead to do whatever was needed. They got her on a medication and warned us that she would get extremely drowsy. So we finally left the hospital at around 7:00 p.m. A long day considering that we were originally only supposed to be there for around one hour in the morning. The next day we checked back into the hospital at 11:30 for the stem cell infusion. It took them about 1/2 hour just to get the IV set up. Poor Elisabeth....its not easy to find a vein on a newborn. ...

Cancer is an adventure and one part of that adventure for many cancer patients is surgery. Would you like a long, detailed update about my life since August 31st? Ok! Here we go.. Well, its been two weeks since I had surgery to remove half my liver (which, Im told is almost totally grown back already!) and my entire gall bladder. Forgive me for the delay, but between the anesthesia, the pain and the side effects of the pain medication, it would have been a bad idea. For example, a few days ago, I went to text a friend (Kelly Cooke) and realized that I had texted her in the middle of the night from the hospital These pain meds are awesome. Im not sure if its funnier that I wrote that or that she totally ignored it.. Surgery was…surgery. Ill give you some details because sometimes people get sent to my page because they have a similar diagnosis and they Google what to expect.. We arrived at the hospital at 4:45am. It is always interesting to see the groupings of people there for early ...
