Serum-derived bovine immunoglobulin/protein isolate (SBI), sold under the brand name EnteraGam among others, is a derived product from the blood of adult cows. It is primarily used to manage enteropathy in people whose ability to digest is impaired. It can also be used to manage chronic loose or frequent stools like those commonly associated with diarrhea-predominant irritable bowel syndrome (IBS-D), inflammatory bowel disease (IBD) or HIV-related enteropathy. Serum-derived bovine immunoglobulin/protein isolate is most often used in the fat loss prevention of post-weaning piglets. Due to dietary change post-weaning piglets often develop diarrhea and it has been shown to have some efficacy in preventing this fat loss related to diarrhea in this animal population. Oral immunoglobulins may improve function in the gastrointestinal (GI) tract. Conditions like HIV often limit or impair the bodys ability to digest select nutrients. Studies have indicated that serum-derived bovine ...

N2 - Human immunodeficiency virus (HIV)-infected patients often develop malabsorption and increased intestinal permeability with diarrhea, called HIV enteropathy, even without enteric opportunistic infections. HIV. gp120-induced calcium signaling, microtubule loss, and physiological changes resembling HIV enteropathy were previously found in the HT-29 intestinal cell line. How gp120 caused these changes was unclear. We show that the HIV co-receptor Bob/GPR15, unlike CCR5 and CXCR4, is abundant at the basal surface of small intestinal epithelium. The gp120-induced effects on HT-29 cells were inhibited by anti-Bob neutralizing antibodies, the selective G protein inhibitor pertussis ...

C. difficileis a bacterium known to cause colitis, with an estimated mortality rate of 6.5% in both non-severe and severe infections.9 Since 2003, CDI has increased in severity and has become more refractory to therapy.10 Historically, the treatment of CDI has been pharmacologically centered. However, nonantibiotic treatment for refractory, complicated cases have evolved and include fecal bacteriotherapy, probiotics, surgery, and monoclonal antibodies against C. difficile toxins.11,12. SBI is a newer nonantibiotic therapy. This plasma protein concentrate is approved by the U.S. Food and Drug Administration and is marketed as a medical food.13 Oral preparations of SBI are safe when administered in doses up to 20 g/d and are available by prescription.14 Routine follow-up is suggested to monitor for side effects. The most common complaints include mild nausea, constipation, stomach cramps, headache, and polyuria.3. In vitro, the mechanism of action of SBI occurs through immunoglobulin binding of ...
Method Fifty adults were recruited from a poor community in Lusaka where we have previously established that EE is ubiquitous, and then underwent CLE on the day after permeability (lactulose 5g, rhamnose 1g) and net zinc absorption from a 25mg test dose were measured. Between 35 and 225 CLE images per procedure were quantified for a range of cellular abnormalities and fluorescein leakage. Duodenal biopsies were assessed morphometrically and claudin-4 expression quantified. Of the 50 participants, one had pyloric stenosis and in 8 the image quality was sub-standard, so full data are available from 41 (14 men, mean age 32 y, 11 HIV seropositive). Watson scores were 1 in 4 people, 2 in 6, and 3 in 31, which contrasts markedly with published control data in healthy German adults in which nearly all were grade 1 (P , 0.0001). ...
Children in resource-poor settings are at risk of multiple episodes of diarrhea, enteric infections, and environmental enteropathy, an inflammatory disorder of the intestines that compromises nutrient absorption (1). In cross-sectional analyses, repeated episodes of diarrhea and chronic environmental enteropathy in early childhood are associated with reduced growth and cognitive function, and impaired school performance which can reduce income later in life (2-8). Although more evidence is needed to establish causal links, repeated episodes of childhood diarrhea and enteric infection may exact a long-run toll, perpetuating a cycle of poverty and ill health.. Infection and inadequate diet are proximate risk factors for undernutrition and early life growth faltering; the two processes likely act reciprocally in a vicious cycle that perpetuates physiologic and metabolic deficits and increases the risk of mortality. Children who exhibit growth faltering are more likely to have deficits in cognitive ...

In this IDS Working Paper, Robert Chambers (CLTS Knowledge Hub, IDS) and Gregor von Medeazza (UNICEF) argue for a more inclusive framework for thinking about and dealing with undernutrition. One concept is FTIs (faecally-transmitted infections). This is designed to avoid the reductionisms of faecal-oral infections, waterborne diseases, and the focus on the diarrhoeas to the neglect of less dramatic and less measurable FTIs especially environmental enteropathy. A second concept is the 5 As - availability and access which both have oral associations, and absorption, antibodies and allopath ...

Histological stains are useful tools for characterizing cell shape, arrangement and the material they are made from. Stains can be used individually or simultaneously to mark different cell structures or polymers within the same cells, and to visualize them in different colors. Histological stains can be combined with genetically-encoded fluorescent proteins, which are useful for understanding of plant development. To visualize suberin lamellae by fluorescent microscopy, we improved a histological staining procedure with the dyes Fluorol Yellow 088 and aniline blue. In the complex plant organs such as roots, suberin lamellae are deposited deep within the root on the endodermal cell wall. Our procedure yields reliable and detailed images that can be used to determine the suberin pattern in root cells. The main advantage of this protocol is its efficiency, the detailed visualization of suberin localization it generates in the root, and the possibility of returning to the confocal images to analyze and re
The aim of the study was to map out pain and pain treatment among patients in hospitals and persons in special accommodations as well as in ordinary living.. Method: Nurse students collected the data during their clinical education at hospitals and in special accommodations or in ordinary living. Also the clinical teachers helped out with supporting the students during data collection. The students made assessments of pain where they had their clinical practice during the spring 2009. The students got, though collecting data, a training in how to provide information to the staff and patients/residents, compile results and to give feed-back about the findings to the department. The study was not conducted as a total survey, in other words was a selection of respondents that were included in the results.. Results: In total 419 assessments of pain was made and 35% of respondents reported long standing pain and 14% reported acute pain. The respondents with long standing pain were older (76 years and ...

For those who work in urgent care, and the emergency room, how many times have patients come in, especially the older adult, and they are dehydrated? Often you find it hard to get an IV going, the veins collapse and roll. The person may not even remember when they took in the last fluids. Their skin tents when pinched, their lips are cracked and dry and they appear to have cotton mouth and poor oral hygiene. Dehydration can be a major issue with patients / residents in facilities due to inability to physically reach for the water pitcher (if even within reach), do not like water or even better, they do not want to drink because of having to go to the bathroom. Where is the nursing staff and the administration? Do they not see this problem?. During the review of a case presented to me for deviations in the Nursing Standard of Care, the dietary assessment is included or should be. If it has not been included, this is a part of the medical records which need to be requested during discovery. A ...

One of the major drawbacks of droplet sorting in a flow cytometer is the relatively low sorting speed. Thus, we have developed an alternative, faster sorting technique: photodamage cell sorting. In a photodamage cell sorter all unwanted cells, as detected with the first, measuring laser, are killed with the second, damaging laser. Thus, the cells need to be photosensitive to the second laser. In addition, a mechanism is needed to switch this laser on and off based on the sorting criteria. In our photodamage cell sorter, the ZAPPER, we use an acousto-optic crystal to switch the laser beam. Cells are made photosensitive by vital staining with photosensitizers. With cells grown in the presence of 5-bromo-2′-deoxyuridine (BrdUrd) and stained with Hoechst 33342 (H42) at least a 5-decade cell reduction is accomplished after irradiation with 400 mW UV light. With this system, sorting rates have been achieved of 30,000 cells per second. Due to the selection based on photodynamic killing, this sorting ...

Enteropathy refers to any pathology of the intestine. Although enteritis specifically refers to an inflammation of the intestine, and is thus a more specific term than enteropathy, the two phrases are sometimes used interchangeably. Specific types of enteropathy include: Enteropathy-associated T-cell lymphoma Environmental enteropathy An incompletely defined syndrome of inflammation related to the quality of the environment. Signs and symptoms include reduced absorptive capacity and reduced intestinal barrier function of the small intestine. It is widespread among children and adults in low- and middle-income countries. Eosinophilic enteropathy A condition in which eosinophils (a type of white blood cell) accumulate in the gastrointestinal tract and in the blood. Eosinophil build up in the gastrointestinal tract can result in polyp formation, tissue break down, inflammation, and ulcers. Gluten-sensitive enteropathy (which can progress to coeliac disease) Coeliac disease A malabsorption ...
Tropical sprue is largely limited to within about 30 degrees north and south of the equator. Recent travel to this region is a key factor in diagnosing this disease in residents of countries outside of that geographical region.[1]. Other conditions which can resemble tropical sprue need to be differentiated.[4] Coeliac disease (also known as coeliac sprue or gluten sensitive enteropathy), has similar symptoms to tropical sprue, with the flattening of the villi and small intestine inflammation and is caused by an autoimmune disorder in genetically susceptible individuals triggered by ingested gluten. Malabsorption can also be caused by protozoan infections, tuberculosis, HIV/AIDS, immunodeficiency, chronic pancreatitis and inflammatory bowel disease.[1] Environmental enteropathy is a less severe, subclinical condition similar to tropical sprue.[1]. ...

The optic tectum of lower vertebrates and the LGN of higher mammals are divided into multiple sublaminae that are readily distinguished by conventional histological stains and that are targeted by axons of specific RGC subtypes (Yamagata and Sanes, 1995; Yamagata et al., 2006; Xiao and Baier, 2007; Nassi and Callaway, 2009). In contrast, the retinorecipient zones of mouse superior colliculus and LGN are relatively homogeneous when viewed with conventional histological stains, and few distinctions have been documented among the RGCs that populate subdivisions within them (Sachs and Schneider, 1984; Hofbauer and Dräger, 1985; Grubb and Thompson, 2004). Transgenic lines in which RGC subsets are labeled have begun to reveal such distinctions (Huberman et al., 2008, 2009; Kim et al., 2008). Our results indicate that there are at least four distinguishable termination zones for retinal axons within the retinorecipient layer of the superior colliculus: a superficial sublamina occupied by W3-RGC axons, ...

The lipid fraction of latent fingermark residue encompasses a number of compounds crucial to fingermark detection on wetted porous substrates. This thesis describes investigations into the detection and chemical analysis of the lipid fraction of latent fingermarks. Two novel reagents for fingermark detection were developed from lipophilic histological stains. Chemical analysis of fingermark lipids found insufficient variation to discriminate between individuals or donor traits, however significant differences were observed as a function of sample age.. ...

Diarrhea and constipation are exceedingly common and, together, exact an enormous toll in terms of mortality, morbidity, social inconvenience, loss of work productivity, and consumption of medical resources. Worldwide, >1 billion individuals suffer one or more episodes of acute diarrhea each year. Among the 100 million persons affected annually by acute diarrhea in the United States, nearly half must restrict activities, 10% consult physicians, ~250,000 require hospitalization, and ~5000 die (primarily the elderly). The annual economic burden to society may exceed $20 billion. Acute infectious diarrhea remains one of the most common causes of mortality in developing countries, particularly among impoverished infants, accounting for 1.8 million deaths per year. Recurrent, acute diarrhea in children in tropical countries results in environmental enteropathy with long-term impacts on physical and intellectual development. ...
In 2015 there were 965 million people in the world forced to practise open defecation (OD). The adverse health effects of OD are many: acute effects include infectious intestinal diseases, including diarrheal diseases which are exacerbated by poor water supplies, sanitation and hygiene; adverse pregnancy outcomes; and life-threatening violence against women and girls. Chronic effects include soil-transmitted helminthiases, increased anaemia, giardiasis, environmental enteropathy and small-intestine bacterial overgrowth, and stunting and long-term impaired cognition. If OD elimination by 2030 is to be accelerated, then a clear understanding is needed of what prevents and what drives the transition from OD to using a latrine. Sanitation marketing, behaviour change communication, and enhanced community-led total sanitation (CLTS + ), supplemented by nudging, are the three most likely joint strategies to enable communities, both rural and periurban, to become completely OD-free and remain so. ...

NOTES: Celiac disease (celiac sprue) is one of the protein related autoimmune enteropathies. When the protein related problem is associated with increased intra-epithelial villus tip lymphocytes (VTLs), it is a protein related autoimmune disease. Increased duodenal and/or small bowel villus tip lymphocytes (sort of like the IFA test for ANA on HEp-2 cells) represents a global marker for small bowel impacting autoimmune disease. In the case of gluten, the disease is additionally associated with certain detectable serum autoantibodies. A protein intolerance is a gut problem related to dietary intake of a protein but without (1) presence of fixed-tissue increased villus tip lymphocytes or (2) presence of the required specifically diagnostic circulating serum autoantibody. An example would be non-celiac gluten sensitivity (NCGS14)...nonceliac gluten intolerance (NCGI15)...where the clinical abnormal symptoms are related to dietary intake of gluten containing products but there (1) is not an ...
OPINION STATEMENT: Patients under 40 years with other than high risk disease do not benefit from an allograft in CR1, particularly if they have received gemtuzumab ozogamicin in induction. High risk can be defined by cytogenetics (Complex; abnormalities of chromosome 5 and 7; 3q-; Ph(1)) in addition, adverse features could include presentation with high white cell count or secondary disease, or a failure to have a satisfactory blast cell reduction to induction course 1 (to |15% marrow blasts), an EVI-1 mutation, although the risk with that mutation is frequently primary resistance. While there is an increasing list of molecular characteristics that could indicate a higher risk, there is in general an absence of clear evidence that they are predictive of a benefit from transplant. This does not mean that they should not be transplanted, just that it is uncertain. Some of the newer markers like minimal residual disease fall into the same category. The decision to allograft is also influenced by the

Eosinophilic means loves eosin, and refers to the staining of certain tissues, cells, or organelles after they been washed with eosin, a dye. Eosin is an acidic dye, thus the structure being stained is basic. Eosinophilic describes the appearance of cells and structures seen in histological sections which take up the staining dye, eosin. This is a bright pink dye that stains the cytoplasm of cells as well as extracellular proteins such as collagen. Such eosinophilic structures are generally composed of protein. The stain eosin is usually combined with a stain called haematoxylin to produce a haematoxylin and eosin stained section (also called an H&E, HE or H+E section). This is the most widely used histological stain in medical diagnosis - for example when a pathologist looks at a biopsy of a suspected cancer they will have the section stained with H&E. Some structures seen inside cells are described as being eosinophilic, for example Lewy bodies, Mallory bodies. ...
Archival slides are a potentially useful source of DNA for mutation analyses in large population-based studies. However, it is unknown whether specimen age or histological stains alter the accuracy of Taq polymerase or induce secondary mutations in sample DNA. To address this question, we evaluated five methods for extraction of genomic DNA from archival bone marrow slides of 17 leukemia patients and analyzed exons 1 and 2 of the N- and K-ras genes for the presence of mutations. Of the five methods, optimal DNA purification was achieved by boiling and phenol:chloroform extraction. N-and K-ras exons 1 and 2 were independently amplified using 35 cycles of PCR, and 6-12 clones for each exon were isolated and individually sequenced for each patient. Mutations were confirmed by repeat extraction, cloning, and sequencing. Sixteen of 17 patient samples were successfully amplified (94%), including slides up to 29 years old. Twelve slides had been stained with Wright-Giemsa, I stained with toluidine ...

Intermittent courses of dexamethasone (DEX) were administered to 112 consecutive, previously untreated patients with multiple myeloma (MM). Using criteria based on a 75% or greater reduction of calculated tumor mass, the overall response rate was 43%. Among comparable patients, response rate were approximately 15% less than those observed previously with vincristine-doxorubicin by continuous infusion with intermittent DEX (VAD) and similar to those with melphalan-prednisone. The projected survival times with VAD or DEX were similar. Results indicated that DEX accounted for most of the plasma cell reduction achieved with VAD. Serious complications occurred in 27% of patients treated with VAD, but in only 4% of those who received DEX. In view of the similar outcome with fewer serious complications, DEX provided a simple, effective, and safe primary treatment for a large fraction of patients with MM. Patients who appear most likely to benefit include those with hypercalcemia or pancytopenia, or who ...

Dientamoeba fragilis is one of the most important diarrhea-causing protozoa and is distributed worldwide. Recent studies demonstrated the pathogenic potential and implicated it as a common cause of gastrointestinal disease. Infection with D. fragilis may be either symptomatic or asymptomatic. Symptoms of dientamoebiasis are abdominal pain and diarrhea. The prevalence of D. fragilis varies from 0.3 % to 52 % and often exceeds that of Giardia lamblia.. Classically, diagnosis of Dientamoeba fragilis is achieved by microscopial examination of faecal samples, which require experienced personal. RIDA®GENE Dientamoeba fragilis real-time PCR is a new and attractive alternative method for testing stool samples and has proven to be highly sensitive and specific for the detection of Dientamoeba fragilis.. ...
Background: Topical calcineurin inhibitors including tacrolimus and pimecrolimus are used in the treatment of many inflammatory skin diseases mainly via blocking T-cell proliferation. Our previous studies found that pimecrolimus 1% cream could reverse high-dose ultraviolet B (UVB) irradiation-induced epidermal Langerhans cell (LC) reduction via inhibition of LC migration. We conducted this study to investigate the effects of topical tacrolimus 0.03% ointment on high-dose UVB-irradiated human epidermal LCs.Methods: Twenty fresh human foreskin tissues were randomly divided into four groups as follows: Control, Tacrolimus (0.03%), UVB (180 mJ/cm2), and UVB (180 mJ/cm2) + Tacrolimus (0.03%). Four time points were set as follows: 0, 18, 24, and 48 h. We collected culture medium and tissues at each time point. The percentage of CD1a+ cells in the medium was detected by means of flow cytometry. Each tissue was prepared for immunohistochemistry, real-time quantitative PCR, and western blot. HaCaT cells were

Redd et al. (1) suggest that microbial translocation is not an important contributor to HIV disease progression in Africa, in contrast to its possible role in HIV pathogenesis in North American cohorts (2-4). They further postulate that this discrepancy may relate to mode of HIV transmission, because the Ugandan cohort in which they base their study is composed of heterosexual men and women, whereas North American cohorts typically contain more men who have sex with men and injection drug users. These conclusions are based on a study in which they prospectively examined whether plasma markers of microbial translocation correlated with HIV progression and found that they did not. However, we suggest that these data are not sufficient to conclude that microbial translocation is not an important contributor to HIV disease in Africa. First, significant levels of bacterial endotoxin [lipopolysaccharide (LPS)] were demonstrated in HIV infected subjects but were not directly compared with levels in ...

The catecholamines epinephrine and norepinephrine play critical roles in the maintenance of cardiovascular function. Phenylethanolamine-N-methyltransferase (Pnmt) catalyzes the conversion of norepinephrine to epinephrine and serves as a marker for adrenergic cells. We have previously shown that the selective destruction of Pnmt+ cells in the mouse produces severe left-ventricular dysfunction under anesthesia and that epinephrine deficiency alone does not recapitulate the phenotype. Here, we test the hypothesis that Pnmt+ cells are key modulators of the stress response to immobilization. Using a suicide reporter mouse model to ablate Pnmt+ cells (Pnmt-Cre/DTA), we achieve greater than 50% Pnmt+ cell reduction in the adrenal medulla and 97% reduction in Pnmt transcript. Remarkably, Pnmt+ cell destruction does not markedly diminish the cardiovascular response to restraint stress. At one hour of immobilization, heart rate and ejection fraction showed a similar increase in response to restraint in ...

During the early phases (phases 1 and 2), researchers assess safety, side effects, optimal dosages and risks/benefits. In the later phase (phase 3), researchers study whether the treatment works better than the current standard therapy. They also compare the safety of the new treatment with that of current treatments. Phase 3 trials include large numbers of people to make sure that the result is valid. There are also less common very early (phase 0) and later (phase 4) phases. Phase 0 trials are small trials that help researchers decide if a new agent should be tested in a phase 1 trial. Phase 4 trials look at long-term safety and effectiveness, after a new treatment has been approved and is on the market. ...

Findings: The patients in G1 group had 500 episodes of infections before diagnosis in 256 patient-years (0.08 per patient per year) and 203 times of hospitalization (0.03 per patient per year), which were significantly higher than in G2 patients, who had 75 episodes of infections (0.015 per patient per year) and 88 hospital admissions (0.018 per patient per year) during 207 patient follow-up years. Frequency of enteropathies and liver diseases in G1 were also significantly higher than in G2. Lack of awareness about nature of disease, especially among rural and suburban physicians, single organ involvement as a site of clinical presenting, and predomination of non infectious presentation in G1 were the major factors of delayed diagnosis ...
